Employee suffered minute hand fracture (diagnosed via x-ray at urgent care) on Feb 12, 2026, ~10AM CST when loose bearing fell on left hand while working on IBC4 pulley at batch plant. No meds prescribed; first aid only.

The employee was in the crusher clearing a rock jam when a rock rolled onto the employee's hand, injuring a finger

Employee was holding a carriage bolt head to keep it in place while an impact was being used to remove the nut. The bolt pushed out and spun in EE's hand, lacerating the right index finger and requiring stitches.

While cutting a conveyor belt for a splice, the knife slipped and struck the employee in the leg, causing a laceration requiring stitches

Employee was using a weed eater to trim weeds and got some poison ivy on ear and neck, causing a severe reaction

Employee was swinging a pick to clean material buildup from the top of a crusher and struck employee's hand on a protruding bolt causing a laceration.

Employee was walking to shop when they struck a piece of iron sticking out of a rack, knocking off their safety glasses and injuring their eye.

Employee was stung by a bee while descending stairs at the primary crusher and had an allergic reaction requiring medical attention beyond first aid.

Employee was climbing down from loader and fell off ladder landing on ground. Bruise to wrist and shoulder requiring restricted duty status

Employee was standing in the Wobbler Feeder clearing a rock jam when the rocks shifted and a large boulder fell on the employee's foot, collapsing the steel toe and causing an open fracture of the big toe.

Loading from muck pile with front end loader when a boulder rolled off and landed in the bucket causing the loader rear to leave the ground. When the boulder fell off the bucket, the rear of the loader fell back to the ground and bounced 3 times. The impact caused the injuries to the operator

On the date of accident, employee lifted rock breaker extension and felt pain in abdomen. Was not reportable since employee worked with no restrictions until hernia surgery on 05/16/2014. Reportable Lost time began on 05/19/2014.

Employee received a laceration to his right forearm when he hit a crusher anvil with a 10 lb sledge hammer and a piece of metal broke off and struck him.

Changing flights on apron feeder. Injured employee was putting a nut on a bolt when other employee hit the trigger on the impact gun that was on the bolt, spinning the nut cutting employee's right thumb.

Employee tripped and fell fracturing his left wrist.

While crawling out from a screen deck, employee dropped one foot to scaffold board. The board broke and employee fell, injuring right middle finger, and slid down chute to the conveyor belt.

Dropped a piece of plate on finger while fabricating parts for plant. Laceration of right little finger requiring 4 stitches.

EE was cutting a pin with a torch when slag blew out and went into left ear

Co-worker accidently struck employee's hand with a hammer while removing a bucket pin.

Employee had a personal injury due to a fall at home on July 4th. After working the shift on 08/03/2009, he went to his personal doctor for a check-up but while there claimed his back and left shoulder hurt him and it was from 'working too fast in the loader'.

Maintenance employee was helping to install a differential in a loader. He was using an air jack which he placed on top of a RR tie for easy access. Employee's hand was resting on the ground next to tie when jack fell off and impacted hand. Employee denied medical treatment until 9/2/08 when we sent him to the hospital for x-rays and discovered his hand was broken.

Employee lost balance and fell from back loader platform.

Employee was driving haul truck when he felt something crawling (insect) under his left pant leg along his calf. The employee swatted his leg and it went away. The next day he had some swelling and complained of some pain. The initial visit the doctor observed his injury and told him his options. The follow-up visit the doctor provided him with some antibiotics (prescription).

Employee did not report injury until 12/22/2006. He was then taken to the doctor for an initial evaluation. Employee was replacing idlers on conveyour belt and it caused tendinitis to his right hand wrist.

Employee slipped while dis-mounting dozer. The employee caught himself with one arm; thus straining his back. Refused medical treatment at first. Employee then went to the doctor for observation (No medication, No restrictions, No therapy). The employee then went to the doctor again (12/4/06) and received physical therapy treatment to resolve issue.

There were no guards in place to protect the employee. The guards were previously removed some time in the past and were not replaced. Employee heard a noise coming from the fan area and stuck his hand up in the area as a natural reaction. The fan blade cut two of his fingers.

Employee was replacing skirt board rubber on conveyor tail pulley; knife slipped and he cut his thumb.

While shoveling, he twisted to the side and felt sharp pain in lower back.

Employee stepped off steps, slipped on gravel, hurt (L) knee.

OPERATORS FOOT CAUGHT ON DESK WHEN GETTING UP, WHILE FALLING, TWISTED KNEE.

EE WAS PERFORMING ROUTINE GREASING ON BREARING AT PRIMARY APRON FEEDER WHEN SHIRT SLEEVE WAS CAUGHT BETWEEN ROLLER AND RETURN SIDE OF FEEDER PLATE PULLING EE LEFT FOREARM THROUGH THE ROLLER AN D RETURN SIDE OF FEEDER PLATE.

EE WAS OPERATING A SKID LOADER AND RAN OVER A ROUGH AREA, JARRING HIS BACK.

EE STEPPED ON ROCK AS HE WAS GETTING OFF DOZER. TURNED ANKLE AND LEG, TWISTING BACK.

EMPLOYEE WAS SERVICING EQUIPMENT WHEN HE SLIPPED ON ICE AND FELL ON CONCRETE SLAB.

EE WAS USING A STEEL BAR TO DISLODGE A ROCK IN CONVEYOR PULLEY, WHEN BAR SLIPPED CAUSING HIM TO STRIKE FINGER ON CONVEYOR FRAME.

CUTTING CONVEYOR BELT. KNIFE SLIPPED, CUTTING FINGER.

EE WAS CUTTING A PIECE OF CONVEYOR BELT, UTILITY KNIFE SLIPPED, CUTTING LEG.

EE WAS LOADING STEEL ANGLE IRON AND EXPERIENCED PAIN IN BACK & LEG.

INSTALLING 12"" STEEL PIPE WITH CRANE. PIPE SHIFTED, CATCHING HIS FINGER.

STEPPED DOWN OFF HAUL TRUCK & TWISTED KNEE ON UNEVEN GROUND. (SAID IT WAS NOT HURTING AND DID NOT WANT TO SEE A DR. ON 1-28, KNEE WAS HURTING, SO HE WENT TO DR. RECEIVED MEDICATION.)

EE WAS CLEANING CATWALK AND CAUGHT FINGER BETWEEN ROCK AND METAL FRAME.

OPERATOR CAME DOWN LOADER IMPROPERLY, CAUGHT HEEL ON LADDER AND FELL TO GROUND, TWISTING KNEE.

EE WAS TRYING TO LIFT A 5 GALLON STEEL BUCKET OF 3/4"" BOLTS. NUTS A WASHERS INTO TOOL CART WITHOUT ASSISTANCE OR ANY MACHINERY.

EE WAS ENTERING MACHINE WHEN WIND BLEW DOOR CLOSE ON LEFT RING FINGER.

ROTARY HEAD IN THE DRILL MAST WAS NOT SECURE PRIOR TO WORKING ON THE BROKEN CHAIN. WHEN EE STARTED RAISING THE BROKEN CHAIN WITH A MANUAL COME ALONG, THE ROTARY HEAD SHIFTED AND DROPPED. HE WA S STRAPPED IN A SAFETY HARNESS, BUT WAS SHOCKED BY THE SUDDEN MOVEMENT AND GRABBED THE CHAIN WHICH DREW HIS HAND INTO A PROCKET, INJRING HIS FINGERS.
