Employee was putting cab corners on a truck. EE was running a hand grinder when a very small piece of metal bounced off the bed and under safety glasses into right eye. EE came to work 8/4 but was sent to doctor that day. Came back 8/5 no restrictions. EE is trained to wear a face shield but didn't.

Showing all 3Employee was cutting welds off of the shaker screen shaft housing cover so it could be removed and replaced. The angle grinder employee was operating was caught on a bind and caused it to kick back into employee's chin under the face shield. This caused a laceration on employee's chin.
While changing out a drill steel on the roof bolter the employee got his hand caught in between the front drill support, which is a split bushing type that opens during the installation process and closes after steel installation.
