Employee was clearing out a plug up from the kiln feed hopper through a side hatch cover. As the employee was performing this task, hot debris came out of the hatch cover dusting the employee resulting in burns to the employee's forearms.

Injured was in the process of removing a screw conveyor with a crane. Injured was grasping the crane cable when the load shifted causing an injury to his right thumb.

EE suffered an injury when he smashed the tip of this little finger between a hand tool and a concrete ceiling. He was attempting to re-engage the coal mill airlock clutch. While pulling down to engage the clutch, the airlock clutch coupler engaged causing the wheel and tool to spin up and smash the His finger. EE was given Rx Drugs, and Surgury
EE was cutting bolts on a screen deck using a hand grinder. The sparks created by the grinder where hitting his leg & one of them burned thru his jeans & burned his shin. The burn was small and had a small blister. The blister broke and he got an infection in it so he went to the clinic and was given an antibiotic.
Injured was using an impact wrench, with a 4 inch socket, to remove a nut from the axle of the plant dump truck. The socket broke and struck the injured's left index finger, causing a laceration.
Injured was removing coal mill liners using a pneumatic impact wrench. The employee was holding a bolt between his thumb and index finger when he activated the wrench. The head of the bolt caused a laceration to his thumb and index finger.
EE went to spray out the top of the chute from 5/7 ft away from the chute door with a water gun. As he was spraying a back pressure blowout occured, shooting hot mud & lime towards the EE and hitting him in the face. The EE was treated for 1st & 2nd degree burns & was given Rx medical ontiment & Work Restrictions.
Injured employee received a laceration to the back of his right hand when Kiln 1 auxiliary motor clutch came apart. A piece of the clutch struck the employee's hand, causing a laceration requiring stitches.
While shooting kiln gun at kiln #5, employee reported swollen right hand.
Employee was tightening, adjusting bolts on frame in an attempt to raise the frame. The frame would not raise so the employee used a hydraulic jack. The hydraulic jack popped out and struck the employee in the left leg.
STARTING THE LOAD OUT SCREEN, THE EE GOT LIME IN HIS EYES.
Injured was palletizing 50 pound bags when he developed pain in his right shoulder.
EE WAS WORKING ON #4 BRIQUETTER WHEN A FLANGE HE WAS WORKING ON CAME DOWN AND SMASHED HIS RIGHT HAND CATCHING HIS FINGERS, HE WAS TAKEN TO THE COMPANY CLINIC.
WHILE LOOKING INSIDE KILN TO VIEW FLAME, KILN PUFFED AND SOME LIME DUST BLEW AROUND GLASSES INTO LEFT EYE.
WHILE TESTING A SHOP-VAC, ELECTRICIAN HAD CHECKED ALL CONNECTION WITH THE MACHINE LOCKED OUT ANDCOULD FIND NO PROBLEM WITH IT. HE SUPPLIED POWERIT (110 VOLT) AND STARTED CHECKING CONNECTIONS WITH TEST INSTRUMENT. THE WEIGHT OF THE TEST PROBE CAUSE A LOOSE CONNECTION TO BE COMPLETED ALLOWING THE MOTOR TO RESTART. THE FINS CUT THE EE FINGER.
EE WAS OPERATING SCALING MACHINE WITH HAMMER, SCALING DOWN RIBS WHEN THE ROCK UNEXPECTEDLY CAME LOOSE, ALLOWING THE BOOM TO STRIKE THE FLOOR, JARRING THE OPERERATOR'S CAB.
EE WAS ATTEMPTING TO CLEAR OBSTRUCTION FROM AIR LANCE, PRESSURE WAS TURNED OFF, BUT PRESSURE WAS BEHINE OBSTRUCTION WHEN THE PRESSURE RELEASED UNEXPECTEDLY IT RESULTED IN A LIME BURN ON EE'S RIGHT EYE. TREATED BY A DR. ON 12/31,01/01,01/02, & CLEARED TO RETURN TO REGULAR SCHEDULE.
EE WAS INSTALLING ROOF BOLTS IN BACK, WHEN A ROCK CAME DOWN AND STRUCK THE EE'S LEFT HAND BETWEEN THE INDEX & WNE FINGER, FRACTURING LARGE BONE ON INDEX FINGER. ALSO REQUIRING 7 STITCHES TO CL OSE WOUND.
ELEVATOR COUNTER WEIGHT FELL ON INDIVIDUALS RIGHT THIGH & LEFT SHIN WHILE HE WAS VACING MATERIAL OUT. THERE WERE NO BROKEN BONES - JUST SKIN ABRASIONS.
EE WAS PERFORMING NORMAL WORKING DUTIES WHEN HE RECEIVED LIME DUST INTO HIS L EYE. EE WAS TAKEN TO OCCUPATIONAL HELATH CLINIC WERE HIS EYE WAS FLUSHED OUT AND EE RELEASE BACK TO NORMAL WORK DU TIES. WORK UNTIL HE COULD VISIT A EYE SPEICALIST
CUTTING A PIECE OF OVERHEAD METAL W/TORCH WHEN A NICKEL SIZE PIECE OF HOT SLAG FELL DOWN SLEEVE AND BURNED RIGHT WRIST.
EE WAS USING PORTABLE GRINDER TO COMPLETE REPAIR TASKS ON TRUCK SCALE. EE WAS WEARING ALL REQUIRED EYE PROTECTION(S.GLASSES/FULL FACE SHIELD). WIND APPARENTLY WAS BLOWING AT EE'S BACK & AIRBOR NE MATERIAL WAS BLOWN INTO THE FACE SHIELD CAUSING A PIECE OF MATERIAL TO FLY INTO HIS EYE.
EE WAS ON TOP OF BAGHOUSE HOLDING VAC HOSE. WHEN IT WAS TURNED ON THE PRESSURE IN THE HOSE CAUSED IT TO WRAP AROUND HIS LEFT FOREARM CAUSING BRUISING AND MUSCLE STRAIN.
EE RECEIVED FLASH BURNS TO EYES WHILE ARC GOUGING WELDS. EE WAS WEARING PROPER PPE ON EYES. IT IS SUSPECTED THAT REFLECTIONS OF THE ARC CAUSED THE CONDITION. EE WAS SENT TO THE DR. AND DIAGNO SED W/ULTRAVIOLET KERATITIS & RECEIVED INJECTIONAND PRESCRIPTION OF TETRACAINE.
INJURED EE WAS GRINDING STEEL PLATE, GRINDER SLIPPED OUT OF HAND. CUT RIGHT PINKY.
WORKER USING RIVET BUSTER CHIPPING OUT BRICK INSIDE KILN AREA WHEN A BRICK FELL STRIKING HIM ON THUMB ON RIGHT HAND.
EMPLOYEE STACKED BAGGER WITH 1/2 FULL BAGS ON CHAIN. WHEN BAG WAS KICKED OUT AND HIT BELT, LIME BLEW FROM BAG SPOUT AND WENT UNDER EMPLOYEE'S GLASSES INTO HIS EYES. EMPLOYEE WAS SENT TO MEDI CAL FACILITY TO CLEAN LIME FROM EYES. FIRST AID INCIDENT.
