SH8 Operator sustained rough loading injury after hitting a hard spot. The seat was discovered to be damaged allowing forward and back movement of about 3-4 inches.
A mantle liner was being prepared to be hoisted to another position - rigging from the crane caught one of the ears on the mantle, knocking it off the cribbing onto employee's foot. Foot was not amputated at the time of incident.
Rubber Tire Dozer Operator working on the N3 near SH8 stated their back was hurting and was evaluated by the WVFD. Operator returned to work and was seen at the Clinic the following day and they were released to work with restrictions.

Driver of haul truck was backing under shovel and was not square to the house. Shovel Operator loaded the haul truck anyway, creating a rocking motion which caused pain in the Driver's back.

Employee was air-arcing on the revolving frame beneath a P&H 4100A shovel. Hot slag entered through the employees protective coveralls and fell down into EE's steel-toed right boot. The employee sustained 2nd and 3rd degree burns on the top of EE's right foot.
EE was using rotary/demo hammer to remove tile. The spade bit needed to be repositioned. EE held hammer in lap to reposition bit. Hammer slipped & EE reacted by grabbing handle and hit trigger. The chuck closed and EE's glove caught in the chuck and spun. EE's right 2 middle fingers were lacerated by the glove material tearing when caught in the rotation.
Employee assisted in the welding repair on 8/4 at 16:30 - the next day employee reported to the supervisor of the possibility of a flash burn to EE's eye.

While operating D10-23 on the IOS Mill Stockpile, it tracked backwards over a small windrow causing the employee to feel a sharp pain in the right hip.
While employee was drilling through 3" channel steel with a hand drill, the drill bit caught and spun the drill motor around hitting employee in the lip with the battery.

While tack welding on rock knockers on a haul truck bed in the overhead position, a hot chunk of molten slag fell from the weld puddle and landed on employee's upper right thigh causing a burn.

As cable was being rolled up it became stuck in some mud and when it broke free the cable struck employee. Employee regained footing and while walking back to the vehicle started feeling pain. Employee called for supervisor and was taken to clinic for evaluation. The employee's leg was fractured and they were unable to return to work for next scheduled shift.

Technician was replacing a step on the pusharm of a D10. Tech tack-welded it in place and went to weld the underside when the 100lb step fell off and landed on thumb.
Truck was padded with loose material for a boulder to be loaded. When shovel swung over the bed to load the boulder, the boulder fell through the bucket door onto the back of the haul truck bed. This caused the truck bed to pick up and fall back down jarring the truck driver.
