Showing all 7Using pneumatic jackhammer to tear out refractory lining inside the cylinder wall of kiln. Worked up wall to about shoulder height when jackhammer vibrated backward. Employee's right hand was caught in the handle. Diagnosed as soft tissue strain. Returned to work the next day before traveling to follow up with home doctor for possible ortho referral.
The bag rack had jammed and the employee was trying to get it out. The arm on the rack came down on the employees right knuckle. Right knuckle was swollen.
EMPLOYEE WAS GRINDING, CUTTING & WELDING WHEN SOMETHING GOT IN HIS LEFT EYE. EE WAS TAKEN TO THE E.R.& METAL WAS REMOVED THAT WAS EMBEDDED IN EE'S LEFT EYE. EE WAS WEARING SAFETY GLASSES.
EE WAS ON THE PACKAGING B-LINE, MOVING THE SEWING HEAD WHILE THE MACHINE WAS STILL RUNNING. THISRESULTED IN HIS RIGHT PINKY BEING FULLED IN THE BELT DRIVE. THE FINGERTIP WAS CUT OFF. HE WAS TA KEN TO THE HOSPITAL WHERE HE RECEIVED STITCHES. THE EMPLOYEE WAS RELEASED TO RETURN TO WORK.
ON 6-27-01, EE WAS WELDING A LID FOR A BLOWER FILTER, WHEN A PIECE OF SLAG CAME UNDERNEATH THE WELDING HOOD, ENTERING HIS RIGHT EYE. ON 6-28-01 HE REPORTED THAT HE HAD SOMETHING IN HIS EYE. HE WENT TO THE E.R. AND HAD SOME OF THE OBJECT REMOVED. ON 6-29-01 MARION EYE CENTER REMOVED THE REST OF THE FOREIGN BODY FROM HIS EYE. HE WAS OFF FOR ONE DAY.
THE AUGER ON THE #3 DRYER WENT DOWN. EE WAS TRYING TO RELEASE PRODUCT FROM THE DRYER TO FREE UP THE AUGER SO IT COULD BE RESTARTED. WHEN THE UNLOCKED DOOR CAME OPEN, A RUSH OF STEAM AND HEAT C AME OUT CAUSING 2ND DEGREE STEAM BURNS TO THE R-FORE ARM, FROM WRIST TO ELBOW, L- FOREARM, AND THE LEFT SIDE OF NOSE. MEDICAL TREATMENT WAS PERFORMED WITH DEBRIDEMENT ON BOTH ARMS.
