Over-exertion (Not Elsewhere Classified)

The IE was walking down a flight of steps. On the last one they stepped awkwardly causing them to roll ankle. The X-ray came back negative and the employee was diagnosed with a sprained ankle. A follow up appointment on 2/13 resulted in restrictions.

Caught in, under or between running or meshing objects

IE was replacing a g-bolt on a screw conveyor gear box, after locking out the screw the IE attempted to rotate the screw to line up the bolt holes. IE did this by grabbing the drive belts to rotate the shive. This pulled the IEs right ring finger in to a pinch point causing a laceration that required 3 stitches.

Employee was lowering door to bottom of lime pit (about 15') using an attached rope. The employees thumb got caught in the rope under the weight of the door cutting off the tip of the thumb below the nail.

IE stood on a belt conveyor to inspect a stone feeder that may have been leaking. IE did not lock out the belt prior to accessing it. Conveyor belt started up and pulled the IE through a 14' height clearance before IE was able to pull the E-stop cable and climb off the belt. Incident caused multiple fractures in employee's back requiring EE to have surgery.

Fall from ladders

Employee was removing the guard on a conveyor belt while standing on a ladder. Fell off of ladder about 4 feet and landed on right side. MRI review on 12/2 revealed torn rotator cuff and employee was placed on restricted duty.

IE was unplugging a material transfer chute by banging on it with a pry bar. Felt a pinch in right hand. Went to ER on 8/27 and received a negative x-ray. Followed up at ER on 9/2 when pain and swelling persisted and received restrictions and a referral to Ortho.

Employee felt a pop in right shoulder while raking kiln. X-ray showed no breaks. Follow up with ortho on 5/4/21 resulted in pain medicine and a sling. Awaiting further diagnostic procedures.

Fall from machine

Employee tripped while climbing down from the back of a flatbed truck. Fell on upper back/shoulder. Fractured one rib.

Struck against a moving object

At approximately 5:50 AM, EE entered level 5 of the quarry with the M503 water truck for dust suppression activities. In traversing towards the working face, EE hit a pile of chat used for leveling the quarry floor. The impact launched EE in the cab. Despite being restrained by seat-belt, EE received facial wounds and a fracture of the L3 vertebrae.

EE was raking kiln 2 cooler grates when in pulling the rake back, the handle struck a hand rail behind EE causing a left shoulder rotator cuff strain. EE reported the discomfort and was sent to company doctor. EE was diagnosed on 02/06/20 and physical therapy recommended.

Struck by flying object

EE and another employee had removed a blockage at the 217 vibratory shaker screen. They completed the task and released the equipment. While walking at the base of the structure, a rock fell from the upper level, ricocheted from a rail and struck the EE on left hand. On 01/06, EE was informed ny the doctor that they had a fracture.

Emptying Dribble Chutes. Airborne object got into left eye (EE was wearing safety glasses). After attempting to wash out object, EE was taken to the ER and was treated with antibiotics and prescription eyewash.

Worker was operating dust truck that hit the road edge and tipped over into the berm. Had mild abrasion to shoulder and a small contusion to the back of the thigh. Complained about elbow pains a few weeks after. On 08/13/19, recommended for physiotherapy as an MRI showed minor tears in tendon around the left elbow, making this a reportable incident.

Over-exertion in wielding or throwing objects

Employee was hammering a G-bolt out of a screw conveyor when employee felt a pop in right elbow. Employee was examined by an orthopedic specialist who diagnosed the injury as a strain. Worker returned to work with no restrictions. Follow-up visit to orthopedic specialist on 11/29/2018 resulted in doctor writing script for physical therapy making injury reportable.

Employee was shoveling material buildup on belt conveyor when EE felt a pop in EE's left shoulder. Employee was examined at an orthopedic clinic and diagnosed with a possible rotator cuff tear/strain. Employee scheduled to have physical therapy 2-3 times per week until follow-up visit April 5, 2018. Worker returned to work on regular shift.

Employee was changing bags inside a nuisance dust collector and while exiting through the access door, accumulated lime dust on top of collector sluffed off and fell onto the employee's face and entered EE's right eye causing eye irritation. Employee received medical first aid and was given a prescription ointment for use in treating the irritation.

Fall from ladders

Employee was climbing down Ducon tank ladder, a short 3 rung stationary ladder. Took left leg off to place it on next rung and right foot slipped off. Left leg went down and landed akwardly on catwalk and right ankle got caught inbetween ladder and frame work. Resulted in strained left knee and sprained right ankle.

Employee strained lower back while performing a sample size distribution analysis with the sample shaker.

The employee was using a jackhammer with bit to remove buildup that had accumulated between the preheater bull nose and feed bed. The jackhammer bit was wedged between the bull nose and rock bed. When proceeding to begin jackhammering the employee used only one hand to grab the jackhammer, it fell pinching the employees thumb between the jackhammer and preheater ram guard.

Employee was coming down a ladder from platform when stepped to first rung in ladder and his foot slipped causing him to fall. He grasped the railing with his left hand and all of his weight was on his lift arm causing a left shoulder strain.

Employee was changing screens on 2nd deck of vibrating feeder, as he was exiting top feed end of deck his footing slipped and twisted his upper body causing his shoulder to twist in awkward position causing dislocation of right shoulder.

The worker was doing contracted conveyor belt splicing & was kneeling by the belt when he turned to get a hand tool, twisted his knee, heard a pop. He worked for the rest of the day without reporting the incident to his supervisor. He also worked every work day through 7/15/13 & reported the incident on 7/16/13. He's been out of work since 7/17/13 & has filed a workers comp claim.
