Showing all 4Employee was trying to straighten a roof bolt that was slightly bent using the step on the Fletcher Roof Bolter when the bolt rotated and slipped while in the step causing the employee to hit his left wrist on the step causing it to have a small slight fracture to his wrist.
He finished drilling a hole and was backing drill on an even bench, the right track went into a ditch and the drill tipped over. the mast was not centered, which caused more weight to the right side.
