Showing all 6Maintenance Tech was completing work order to install safety chain on railing when cutting a chain with a grinder. Grinding wheel began to fail and pushed the tool into the workers ungloved thumb.
Employee was using a hand grinder to cut off the inner race of a bearing that had seized on a fan shaft. The grinder kicked back and cut the left hand that was resting on the fan shaft. The employee's hand was cut just below the thumb knuckle, cutting one tendon and nicking another tendon.
While operating a dozer the track slid on a boulder causing the operator to hit the left side window of the cab.
