An EE was jack hammering the Debris when it broke free. IE's hand was on the stack wall when the debris impacted EE's fingers

Installing a monkey face with a hilti drill and pinched EE's finger between the roof and the monkey face causing a laceration that required sutures to close.

A mechanic's hand was injured by a fan sheave and pinched by the V-belt mechanism resulting in a laceration and a fracture of the hand.

While grinding 6 inch angle iron to fit to an I-beam, an employee was on a step ladder with a foot on the I-beam. While losing his balance, attempting to grab something so he wouldn't fall, the grinder slipped hitting the back of his right hand causing 2 lacerations. He received 4 sutures and returned to work.

Using a jack hammer to break up rock when it slipped smashing his hand in between the hammer and the end of the pan line.

Cleaning out the inside of a bearing housing with a grinder, a foreign object went under the face shield and into his left eye.

While operating roof bolter #15, the employee experienced intermittent problems with the centralizers closing on their own. During the troubleshooting process the electricians asked the employee to move the boom of the roof bolter. While moving the boom a drill steel broke free striking the employee in the forearm.

A Roof Bolter Operator had a drill steel stuck in the drill chuck and was attempting to pull it out with a chain that was tied off to the centralizer arms. Once the drill steel was in rotating motion the chain spun loose and struck the employee in the face and nose.

EE sent his co-worker downstairs and cont'd. to vacume alone. The vacume hose came loose and the vacume suction pulled EE's arm into the flex hose.EE was unable to pull the vacume breaker , his co-worker saw what was happening and pulled the vacume breaker. EE has compartment syndrome to left arm.

An employee and was reaching with his right hand to clear a rock that was located in the bretby tray. It appears that a rock was propelled from the head gate drum striking the employee in the left hand and chest. The employee was diagnosed with having sustained a severe strain to the wrist and lacerations to the thumb. On January 3rd, he was placed on restricted duty for 10 days.

Two employees were securing a head roller frame to the mine roof. One of was operating a roof bolter& the other ee was holding the glue resin in the bolt hole. The ee raised the bolt through frame & the bolt struck the other ee's finger between the frame & roof. He received 17 sutures to close the laceration on his left index finger.

An employee sustained injuries to both his right and left hands while operating a Fletcher Roof Bolter placing bolt in a corner section. The drill steel became stuck and seperated from the extension chuck. The employee then grasped the drill steel with his right hand and began pulling it back into alignment with the chuck extension. His glove became entangled in the steel.

The employee was using a hydraulic metal shear to cut strips of stainless steel metal. He reached around the back side of the shear to grab the metal to slide it into position when the actuating lever to lower the shear was activated. His right thumb was underneath the cutting edge resulting in the thumb being amputated just below the first knuckle.

The employee was lowering the boom to remove a bolting chuck that had been left in the bolter. As he did the suction hose caught the end of a bolting steel which was left on the canopy causing the steel to flip up and strike the employee. He sustained a laceration to his nose and upper lip. The employee was treated with sutures and given a full work release.

An employee was operating a roof bolter when he felt something enter his right eye. The employee flushed his eye and later his eye become irritated & he sought medical attention. He had a piece of trona or shale located in the upper eye lid which was removed. The employee was given a prescription as a precaution to treat infection, thus constituting medical treatment.

A longwall production employee sustained a fracture to his left tibia and right ankle. He was standing on #12 support and the shield was being pulled forward. As the shield pulled forward the employees lower legs were pinched between a rock which was on the floor and a base-lift cylinder. He is awaiting an ortho appointment.

During the operation of the shearer, a rock came out from the back of the drum and struck the operator in the left elbow causing a laceration. The employee was taken to the emergency room where he received three stitches to close the wound. The employee received a full work release and reported to his next scheduled work shift.

Employee has been noticing sharp pains in his right shoulder, back and chest muscles while roof bolting. The pain does not seem to go away. He went to seek medical attention on 7/24/06 and received two week restricted duty to allow the strain to rest and heal.

