Employee was holding a carriage bolt head to keep it in place while an impact was being used to remove the nut. The bolt pushed out and spun in EE's hand, lacerating the right index finger and requiring stitches.

Changing flights on apron feeder. Injured employee was putting a nut on a bolt when other employee hit the trigger on the impact gun that was on the bolt, spinning the nut cutting employee's right thumb.

ROTARY HEAD IN THE DRILL MAST WAS NOT SECURE PRIOR TO WORKING ON THE BROKEN CHAIN. WHEN EE STARTED RAISING THE BROKEN CHAIN WITH A MANUAL COME ALONG, THE ROTARY HEAD SHIFTED AND DROPPED. HE WA S STRAPPED IN A SAFETY HARNESS, BUT WAS SHOCKED BY THE SUDDEN MOVEMENT AND GRABBED THE CHAIN WHICH DREW HIS HAND INTO A PROCKET, INJRING HIS FINGERS.
