WHILE BACKING UP; OPERATOR HIT ROUGH AREA CAUSING HIM TO JOLT STRAINING HIS BACK.

Rough ground conditions causing Operator to hit hole in haul road.

WHILE HAULING FILL; OPERATOR HIT HOLE IN ROAD CAUSING SHARP PAIN IN HIS BACK.

employee working was lubricating a 3/4 inch air ratchet with oil. While clearing excess oil from ratchet, oil mist sprayed into eyes. Employee treated and evaluated with chemical conjunctivitis in both eyes. He was prescribed anti-inflammatory and antibiotic medications and released to return to work with no restrictions.

Mechanic was using a lathe to fab part for a track torch. Metal shavings spun around & caught his left index finger, causing a laceration. He was treated by the facility Nurse & sent to local clinic where he received 11 sutures and was returned to work with restrictions.

A Maintenance employee was working in the Washer structure. Employee was standing on the ground directing a crane into place when he was struck in the mouth by a falling piece of rock. The employee was taken to Occupational Health Partners where he received 13 sutures. The employee was released to return to work with no restrictions.

While using chipper hammer to clean rails off; hammer hit metal causing a piece of metal to come off corner of hammer and lodging into middle finger of right hand.

Employee was engaged in cutting, grinding, and welding activities when he noted discomfort in his right eye. A subsequent examination later the same day by an Optometric Physician revealed the presence of a paint chip. It was removed and the employee was allowed to return to work with no restrictions.

An employee was removing lube piping while another employee about 10 feet behind him was operating a grinder. A piece of metal from the grinding hit the inside of his safety glasses and bounced into his eye.

Employee was working on the back end of a moving trailer when he lost his footing and fell to the ground. A Straw Blower machine attached to the back of the trailer ran over the employee's pelvic area.

Employee was using a 3" hole saw bit on a 1/2" variable speed drill when the pilot broke causing the hole saw to bite into the metal and rip the drill out of his right hand.

Machine turned over landing on worker.

MACHINE BACKFIRING AFTER NOT BEING STARTED FOR A FEW DAYS, COLD WEATHER

PIECE OF GRINDING DISC HIT FINGER.

Employee was using an oxy-acetylene torch when the head of the lit torch came in close proximity of his left forearem and burned it.

EE WAS USING AN AIR ARCH TORCH WHEN A FOREIGN MATERIAL ENTERED HIS LEFT EYE.

WHILE BEING LOADED PAN PULLED AWAY FROM DOZER, WHEN DOZER CAUGHT UP WITH PAN, HE HIT THE PAN TOO HARD.

EE WAS SETTING UP A JOB WHEN HIS WELDING STINGER WAS DRAGGED ACROSS THE EQUIPMENT AND ARCHED. HIS LEFT HAND WAS CLOSE TO THE ARCHING AREA OF THE STINGER.

WHILE USING CUTTING TORCHES TO REMOVE CUTTING EDGE BOLTS ON PAN, SPARK WENT IN EAR.

BACKED INTO DRAINAGE DITCH.

EE WAS ACTING AS A FIRE WATCH WHEN A BALL OF SLAG FELL, HIT A BEAM, BOUNCED 15' & FELL DOWN THE SHIRT COLLAR & BURNED HIS UPPER BACK.

EMPLOYEE WAS REMOVING BOLTS WITH A 1/2 INCH IMPACT WRENCH WHEN THE SOCKET FLEW OFF AND STRUCK HIM ON THE BACK OF THE RIGHT HAND.

EE WAS AIR GOUGING ON A DRAGLINE BUCKET WHEN A SMALL PIECE OF SLAG FELL INTO HIS WORK GLOVE AND BURNED THE MIDDLE FINGER ON HIS LEFT HAND. THIS ACCIDENT REPORTED UNTIL 6-19-01 AFTER THE BURN B ECAME INFECTED.

EE REPORTED TO THE NURSE ON 5/15 WITH WHAT APPEARED TO BE A SLAG BURN ON HIS RIGHT WRIST. HE STATED THAT HE WAS WORKING IN THE MAINTENANCE SHOP ON 5/11 AND WASN'T SURE WHEN IT HAPPENED. THE BU RN BECAME INFECTED AND NEEDED MEDICAL ATTENTION.

DURING THE DAY EE WAS USING A HAND HELD PORTABLE GRINDER IN A CONFINED SPACE. SOMETIME DURING THE DAY A PIECE OF METAL ENTERED HIS EYE BUT IT DID NOT EFFECT HIM UNTIL HE GOT HOME THAT NIGHT. E E WAS WEARING A GRINDING SHIELD AND SAFETY GLASSES DURING THE GRINDING TASKS.
