Fall to the walkway or working surface

Drilling Boulders with sinking hammer to be shot miss stepped on small 5 inch rock lost balance and fell to ground, tried to stop fall w/ left arm and fractured left radius bone

Over-exertion in pulling or pushing objects

While bolting a rib with a jackleg drill the miner twisted his knee while pulling the drill out of drill hole. Third Dr visit showed a torn ACL on 11/11/14

Miner was helping another miner bolting a rib with a jack leg drill and when pulling drill out of hole he placed his finger in pinch point between leg and drill

Over-exertion in lifting objects

Miner was lifting half of a 150 lb. deck grate and suffered a hernia on right side

Miner was drilling a hole in the rib and after completion of the hole leaned the drill against the rib and had his hand in a pinch point between the dog nut of the steel and the chuck of the drill.

Struck by falling object

Miner was drilling a raise round when his chuck tender broke on his jackleg drill causing the drill to come off the drill steel and the steel fell out of the hole striking his finger breaking it.

Miner was hanging utilities and slipped on wet ground, falling and landing on a jagged rock with his right forearm, causing a cut near the elbow, requiring 4 stitches.

While drilling with a jackleg, the jackleg was extended up faster than its penetration rate and broke off the steel, the hammer drill then struck the face of the Miner
