Miner had completed drilling hole in ore face, their right hand slipped from the D handle as the hammer cammed over and pinched right middle finger between the hammer and the leg connection point.

Employee received a laceration to the tip of the index finger on the left hand while operating a drill press. This required 5 stitches to close the wound.

While training a miner using a jackleg with a 4' steel to drill a bolt hole in the back, EE was advised to reposition the leg. While repositioning the leg, the miner gave the cob too much pressure losing control of the drill. The drill fell backward towards the trainer hitting EE in the face splitting EE's lip. *The miner received 3 stitches by the medical facility's doctor.

A miner was using an angle grinder to cut conduit when the miner reached around the conduit to cut the back side the grinder bound up and bounced towards the miner striking left forearm causing the injury. The miner received 6 stitches and a tetanus shot. There was no time lost and no work restrictions. The guard was in place at the time of the accident.

While drilling a face round with a jack leg drill, miner drilled into a vertical exploration hole from the surface, the hole was full of water and the pressure of 1300' of water forced the drill back into the miner chipping tooth.

Miner was helping another miner bolting a rib with a jack leg drill and when pulling drill out of hole he placed his finger in pinch point between leg and drill

Miner was drilling a raise round when his chuck tender broke on his jackleg drill causing the drill to come off the drill steel and the steel fell out of the hole striking his finger breaking it.

The employee was removing a cutting edge of a loader bucket using an impact wrench. The weight of the impact wrench pushed the spinning edge of the bolt out of the recessed hole of the bucket the bolt cut through the employees gloves and the under side of his wrist and thumb. The employee received several stitches and 10 days of modified duty.

Men were air lancing inside tank to break up solids to drain. Coupler came loose from hose and began whipping, hitting employee on hand.

Injured miner was in process of drilling out 8' round. When finished drilling hole he pulled the steel out of the hole and was in the process of pulling the 8' drill steel out of the jackleg. Placed his right hand at saddle and when machine came off the drill steel the machine came down catching right hand between the machine & leg which caused crush injury to his wrist.

Miner was longhole drilling and trying to replace his steel and lost control of the drill, pinching his right hand between the leg and drill. Miner was taken to clinic and X-ray showed a minor fracture of the right wrist. Miner placed on light duty with no time lost.

Worker was bolting in the 252 xcut he had just installed a bolt in the mat then he started to collar another hole in the front of the mat. while he was drilling the second hole the bolt he had just put in shanked off with the plate on the bolt, it hit worker in the mouth cutting his mouth, needing four stitches.

Worker was rock bolting in east decline. The split set bolt he was driving in was too low to get the drill and driver on in a vertical position. He was trying to force the bolt in when the ring snapped off causing the machine to lunge forward. His hand was on top of the machine and hit the rock bolt cutting his left ring finger requiring 5 stitches.

EE was cleaning breakers removing chloroform & ammonidate. This man realized that the exhaust fan was not functioning correctly at this time, noticed more fumes than usual. Advised his supervisor of this situation at which time they went to dr to see if any damage occured to his lungs.
