While swapping ends of the steel, the Breakout Wrench was attached to the steel and power applied to it when the wrench failed and an undetermined projectile struck an employee on the chin standing approximately ten feet away causing a small cut that required 4 stitches.

While operating a power drill a contractor broke a bone in his right hand. The drill caught metal causing the drill to twist out of his grip. The drill immediately swung around struck the back portion of his right hand fracturing it.

EE WAS USING A 3/4" IMPACK WRENCH WITH A SWIVEL ON IT. THE PIN THAT HELD THE SWIVEL CAME OUT & CAUGHT BETWEEN THE MIDDLE & RING FINGER ON LEFT HAND. THE SKIN WAS MANGLED BETWEEN THE FINGER BUT NO MUSCLES OR TENDONS WERE INVOLVED

EE WAS CUTTING OFF ONE SIDE OF A WALKWAY-THE WALKWAY WAS BEING REMOVED-AT THE CRUSHER WITH A TORCH. WHEN HE CUT THROUGH ONE SIDE, THE WELD ON THE OTHER SIDE BROKE. DROPPING THE EE APPROX. 6 FT ON TO THE ROCKS AND GROUND BELOW. EE LANDED ON HIS BACKSIDE. HE FELT LIKE HE PULLED SOMETHING IN HIS NECK.
