EE was cutting a zip tie with a utility knife and cut EE's hand resulting in receiving stitches. Returned to work same day.

A miner was cutting a piece of skirt board rubber when the belt knife slipped causing them to cut through their jeans into their thigh. The cut was not bleeding but clinic placed two stiches on the cut to prevent it from opening up.

EE was operating a skid steer and noticed the attachment wasn't secured. EE stepped out of the machine to secure the attachment and EE's finger came across a metal burr on the attachment cutting EE's finger resulting in stitches.

While trimming a piece of skirt board rubber, the knife slipped on the surface, piercing left pinky finger, causing a laceration.

Contact with hot objects or substances

A miner was attempting to light the natural gas burner at the Fine Grind Plant when the burner lit with higher intensity than in the past which EE was not expecting. That caused burns on right arm and right hand. EE was transported to the hospital with non life threatening burn injuries. EE was wearing a hooded sweatshirt and gloves at the time of the accident/injury.

While climbing into the CAT truck, something popped in the back of EE's left calf. EE was unable to straighten leg completely or walk more than a few steps unassisted. EE was seen at an occ health clinic for a muscle strain, who recommended PT, which EE has started.

Over-exertion in lifting objects

While loading the railcar, EE went to raise the access platform so the railcar door could clear the platform. EE grabbed the platform with right hand and pulled up to lift it. EE felt a pop in right elbow. Diagnosed with tennis elbow.

Attempting to take sample from discharge chute. EE believes shovel contacted moving belt under chute, causing it to jump. EE's RT pinky finger came into contact with the conveyor frame. The impact caused the nail to catch on the frame and pull nail from nail bed, the nail lacerating the fingertip.

Fall down stairs

Loading customer truck. The air line to the gate valve froze due to subzero temps. EE attempted to access platform, carrying a weed burner to thaw air lines. EE's hand slipped off the rung, EE fell backwards, left arm struck handrail. EE then fell down the set of steps.

Struck against a moving object

Using a loader to move an oversized rock. As he raised bucket & started forward, rear tires of loader came off ground, causing rock to fall out of bucket. Loader rocked back/forth, causing employee to bounce around, developing back pain, he was wearing seat belt.

Struck against stationary object

Using a pry bar to get a cable unstuck on a loading spout for the south mineral filler bin. The cable slipped, causing pry bar to come off spout, his RT hand came in contact with an angle iron, across the top of 5th finger & knuckle. Fracture to right 5th finger. Splinting. Light duty restrictions.

Employee was descending stairs, carrying a small piece of steel in his right hand & using his left hand as guide on handrail. He misjudged a step and slid. His left hand slid on handrail, his left 4th & 5th fingers split between the handrail support, pulling his 5th finger backwards. He went to occ health & was given a splint. No fracture.

EE was operating track mobile to position onto tracks to load railcars. As he was positioning the track mobile to go back on rail, left rear tire rim failed. A piece of the rim swung up against side of the cab, striking cab & employee. The side glass was open and employee leaned out to line up wheels when rim failed causing employee to be struck in the face. Trauma/contusions.

Fall from ladders

He was cutting a piece of truss of stacking conveyor when final cut was made conveyor broke loose & moved. EE was standing on 3rd rung of ladder conveyor did not contact ee or ladder but he jumped off ladder & fell to ground landing on left arm.

Employee had brought a deer skull to work and was cutting off the hide and the knife slipped and cut his hand - required stitches.

EE believed airlock was plugged. He did not de-energize machine. Did not use lock out procudure. EE remved a plate from the chute and stuck his hand into the discharge chute. His hand contracted the energized airlock and finger (3) were cut off.

EE was sliding skirt board plates into hopper. He twisted as he attempted to push the skirt board to a co-worker on other side of opening.
