The employees left index finger was between a bolt and the handle of the wrench, so when he started to torque the bolt, his finger was pinched between the bolt and the handle causing a laceration to the tip of the left index finger.
The employee was reaching for a welding rod to continue welding and his right arm came into contact with the hot metal that he was welding, causing a burn to his arm.
EE MOVED & SET UP BACKHOE ONTO UNSTABLE FILL NEAR EDGE OF BANK, AGAINST PREVIOUS INSTRUCTIONS TO STAY AWAY FROM EDGE. EE DID NOT DISENGAGE TRANSMISSION AND MACHINE DROVE OFF EDGE INTO POND (2 FT. DEEP). EE WAS NOT INJURED BUT MACHINE RECEIVED WATER DAMAGE TO ENGINE.
