Showing all 8Working on #1 hydrator cutting steel with grinder. Maintenance operator stood up after completing the work, turned head to the left and felt something fall into left eye. Employee was prescribed eye drops for eye infection.
The EE was attempting to remove a grease gun coupled to a grease zerk that had become stuck. The EE pulled on the hose and the coupler whipped back hitting his teeth.
EE was cutting floor grating with grinding wheel. The wheel hit the edge of grating and kicked, resulting in a cut on the side of the left hand.
Employee was using a jog control to inspect the hydrator when he noticed the lead light he was kneeling on was still in the hydrator and starting to wrap up. He reached down to move the cord and the hydrator pulled his right hand into the path of the side scraper. The skin on the back of his right hand was torn down toward his knuckles and he also broke the tip of his right finger.
PULLING SCALE RING FROM KILN. HAD JACKHAMMER AT 9/10 O'CLOCK POSITION & WHEN HE PULLED HAMMER BACK A LARGE PIECE OF SCALE CAME LOOSE & ROLLED DOWN JACKHAMMER STRIKING HAND & THUMB.
EE WAS SITTING ON A FIVE GALLON PAIL WELDING AUTOCLAVE SHAFT. WHEN GETTING UP FROM BUCKET HIS BACK HURT AND COULD HARDLY WALK.
USING AIR LANCE TO CLEAN BETWEEN LIME CONVEYOR IDLERS, MATERIAL SHOT BACK AT EE & AS HE TURNED HIS HEAD A PIECE OF LIME GOT THROUGH HIS GOGGLES CAUSING IRRITATION TO THE EYE. SENT TO OCC DRS C HECKED HIM & APPLIED MEDICATION. GAVE HIM A PRESCRIPTION FOR OINTMENT TO APPLY TO EYE.