Employee was task training a new miner on a Joy Roofbolter and after doing so he stepped away and let the new miner work the machine. He stepped away (approx. 20') to bar down some roof when the extention chuck on the bolter flew off striking him in the mouth (lower lip) later requiring 4 stitches.

While employee was roof bolting, he used the lever to raise the mast. The rotating assembly turned when rising and the employee's hand got wrapped around the roof bolt causing damage to the thumb and fingers. He was taken to seek medical attention and put on restricted duty until further examination from the doctors.

While bolting in panel, the employee started to feel pain in his right shoulder. He is scheduled to seek medical attention on 10/19/05. He had surgery on 12/15/05.

Employee was standing at the head gate of the Longwall when a rock flew out of the crusher striking the employee in the right ear. He was taken to seek medical attention and received 6 stitches and one day restricted duty.

Employee was cutting down a rotor shaft tube, the cutting tool got hung up on a bolt hole and pulled the tube out of the chuck. The tube struck the employee in the chest and arm; he had abrasion on his chest and felt numb. He was taken to seek medical attention and has not been released back to work.

While employee was blowing off SN -1 and SN-2 with an air hose, a guard struck her right index finger causing a laceration. She was taken in to seek medical attention and received stitches. She has been put on Restricted Duty.

Employee was raising the floor on a Fletcher roof bolter when he got the tip of his thumb caught in a gate. The gate severed the tip of the thumb off. He was taken to the emergency room and flown to Salt Lake City to try and reattach the finger. They were unable to reattach it.

EE DROPPED THE BOOM SO HE COULD GET GAS HOLE STEELS OUT. WHEN THE BOOM DROPPED, IT SWUNG TOWARD EE AND CAUGHT HIS HAND BETWEEN THE BOOM AND THE STEELS. THE EE WAS TRANSPORTED TO THE EMERGENCY ROOM AND RECEIVED THREE STITCHES.

EE WAS BOLTING A LEFT ARCH WAY NEAR A PIE, WHEN THE BOOM CAME DOWN THE PIE AND SOME OF THE ARCH WAY CAME DOWN AND STRUCK THE EE ON THE KNOCKING THE EE DOWN RESULTING IN A CONCUSSION. THE EE WA S PUT ON LIGHT DUTY.

EMPLOYEE WAS OPERATING THE SHEAR ON THE LONG WALL. EE EITHER GOT DUST DIRECTLY IN HIS EYE OR INTRODUCED DUST INTO HIS EYE WITH HIS HAND/ FINGER.EE WAS FOLLOWING PROCEDURES AT THE TIME AND WAS WEARING ALL APPLICABLE PPE I.E. SAFETY GLASSES WITH SIDE SHIELDS.

EE WAS USING A BLACK & DECKER GRINDER WITH A 4 1/2 INCH GRINDING WHEEL ATTACHED. HE HAD THE GRINDER IN HIS LEFT HAND AND WAS PUTTING HIS GLOVE ON HIS RIGHT HAND. HE TOUCHED HIS FOREARM WITH THE GRINDING WHEEL WHICH RESULTED IN A LACERATION THAT REQUIRED SUTURES.

EE WAS ASSISTING ANOTHER EE ROOFBOLT, WHEN HE FFELT SOMETHING ENTER HIS EYE. THE NEXT DAY HE SOUGHT OUT MEDICAL TREATMENT AND FOREIGN OBJECT WAS REMOVED. BECAUSE HE HAD WAITED SO LONG TO SEEK MEDICAL TREATMENT THE FOREIGN OBJECT WAS OVERGROWN AND HE WAS PLACED ON RESTRICTED DUTY FOR ONE DAY.

EMPLOYEE'S ARM WAS CAUGHT IN THE PALLETIZING MACHINE RESULTING IN A CONTUSION AND LACERATION TO HIS FOREARM THAT REQUIRED SUTURES. HE WAS PLACED ON RESTRICTED DUTY FOR 2 DAYS.

EE WAS OPERATING A ROOF BOLTER WHEN THE CHUTE BECAME PLUGGED. WHILE UNPLUGGING THE SUCTION, THE DRILL STEEL FELL FROM THE ROOF (HE HAD LEFT IT IN THE ROOF), STRIKING HIS RT. MIDDLE FINGER. THIS RESULTED IN A FRACTURE & A LACERATION THAT REQUIRED SUTURES.

