The employee was changing a unplugged drill bit. EE was using EEs hands to loosen and tighten the drill bit in place. EE could not get a sufficient grip on the drill due to wearing gloves that had oil/grease on them as did the drill. EE removed EEs gloves to attempt to improve EEs grip. EEs hand slipped while tightening the drill and struck the tip of the bit causing a laceration.

At the completion of a job on the Bypass Dust Collector the Production Coordinator was ensuring all valves for the system were secure. As they walked to the discharge valve on the bypass compartment (which was closed) a blast of dust came out from the Benetech inspection door. The material hit the employee in the face and head causing an injury to the left eye.

The employee was using a cutting torch to remove corroded bolts from the synthetic gypsum system. After cutting 3 bolts the employee reoriented self to cut the 4th. This changed the direction that fumes would exhaust from the cutting. The employee felt dizzy and short of breath from the exposure and was issued prescription medication and restricted duty.

While installing an electric motor on equipment I.D. 351-BE-10, the employee was standing on the outer casing of the bucket elevator to set the motor in place. The employees were using a metal beam to hoist the motor in place. The beam slipped and dislodged from its resting location and onto the floor decking. When the beam fell, it struck the employee in the upper torso.

The employee was using an air lance while on a scissor lift. The scissor lift tipped over while the employee was in the basket. The impact caused the employee to suffer from broken bones in arm and pelvis.

The employee was using a torch to cut a bolt. When the bolt was cut it fell into the employee's glove causing burns to his hand.

The EE was observing a plazma cutting operation, when the cutter slipped, contacting the EE in the groin area. This caused a burn, requiring medical treatment.

A kiln dust seal was leaking dust near the location the employee was working. The kiln dust got in his eyes and the employee apparently rubbed the dust in his eyes.

OVERHEAD BUCKET OF CRANE HIT THE HOPPER EE WAS WELDING IN. A PIECE OF MARL FELL OFF OF THE TOP OF THE HOPPER AND INTO THE HOPPER, HITTING EE IN HIS RIBS (LEFT SIDE).
