EE was on knees moving parts from the floor to a steel parts bin. As he moved from a left to right position he smashed his right finger against the steel bin and the part he was holding. Which resulted in 4 stitches on his finger.

Employee was going to cut off old hydraulic tank off of frame of old control trailer when the heat got to the tank and a flash fire occurred, when gases heated up inside of the tank and vented. EE had minor 1st degree burn to face & lips. He was wearing the proper PPE and the oil splattered on the glasses.

He finished drilling a hole and was backing drill on an even bench, the right track went into a ditch and the drill tipped over. the mast was not centered, which caused more weight to the right side.

HE WORKED 10 1/2 HOURS ON 10-22-03. HE TOOK OFF WORK ON 10-23-03. HE TOOK A PERSONAL DAY ON 10-25-03. I CALLED HIM ON 10-27-03 TO SEE WHY HE WAS NOT WORKING. HE STATED THAT ON 10-23-03 HE COUL D NOT GET OUT OF BED, DUE TO A SORE BACK. HE STATED THAT HE DID NOT KNOW WHAT HAPPENED TO HIS BACK.

HE STATED THAT THE A-FRAME BELT WAS CURLED UP UNDER THE CHUTE. HE HAD CO-WORKER TURN BELT ON/OFF TO SEE IF IT WOULD UNCURL, WHILE DOING SO, ROCKS START FLYING. HE THREW HIS HANDS UP TO COVER F ACE. HIS GLOVE ON RIGHT HAND HIT BELT CLAMP AND CAUGHT HIS HAND.
UNDER EX 270 HOE USING AN IMPACT WRENCH WHEN METAL FLEW INTO HIS LEFT EYE. DID NOT GO TO DOCTOR UNTIL 1/17/01. HAD FACE SHIELD ON.
