Employee was grinding a plate for the vibrator in the preparation plant, something flew into his eye. It hurt a little at the time, but the pain went away, so he didn't report an injury. The pain returned a week later, he then reported the injury and went to the doctor. The doctor removed a particle from his right eye.

EMPLOYEE WAS OPERATING TRUCK MOUNTED EXCAVATOR IN THE AREA OF THE CRUSHING PLANT. IT IS SUSPECTED THAT AN AIR OVER HYDRAULIC CONTROL DEVICE FAILED ON TRANSMISSION CAUSING THE VEHICLE TO GO OUT OF CONTROL AND CRASH INTO CRUSHER.
