Miner stated that the alledged injury might have happened at a different operation. Miner was taken to clinic where he was diagnosed with cellulitus. He was placed on restricted duty from 12/26 thru 1/1/07.

While using a chipping gun to knock off build-up on the rails to the 7 FK pump, the employee strained his left shoulder.
2nd degree burns on right index and middle finger, return to limited duty.
USING JACK HAMMER AND AIR IMPACTING ON THE OUTSIDE OF MILL. AT THE END OF SHIFT THE EMPLOYEE HAD PAIN IN HIS LOWER BACK. THE EMPLOYEE WAS GIVEN PAIN MEDICATION AND RELEASED TO FULL DUTY. SEVER ITY OF PAIN INCREASED AND DOCTOR TOOK HIM OFF WORK ON JAN 30, 01 FOR 3-5 DAYS.
EE WAS USING A AIR HOSE AND IT IT DISCONNECTED, HITTING HIM IN THE MOUTH AND FACE KNOCKING OUT FRON TOOTH (L) SIDE.
