Employee and technician from Telsmith Company were pulling bearing out of 8x24 Telsmith shaker. Employee was adjusting impact wrench with help of technician. Due to numbers worn off wrench, socket flew off impact wrench hitting shaker and came back, hit employee in mouth. Note: at this time there was no injury. When employee went to dentist he found his false tooth was cracked.

EMPLOYEE WAS USING A ONE INCH IMPACT TO TIGHTEN LUGNUTS ON HIS HAUL TRUCK, HE FELT PAIN IN HIS LOWER BACK.

EMPLOYEE WAS HELPING CHANGE TIRES ON LOADER. HE WAS MOVING THE JACK STAND AND PIN FELL OUT OF THE STAND, PINCHING THE POINTER FINGER OF HIS LEFT HAND. PIN SMASHED FINGER TIP, REQUIRING 6 STITC HES.
EE WAS TORCHING OFF A BEARING RACE ON A SHAFT. APIECE OF SLAG ENTERED THE BOTTOM OF THE TONGUE AREA OF HIS BOOT, BURNING THE TOP OF THE FOOT ANDBETWEEN THE 3RD & 4TH TOES.
