Employee was putting a new bolter steel on and pinched EE's left finger.

While repairing a broken bracket on air door, employee was using a grinder and welder. At some point in the evening the employee thought EE had some dust in EE's right eye and reported it to EE's supervisor, who took EE into the doctor. The doctor at ER removed foreign object.

While tramming the mucker (L-35) the brakes set up. This caused his head to slam into front window causing his glasses to push into his eyebrow. It cut his eyebrow and required 4 stitches.

Miner removing a reamer bit from the cubex; had his hand on the bit reached up to the controls to release the bit. Pulled the wrong lever and the brakeout tool slid forward, instead of the bit being released.

MINER WAS DRIVING A BEAM HOLE RAISE, ON A STEEL LADDER AND WORKING FROM A 2' X 2' DRILL DECK. AS JACKLEG CAME DOWN FROM A HOLE IT CAME INTO CONTACT WITH LEFT INDEX FINGER ON THE LADDER RUNG.

EE WAS ROCKBOLTING AND A PIECE OF MATERIAL GOT IN HIS EYE.

WORKER WAS BOLTING IN THE PRIMARY SUMP WITH THE JACKLEG DRILL. PUT A 6' SPLIT BOLT W/MONSTER MAT BY HAND. BEFORE WORKER COULD GET UNDER IT WITH JACKLEG, BOLT FELL OUT AND MONSTER MAT PLATE CUT THROUGH HIS BOOT AND INTO HIS SHIN.
