Using a pneumatic "tugger" winch to pull drill rods to lay down position while tripping out of the hole. Then putting winch line in stow & was trying to roll winch line on neatly. While doing had his hand in improper place so when the winch line was pulled tight his finger was in between bracket mounted on and winch line. His finger then "smashed" between the cable and bracket.

Employee working on clogged trash pump at 11 tail (Guzzla SV-60 pump). He was back flushing the pump and mud blew out the suction line and got into his eyes. He was wearing glasses. He missed no work, but did receive medical attention in the form of medicated eye drops. No problems with training, PPE, mining methods or compliance issues.

Winching a car of belt with a belt winder on 7 belt. He pulled off rope to get more slack and got his foot caught in some hydraulic hoses. When he energized the winder, the hoses moved and tripped him. Diagnosed with fracture to distal end of fibula. No problems with training, mining methods, PPE or compliance issues.

During a Part 50 audit and citation 8060588 being issued, this injury report is being filed. Employee was installing bolt when foot slipped and caused drill to spin striking employee in the face resulting in a facial fracture. Cause of injury was awareness. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

The employee stated that while guiding a roof bolt through a roof strap with his right hand, the mining machine moved forward pinching the fingers of his right hand between the roof bolt and the roof jack. The accident occurred because the employee failed to keep his hand out of a pinch point area and off the moving roof bolt. He had on his required PPE.

The employee stated that while drilling a roof bolt hole, the drill steel started to bend, as he reached out to grab hold of the drill steel, he accidentally put up pressure on the steel which caught his left hand between the drill steel and the bolt mast. The accident was caused by the failure of the employee to drop the drill mast instead of lifting it. The ee had on his PPE.

Supervisor was preparing shield for removal from the face recovery area. When attempting to raise the shield tip up the shield canopy lowered compressing him between the shield and mine floor. The investigation did not reveal any substandard conditions or indications why the shield lowered. This did not involve job skills or attitude, personal safety equipment was being used.

Standing on right side of Joy miner when supplied roof bolt got stuck in miner boom and swung out striking employee in the right side of his face, resulting in a laceration and contusion below his right eye. Safety glasses absorbed most of impact. No problem with PPE, training, mining machines or compliance issues.

Employee was drilling bolt hole in mine roof when hydraulic line failed on bolter and sprayed hydraulic oil in operators left eye. It didn't involve any compliance issues. No equipment involved. It didn't involve job skills,proficiency,training or attitude. No PPE or protective devices on equipment involved.

Employee was putting drill steel in drill chuck when the drill operator closed the clamp on the helpers hand causing a fracture to his left middle finger. It didn't involve any compliance issues. The ee claims that as he was rockdusting, the rockdust hose plugged. As he tried to use pressure to unplug the hose, it suddendly jerked causing the ee to fall to the ground. The accident was caused by the employee's failure to replace the hose and used excessive air pressure in an attempt to blow the hose clear. The ee had on the appropriate PPE.

Injured was drilling a hole for a 16' cable bolt on the tight side of 23 West belt at 22 block. The drill hit a hard spot about 7' in, stopping the bit and torquing the gopher drill which pulled on his right shoulder resulting in a strain/sprain. No problems with mining methods, training, PPE or compliance issues.

While center bolting the ee placed his gloved hand over the chuck to check the suction. A drill steel fell from the drill hole striking the ee on the tip of the left middle finger resulting in a laceration and fracture of the finger tip. The cause of this accident was due to the ee failing to secure or remove the drill steel prior to placing his hand in a vulnerable position.

When placing steel roof strap across the continuous miner the right hand was pinched between the strap and coal rib when continuous was finishing mining cycle, individual trained on job task. Individual received 11 sutures to the right index finger.

He was installing plastic mesh with continuous miner when the roof strap they were installing fell and struck the ee's hand. No compliance issues involved. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved. The accident occurred because they were installing roof mesh and not watching where the roof strap was.

Individual was baring over a track rail on rail car when the electric winch's steel cable back lashed allowing the tail to drop 12" striking the left knee area, trained on job task.

While roof bolting on the left side of a continuous miner, the employee had pre-made a roof bolt assembly and placed it along the right rib. When the continuous miner backed up, the rib protector caught the roof bolt, which then struck the employee with some force, along the right side of the employee's head just in front of the ear and temple.

Employee was setting a shield when it pick up the adjacent shield and the pontoon came down on his right foot pinching it. It resulted in a fracture. It didn't involve any compliance issues. Employee was operating the gob side walker shield and kneeling on the pontoon, when he began to push out on the E-bar his right boot became caught between the pontoon and the e-bar. The injury resulted in a contusion. It didn't involve any compliance issues. It didn't involve job skills, proficiency, training, or attitude. No PPE or protective devices on equipment were involved.

Employee was drilling a hole for a rib pin on the left side of the 2D miner. When trying to remove the auger the employee pinched his left pinky finger between the auger and the drill chuck when he activated the control. The injury resulted in a laceration and fracture.

