Employee was inside an excavator breaking rock up. A rock slammed the arm of machine down and jarred the machine, causing mid to lower back pain. Diagnosis: Trapezius strain.
EE was using hand held grinding wheel, when 4" grinding wheel broke aprart and struck left knee, causing laceration. A laceration resulted, requiring metal slivers and sutures.
Employee was cleaning sand buildup under the scalper and caught his shovel on a roller. The jerking caused a pinched nerve to his right shoulder. Two visits of physical therapy have been ordered. Restricted duty until 8/30/05.
EMPLOYEE WAS WELDING AND A PIECE OF SLAG GOT CAUGHT BETWEEN HIS WELDING HOOD AND INTO HIS LEFT EYE. THE EYE DOCTOR TREATED WITH ANTIBIOTIC FOR CORNEAL ABRASION.
EMPLOYEE HAS JUST STARTED RUNNING A BULLDOZER FOR THE FIRST TIME. HE SAYS THAT THE JOLTING OF THE DOZER HURTS HIS BACK. HE WAS DIAGNOSED WITH LUMBER STRAIN.
EMPLOYEE WAS WELDING ON A BIN OUTSIDE THE SHOP WITH OTHER WELDERS AROUND HIM AND HE EXPERIENCED FLASH BURN LATER THAT NIGHT.
EMPLOYEE WAS OPERATING A UNILOADER AROUND THE PLANT. THE HYDRAULIC HOSE BROKE AND SOME HYDRAULIC FLUID DRIPPED INTO HIS EYES AROUND HIS SAFETY GLASSES. THE CLINIC IRRIGATED HIS EYE.
EMPLOYEE WAS WELDING ON A CHUTE AND A PIECE OF SLAG BURNED HIS STOMACH.
