The employee was attempting to determine the cause of vibration alarms, and while feeling for vibration, their hand was able to enter the guarded area, which resulted in right pinky finger making contact with fan blades, resulting in the avulsion of the distal end of their finger. The employee did not lose the whole finger, but the flesh on the tip of their finger.

Employee was performing some demolition work at Plant 2. While using a reciprocating saw, the employees thumb slipped up and over the rest into the reciprocating action of the saw, smashing finger. Employee received 6 stitches at the ER and returned to work the following shift.
Employee was operating a bagging station filling 50 lb. bags of product. A bag overfilled and burst sending product into the employee's right eye and causing irritation.
Adjusting saddle on 255 packer, saddle started to slid towards the floor, employee attempted to catch it. Right thumb caught in pinch point, resulting in cut requiring 13 stitches at local ER.
Employee was scaling on 820 level, scaled down small piece of marble, scaling boom rocked forward approx 3 feet, causing operator cab to rock. Employee felt pain in neck from rocking action. Evaluated at local ER with slight neck strain.
Rock being crushed by Jaw entered drive pulley, fragmented rock, fragment hit employee on left hand.
Employee removed an inspection plate cover just below the airlock feeder on the cyclone located on Silo #3. The airlock feeder was running; employee's right hand contacted the airlock rotor, cutting off tips of the index, middle and ring fingers. Employee was treated at local ER.
Employee was using a cordless drill to tighten a screw. Drill twisted in his hand , cutting his middle finger on his right hand against a fan housing. Taken to ER and received 7 stitches. No work restrictions.
Employee was driving Getman Scaler #1 out of upper dolomite area of Mine #4, ran over rock or edge of roadway, causing cab to rock upwards quickly. Employee hit top of forehead on light attached to cab frame. Cut to forehead requiring 6 stitches. Employee was wearing hardhat and seat belt at the time of the accident.
Employee operates a Trackmobile. Over several days, developed a pain in the left knee while performing normal operator duties. Informed manager on 2/22/2005 of the pain and requested medical treatment. No record of an actual injury date. Doctor diagnosed slight strain to left knee. Returned to work same day.
Employee was operating a Getman scaler. Developed back strain (spasms) during his shift. Requested medical treatment on 10/07 (next day).
Strong & gusty winds caused small piece of metal being cut to burn through pant leg & fell into boot, causing burn on right foot.

EMPLOYEE WAS HOLDING A WEDGED HEAD BOLT IN A SLOT ON A JOURNAL BALL WHILE ANOTHER EMPLOYEE TRIED TO TIGHTED BOLT WITH AIR WRENCH, BOLT SLIPPED AND CUT HIS RIGHT RING FINGER. EMPLOYEE WAS WEARI NG GLOVES AT THE TIME. TAKEN TO LOCAL ER FOR 11 STITCHES AND RELEASED.
EE WAS ADJUSTING ELECTRIC EYE ON PALLETIZER IN PLANT #2. PUT MACHINE IN MANUAL, RESTORED POWER AND ATTEMPTED TO CYCLE THE MACHINE TO CHECK ALIGNMENT. EE PUT HIS LT. HAND TOO CLOSE TO THE MOVI NG PLATE AND RECEIVED CUTS TO THE MIDDLE FINGERS ON HIS LT. HAND.
EE WAS OPERATING THE #10 PACKER. THE BAG SADDLE STUCK IN THE TILTED POSITION. HE TRYIED TO MOVE IT & HIS RIGHT MIDDLE FINGERS WERE MASHED. HE RECEIVED TREATMENT AT THE ER & RETURNED TO WORK.
EE DID NOT LOCK MACHINE OUT BEFORE SLIDING UNDER9" H BY 32" W OPENING AT FLOOR LEVEL. MACHINE WAS RUNNING WITH ALL GUARDS IN PLACE. HE WAS LAYING ON HIS STOMACH WHEN ELEVATOR CAME DOWN AND PINNED HIS UPPER THIGHS.
EMPLOYEE WAS HOLDING NUT ON RAYMOND MILL SPIDER WITH WRENCH AS FELLOW EMPLOYEE TIGHTENED BOLT W/PNEUMATIC WRENCH. AS PNEUMATIC WRENCH WAS ENGAGED THE NUT/WRENCH SPUN, PINCHING THUMB ON RIGHT HAND AGAINST SIDE OF HOUSING.
EMPLOYEE WAS ATTEMPTING TO FREE JAMMED BAG FROM SEALER ON HAMER AUTO BAGGER. INSTEAD OF RAISING OR SLIDING SEALER AWAY FROM POINT OF OBSTRUCTION, EMPLOYEE REACHED IN AND PULLED ON JAMMED BAGS. AS BAGS WERE IMMEDIATELY PULLED THROUGH, EMPLOYEE'S HAND WAS CAUGHT BY ROLLERS.
EE WAS PLACING BAG ON PACKET SPOUT AND THE BAG CLAMP CAME DOWN ON FINGER. MASHED LITTLE FINGER ON RIGHT HAND.
EEWAS DISCARDING METAL PLATE THAT WASTOO BIG TO LIFT. EE GOT UP ON METAL & USED CUTTING TORCH TO REDUCE TO MANAGEABLE SIZE. METAL PIECE BROKE LOOSE, SLIPPED 2", EE SLIPPED FORWARD FROM SQUATTI NG POSITION AND STRAINED BACK.
EE WAS FEEDING NEW ROLL OF BAGS THROUGH BAGGING MACHINE AND REACHED IN TO PULL ON BAG, TO ADVANCE ROLL, AND GOT FINGER CAUGHT IN MOVING SHIEVE.
