EE using pneumatic torque gun. Positioned tool on lug in the 6 o'clock position. EE is right hand dominant & was holding grip with right hand & used left hand to set socket on lug. Then placed left hand under barrel of gun. Squeezed trigger with right hand before moving left hand off of barrel. Resulted in distal end of left index finger amputation.

IW was working on cleaning soft lime from under belt using vacuum hose. Vacuum hose pulled IW and IW felt pop from upper back. IW was initially diagnosed with thoracic strain on 8/1/23. Upon follow up on 8/17/23, IW was diagnosed and treated for rib subluxation.

Employee was talking to the employees working at Sacker #1 at the Pool Warehouse. As the lift table lowered, the employee's foot was caught below the motor housing, crushing the employee's foot. Employee was diagnosed with a broken bone in right foot.

When doing welding work, a spark from the torch came into contact with employee's watch, thus heating the watch up. From the hot watch surface a burn developed on employee's left wrist

The employee was tack welding two pieces of metal with a wire welder. The employee had a lapse in concentration and stabbed himself in the fire with the hot wire.

Employee was attempting to take off bolt from allen wrench attached to a 1/2" IR air impact wrench. He was holding the impact wrench with right hand and his left hand was pulling on the bolt. His right hand slipped and made contact with trigger on impact wrench, causing the bolt to rotate in his left hand, causing lacerations to his left hand palm area.

Employee stated that he pulled the disconnect on 219 dust collector and proceeded to remove the guard. The pulley had not completely stopped when the guard contacted the pulley and pulled his right hand into the moving pulley.

EE was using hydraulic bearing puller to pull a bearing off of a shaft, when puller fingers slipped and striking his left pinky finger, resulting in a fracture to the bone at the knuckle on his left hand.

HIS ATTENTION WAS DISTRACTED AND HE TURNED TO SEE WHAT WAS HAPPENING BEHIND HIM. AS HE TURNED HE LIFTED THE STEEL BAR IN HIS HANDS UP, WHICH PUT THE OTHER END IN THE MIXER BOX. THE MIXER PULLED THE BAR OUT OF HIS HAND AND SLAPPED IT CAUSING INJURY

EE WAS THE OPERATOR ON THE HYDRATOR, LIME HAD COLLECTED ON THE WATER RING ABOVE THE MIXER. EE TOOK A STEEL ROD 1/4 IN DIA X 15" LONG TO KNOCK THE LIME INTO THE MIXER. THE ROD HOOKED ON THE MIX ER PULLING HIS HAND INTO THE METAL BOX AND THE ROD CUT OFF HIS RIGHT INDEX FINGER AND BROKE BOTH BONES IN HIS WRIST. THEY HAD PUT IN METAL PLATE IN THE WRIST.