Employee was standing at left side bolter station of 22D miner. Miner advancing to make another cut. A foot jack pin broke, got caught in the miner tracks and was thrown out, striking the employee in the lateral side of his right foot. Caused contusion.

Employee was standing by the rear bumper of the miner cutting 3D overcasts when an unknown object came off the miner head striking him above the right eye causing a laceration and a fracture.

Bolting on left side of miner. Lost suction, lowered steel, grabbed steel to remove from head, but needed more room. Activated rotate instead of down feed and caught left hand in rotating steel. Dislocated pinky and ring finger of left hand.

The employee was assisting in the bolting operation in the belt entry when the operator running the bolter set the stab jack down on the injured employee's right foot resulting in a fracture.

After putting in test bolt, he leaned over the drill head to examine it for obstructions (065 MMU, right side bolter station of Joy Miner). He put his right hand by the bolter mast and lowered the timber jack with his left hand. The timber jack pushed the drill mast down pinching his right hand, breaking the distal phalanges of both the thumb and index finger.

Individual was bolting on the right side of the Alpine Miner. He unknowingly activated the head moving it towards the mine roof (left hand on controls and right hand on steel). This caused the drill steel to bend with the right hand between the steel and the mast. Individual received 19 sutures to the right ring finger.

Employee was installing a break away pin on the 1D Longwall when the relay bar pressurized and caught the employees right thumb between the pin and the pan line.

Lowered the base lift jack on his right foot while trouble shooting a bypass problem with a shield

Individual was checking for a leak in the ATRS system on the Alpine Voist Miner. When the power to the miner was energized the individual hand was over the hole in the hose and individual received a laceration to the left middle and index fingers.

Individual was operating an air winch with the left hand on the winch and the right hand on the control. The left hand was pinched between the winch rope hook and the winch spool. Individual received 13 sutures to the left ring finger and between the thumb and fore finger.

The injured was inspecting the sampling system at the rail road car loadout. The EE was checking secondary cutter on the 4th level and positioned his left arm inside the cutter box door. While his arm was inside the inspection door the cutter moved from right to left pinning his left arm between the cutter and the left side of the box. 2 bones fractured.

The employee got his right foot caught between the relay bar and the pontoon of a longwall shield as it was advancing.

EE was pulling shields on the 18-W longwall face. When he stepped up on the pontoon of 49 shield with the left foot and the right foot under the pontoon of the shield that had not finished it's setting cycle. EE received a fracture to the right big toe and the next toe also.

Attempting to remove a drill steel from a gopher drill when his thumb became caught between the auger and the mine roof. Laceration to right thumb.

The injured was roofbolting on 3 East Mains. The injured was dropping the drill mast on the roofbolter when the drill mast caught a long wrench, causing the wrench to flip and hit him in the right side of the face. The injured sustained a crushing injury to his right eye socket.

Dozer op. decided on his own that he was working 3:1 slope on hollow fill. The op. was told that he wasn't to get on the slope, but he did anyway. Op. backed too far down slope and went off a washout area on the next slope. Inexperienced op. caused the accident.

Employee was installing a roof jack, as he put the jack up into place the top part of the jack slid down and cut his left thumb. His thumb required 6 stitches.

employee was operating a 90 pound jack hammer to break up rock, Hammer slipped off of the rock and went through his boot causing a laceration requiring stitches and a fracture of two small bones.

Employee was struck with gopher drill while installing a roof bolt.

Handling drill steels that were in mine roof after hole had been drilled. When removing one steel from roof, the other two fell pinching his left middle finger between two steels causing a laceration and a fracture.

Employee was rib bolting at 280 block #1 entry of 9 South Mains off of the Joy 14ED25 miner. While pushing the rib drill out he felt pain in his left shoulder. There was no issue with training, PPE, mining machinery, or compliance.

Individual was bolting on the Fletcher bolter in the 16 W tail gate. When attempting to align the steels, individual pinched the left hand while applying up pressure with the mast. Individual received 12 sutures to the left palm.

He was in the process of trying to remove a bolt that stuck in the roof. When the bolt fell out it pinched his left index finger between the bolt and the bolt wrench. this resulted in a laceration to the left index finger.

Individual was assisting in moving the Cat Fletcher Bolter. He grabbed onto a rib bolt while stamping on a piece of curtain which was stuck in the cats of the bolter. The bolter moved jerking the curtain out from under his feet causing him to fall. The right hand was holding the rib strap, cutting through the glove causing a laceration on right middle finger.

Pumping water at 18 D Transfer when the airline that was hooked to the pump came loose and struck the employee in the right eye.

Attempting to lower an air drill after a cable bolt was installed when the drill spun around and the handle struck him in the head causing a contusion.

In the process of installing a bolt, when he pinched his right middle finger between two bolt plates.

EE was placing a roof channel on the timber jack of the miner and caught his right index finger in between the timber jack and roof channel causing a laceration to his right index finger.
