A miner was cutting a piece of metal with a torch. A piece of hot metal fell from the cut and on to the miner's jeans. The metal melted through the jeans and fell down in to the miner's slip-on boot and came to rest on the top and outside of the miner's left foot - burning their foot.

Showing all 2Employee was removing a cutting edge wear liner from loader bucket. Employee was loosening nut with air-powered tool when bolt began to spin, employee used hand to push bolt up into the wear liner's bolt socket. Weight of the tool pushed the bolt from the socket, thus allowing the bolt to spin freely. The employee's hand was cut when bolt head spun.
