When attempting to close tailgate after dumping off, driver grasped tailgate bottom and pulled outward. As they let the tailgate fall to latch, they did not ensure fingers were clear of the pinch point. Their right, little finger was smashed. EE was diagnosed with a open fracture of the distal phalanx of the right, little finger and required 14 stitches to close the wound.

Employee was climbing off of the rock truck when employee lost footing and fell approximately 4ft. The employee tried to catch themself but landed on left shoulder and left hand.

EE was walking and using the hand rail on walkway down to #9 belt Tunnel when EE's left foot slipped forward and EE's left leg extended outwards. EE caught self before EE fell but said EE heard a popping noise around left knee. This was not a lost time until EE went to doctor on 5/16.

Employee was putting cab corners on a truck. EE was running a hand grinder when a very small piece of metal bounced off the bed and under safety glasses into right eye. EE came to work 8/4 but was sent to doctor that day. Came back 8/5 no restrictions. EE is trained to wear a face shield but didn't.

Employee was cutting welds off of the shaker screen shaft housing cover so it could be removed and replaced. The angle grinder employee was operating was caught on a bind and caused it to kick back into employee's chin under the face shield. This caused a laceration on employee's chin.

The employee was cutting a gasket off of a steel plate with a razor knife when the knife slipped and cut the thumb on the left hand. The injury required five stitches to repair the thumb.
Not wearing any gloves using a knife to cut nylon wire tie from a piece of wire.

While setting an engine back into the scaling truck the engine shifted and caught the mechanic's left index finger resulting in an amputation right at the base of the finger nail.
A roof fall occurred in heading #25 between 25-2 & 25-6 blocks. The fall was approximately 450' long & spanned the width of the 60' heading. The area had been previously supported by 14', grade 75 mechanically anchored roof bolts installed on a 5 x 5 pattern.
While climbing a conveyor ladder the employee's foot slipped off and jerked his back when he caught himself with his arms.
