EE using pneumatic torque gun. Positioned tool on lug in the 6 o'clock position. EE is right hand dominant & was holding grip with right hand & used left hand to set socket on lug. Then placed left hand under barrel of gun. Squeezed trigger with right hand before moving left hand off of barrel. Resulted in distal end of left index finger amputation.

Showing all 34IW was working on cleaning soft lime from under belt using vacuum hose. Vacuum hose pulled IW and IW felt pop from upper back. IW was initially diagnosed with thoracic strain on 8/1/23. Upon follow up on 8/17/23, IW was diagnosed and treated for rib subluxation.
Employee was talking to the employees working at Sacker #1 at the Pool Warehouse. As the lift table lowered, the employee's foot was caught below the motor housing, crushing the employee's foot. Employee was diagnosed with a broken bone in right foot.
Employee strained lower back while performing a sample size distribution analysis with the sample shaker.
When doing welding work, a spark from the torch came into contact with employee's watch, thus heating the watch up. From the hot watch surface a burn developed on employee's left wrist
The employee was tack welding two pieces of metal with a wire welder. The employee had a lapse in concentration and stabbed himself in the fire with the hot wire.
Employee was attempting to take off bolt from allen wrench attached to a 1/2" IR air impact wrench. He was holding the impact wrench with right hand and his left hand was pulling on the bolt. His right hand slipped and made contact with trigger on impact wrench, causing the bolt to rotate in his left hand, causing lacerations to his left hand palm area.
Employee was using a jackhammer to clean scale from a kiln, got dust in his eyes resulting in burning.

Jackhammer steel lock failed, allowing the steel to come out of the hammer noze, pinching operators finger between hammer noze and preheater wall.
Employee stated that he pulled the disconnect on 219 dust collector and proceeded to remove the guard. The pulley had not completely stopped when the guard contacted the pulley and pulled his right hand into the moving pulley.
Drilling hole in PVC pipe. The drill caught the metal fram & swung around & cut his right pinky finger.

Employee was climbing out from underneath roof on #2 preheater jack hammer in right hand and rolled left.
HE WAS CLEANING OUT LIME FROM THE CHUTE TO THE MIXER BOX WITH A STEEL BAR. THE MIXER WAS RUNNING TO KEEP THE PUMP FROM PLUGGING. HIS ATTENTION WAS DISTRACTED AND HE TURNED TO SEE WHAT WAS HAPPENING BEHIND HIM. AS HE TURNED HE LIFTED THE STEEL BAR IN HIS HANDS UP, WHICH PUT THE OTHER END IN THE MIXER BOX. THE MIXER PULLED THE BAR OUT OF HIS HAND AND SLAPPED IT CAUSING INJURY
HE WAS PRESSURE WASHING THE YARD TRUCK. HE WAS USING ACID. AFTER WASHING TRUCK HIS EYE WAS BURNING. AFTER GOING TO DOCTOR FOUND HE HAD A PIECE OF METAL IN HIS EYE.
EE was removing an ash ring from inside kiln #3 using an air-hammer. The ash ring collapsed striking EE's right hand, both thighs, right knee & right hip. EE was inexperienced on this job and was not instructed about the dangers of this job by his supervisor.
EE WAS ATTEMPTING TO CLEAN AIRLOCK WITH DISCONNECT ENGAGED AND NOT LOCKED OUT. AMPUTATED MIDDLE FINGER AND INDEX FINGER ON RIGHT HAND - BOTH AT CENTER OF MIDDLE PHALANGE.
EMPLOYEE WAS CLEANING PUG MILL WITH AIR LANCE. AIR LANCE GOT CAUGHT IN PUG MILL, FORCING LANCE AGAINST BERM. MIDDLE LEFT FINGER WAS CRUSHED BETWEEN LANCE AND BERM.
EE WAS THE OPERATOR ON THE HYDRATOR, LIME HAD COLLECTED ON THE WATER RING ABOVE THE MIXER. EE TOOK A STEEL ROD 1/4 IN DIA X 15" LONG TO KNOCK THE LIME INTO THE MIXER. THE ROD HOOKED ON THE MIX ER PULLING HIS HAND INTO THE METAL BOX AND THE ROD CUT OFF HIS RIGHT INDEX FINGER AND BROKE BOTH BONES IN HIS WRIST. THEY HAD PUT IN METAL PLATE IN THE WRIST.
WHILE GRINDING SAMPLES, A PIECE OF LIME FLEW UP, THEN CAUGHT IN EE'S LEFT EYE.
THE EMPLOYEE WAS UNPLUGGING A HYDRATOR MIXER BOX. HE WENT TO PULL UP ON THE BY-PASS VALVE (HANDLE) WHEN THE HANDLE CAME OFF AND STRUCK THE EMPLOYEE IN THE FOREHEAD.
EE WAS GRINDING A BRACKET HE HAD WELDED WHEN A PIECE OF SLAG WENT UNDER HIS SAFTY GLASSES, CAUSING PAIN IN HIS LEFT EYE.
EE WAS BACKING UP DRILL. DRILL WOULDN'T GO. EE TURNED RIGHT, DRILL SLIDE ON ROCK AND OVERTURNED.
WHILE OPERATING EXCAVATOR W/ROCK BUCKET, A DUMP TRK OWNED BY H20 ENVIRONMENTAL WAS BACKING UO NEXT TO THE EXCAVATOR THE TAILGATE SWUNG OPEN ONTHE DUMP TRK PINNED EE'S LEG INSIDE THE CAB OF T HE EXCAVATOR. DURING THE CONTINUING BACK OF THE DUMP TRK EE'S LEG RECD A BIG GASH/WOUND HE WAS TAKEN TO MH VIEW HOSPITAL