EE WAS ROOF BOLTING WHEN A SECTION OF ROOF FELL STRIKING EE. HE SUSTAINED NECK INJURIES AND WAS TRANSPORTED TO THE HOSPITAL. MSHA LOCAL OFFICE WAS NOTIFIED.

EMPLOYEE WAS STRUCK BY A ROCK THAT FELL FROM THE ROOF, RESULTING IN A LACERATION ON HIS HEAD THAT REQUIRED SUTURES.

EE WAS OPERATING A JOY ROOFBOLTER. HIS RT. INDEXFINGER WAS CAUGHT IN THE CENTRALLIZER, HE RECEIVED A CRUSHING INJURY/LACERATION TO THE TIP OF HIS FINGER. NO FRACTURE.

EMPLOYEE WAS GRINDING WHEN HE FELT SOMETHING IN HIS EYE. HE WAS TAKEN TO SEEK MEDICAL ATTENTION. IT WAS DETERMINED THAT THERE WAS NOTHING IN HIS EYE BUT HE DID HAVE AN ABRAISON WHICH WAS TREAT ED WITH ANTIBIOTICS.

EE WAS ROOFBOLTING. A ROCK FELL AND HE JUMPED OUT OF THE WAY. WHEN HE JUMPED OUT OF THE WAY HE RECEIVED A LACERATION FROM THE ROOF BOLT PAN THAT WAS IN HIS HAND.

WHILE WELDING AND CUTTING, EE FELT AN IRRITATION IN HIS EYE. ON 10-1 IT WAS STILL PAINFUL AND RED. HE SOUGHT MEDICAL TREATMENT, WAS PRESCRIBED AND ANTIBIOTIC AND RELEASED TO FULL DUTY.

HOLDING ONTO A PIECE OF 16" PIPE THAT WAS BEING UNLOADED. THE BOOM TRUCK CABLE DROPPED, JERKING HIS LEFT ARM AND LEFT SHOULDER.

WHILE GRINDING, EMPLOYEE CUT HIS FINGER ON GRINDING WHEEL.

EE WAS USING A DRILL. THE DRILL BIT BECAME STUCK AND THE HANDLE OF THE DRILL TWISTED RESULTING IN EE'S HAND BEING FRACTURED.

EE WAS STRUCK BY A ROCK ON HIS HARD HAT. THE BAND OF HIS HARD HAT CUT HIS HEAD AND HE RECEIVED SUTURES.

EE WAS CUTTING AN I BEAM WHEN IT STARTED TO FALL. WHEN HE TRIED TO CATCH IT HE FELT PAIN IN HIS SHOULDER. HE SOUGHT MEDICAL TREAMTNE ON 4-9-01 AND WAS PLACED A RESTRICTED DUTY PENDING FURTHER EVALUATION.

SOS TEMPORARY WAS USING A DRILL TO INSERT A SCREW. HIS GLOVE WAS CAUGHT IN THE DRILL TWISTING HIS THUMB & RESULTING IN A SLIGHT FRACTURE

A ROCK FELL FROM THE ROOF AND STRUCK EE ON THE HAND. HE WAS TEARTED AND RELEASED. SUBSEQUENTLY, HIS HAND WAS SWOLLEN AND SOUGHT ADDITIONAL MEDICAL TREATMENT AND WAS DIAGNOSED WITH A FRACTURE O N 11/1/00.

EMPLOYEE WAS BOLTING. AFTER RUNNING THE BOLT IN, HE HELD OUT HIS HAND TO CATCH THE STEEL AND HIS HAND/WRIST WAS STRUCK BY THE DRILL STEEL. HE WAS PLACED ON RESTRICTED DUTY.

EMPLOYEE SMASHED HIS THUMB RESULTING IN A FRACTURE TO THE TIP.

DEBRIS FELL FROM OVERHEAD AND GOT INTO EMPLOYEE'S LEFT EYE.
