Operator was diagnosed with post-concussion syndrome from impacting EE's head against loader windshield

Contact with heat

Employee developed neck cramps during shift and was able to complete scheduled shift. Employee was evaluated for a fit for duty and given prescription muscle relaxers for dehydration and fatigue cramps. Employee was cleared for duty with no restrictions. Determined to be an occupational illness due to dehydration and lack of sleep.

Maint tech was lifting a piece of mechanical equipment into a Silo bottom when the maint tech slipped and strained upper back.

Maintenance employee was conducting routine work of rotating cutting edge on Cat 966K Loader when worn carriage bolt spun and caused a laceration on the left middle finger of the employee. Standard leather gloves were being worn.

Employee was changing a dual windshield wiper. EE was loosening the wiper arm from the broken stud. The wiper arm swung back and hit the employee right ring finger.

Struck by falling object

Employee was in the process of lifting a magnet up a couple flights of stairs. While setting up lifting device employee anchored a chain around a section of hand railing and tied a chain hoist to it and began to pull the chain on the hoist to bring the hook down to hook on the magnet when the hand railing broke falling to the ground with something contacting forehead.

Miner was running an auotmated packing system while running the system the bag count got off and messed the bag count up while it was stacking bags on the pallet. The operator stopped the machine and started to re-stack the bags and felt a strain in lower back.

Employee was getting off a creeper in the Mobile Maintenance Shop when EE twisted EE's right knee and tore EE's meniscus.

While picking waste rock out of State 1 ore the employee's back got sore. Began to feel a pinching burning sensation in lower back and buttocks, leg area on the second day of picking rock.

Employee was using an air lance and pry bar to break material free in the impactor chute. The material broke free and employee hit elbow on the top of the chute door causing bursitis on the elbow.

Employee was using a hand grinder to cut off the inner race of a bearing that had seized on a fan shaft. The grinder kicked back and cut the left hand that was resting on the fan shaft. The employee's hand was cut just below the thumb knuckle, cutting one tendon and nicking another tendon.

While replacing shrink wrap roll on stretch wrapper Machine worker swung the roll up and hit a support beam causing a contusion and laceration to his middle finger on his left hand.

Employee deenergized air and power to the packaging equipment. He was retensioning the chain that runs the destacking tray. As he was taking tension off the chain he saw movement in the corner of his eye. He reacted by pulling his arm out of the machine. When he did so his left elbow scraped across the sharp edge of a metal tray.

2 employees were attempting to close a roll-up door. One employee was pulling on the door and a 2nd employee was waking to the other end. As the 1st employee added more tension, the door fell to the floor. The 1st employee tried to get out the the way but the door scraped his back and he fell to the floor with his lower legs pinned under the door.

When stepping backwards off the Bag Unit table onto the landing, employee slipped and fell approximately one foot onto floor. While trying to break his fall he fractured his right thumb.

TANK AT MINE PROPERTY WAS TO BE FILLED WITH CAUSTIC SODA FROM TOTAL TANK.. MURIATIC ACID WAS PLACED INTO TANK INSTEAD OF CAUSTIC SODA. TANK HAD BEEN OPENED PREVIOUSLY TO CHECK PRODUCT LEVEL. CHEMICAL REACTION OCCURRED DUE TO MIXING OF PRODUCTS

AFTER WORK AND THE EE HAD CLOCKED OUT FOR THE DAY THE EE HAD STARTED WORKING ON A PERSONAL PROJECT IN THE SHOP. THE EE STEPPED ON A CRACK IN THE CONCRETE FLOOR HIS KNEE BUCKLED AND HE FELL HIT TING HIS KNEE AND FRACTURE HIS KNEE-CAP.
