This incident was a first aid that escalated into medication that was prescribed for inflammation and pain on 3/18/25. The operator was training in a shovel when they fell against a box on the wall hitting their back impacting ribs area.

Showing all 24An employee was being loaded by a Bucyrus 495 shovel into a CAT 789C haul truck when the bucket of material landed in the bed, it bounced the employee up in the driver's seat. When landing, they landed on the left testicle. Injuring the employee, the employee was evaluated at the local clinic and prescribed Ibuprofen for inflammation.
EE was using a sawzall when the blade came in contact with hard surface and kicked back causing the blade to come in contact with ee hand and cutting hand , stitches were required.
Became a reportable, lost time on July 23, 2013. On July 8, 2013, employee was using a large impact air gun to change haul truck tire. Several days later he had tingling and numbness in his left hand. The tingling and numbness would come and go. He was given work restrictions on July 22, 2013. Employee missed scheduled shift on July 23, 2013.
EE was cleaning rust off tank using a 7lb. grinder, complained about shoulder hurting next day to relief supervisor. On Monday 4/1/2013 ee complained to regular supervisor who then took him to the clinic. Modified duty was prescribed. TCM did not entirely know this was work related due to circumstances and further investigation that was pending. It is now being reported.
EE was driving truck, being loaded by a shovel, a rock hung up in bucket then came out and hit bed of truck, rocked truck.

Removing sheave on pyrite cell using overhead crane. Too much pressure put on sheave, popped up and hit employee on buttocks. This became a reportable when employee had surgery 7-19-10.
Cutting metal with a torch, a piece fell on his right foot. No broken bones but does have restricted duty.
Grinding in the ball mill, foreign object in the eye.
Employee was in the process of installing a rope sheave on a 6"x40" shaft. Shaft was stood on end. When the crane went to position the sheave it bumped the shaft causing it to tip over. The employee tried to keep the shaft from tipping over and tore a bicep muscle in his left arm.
Cutting out bed liner, liner had debris behind it with pressure liner popped out and hit nose and mouth.
While being loaded by shovel, rock came off dipper hitting dovetail of truck and shaking operator, hurting neck.

WORKER WAS TORQUEING THE INSIDE WHEEL NUTS ON A CAT 77 USING A 1" AIR WRENCH. THE WRENCH WAS NOT "SEATED" PROPERLY ON THE WHEEL NUT WHEN THE AIR WAS APPLIED, RESULTING IN IT TURNING & PINCHING HIS RIGHT THUMB AGAINST THE VALVE STEM GUARD. THE WORKER REQUIRED TWO STITCHES IN ORDER TO REPAIR THE CUT.
FOREMAN WAS DRILLING. REACHED UP TO TURN OFF MACHINE. RIGHT HAND WAS STRUCK BY SHARP ROCK (SOFT BALL SIZE) WHICH DISLODGED FROM ABOVE AND IMPACTED HAND RESTING ON DRILL. BARRED DOWN BEFORE DRILLING. FOREMAN WAS WEARING ALL REQUIRED SAFETY EQUIPMENT.
DURING AN EXTRICATION CLASS EMPLOYEE WAS WORKINGWITH THE JAWS OF LIFE (HYDRAULIC SPREADER) & APPARENTLY TWISTED & TURNED, STRAINING HIS BACK.
