employee was roof bolting in the belt line, he dropped the bolter head down and had his hand on bolter in a pinch point as the head came down.
Employee was roof bolting in 4 South right section when he was struck by a roof auger that bent during operation.
EE was roof bolting off the miner in #280 wall crosscut,#5-#6 entry, 3 N. left section. While reaching to pick up a drill steel that had fallen across the drill head to use in retrieving the drill steel still in mine roof, the steel in the mine roof fell & struck him on the L. hand resulting in a fracture to the metacarpal of the left little finger. Also a laceration-3 sutures.
Employee was assisting co-worker in putting a bolt in the roof. He was keeping the drill in line while pushing the bolt up when the bolt slipped off the drill head and caught his right thumb in between the bolt and the drill head.
Employee was center bolting in 4 south mains, the drill steel slipped out of his hand, came out of the hole and struck his left index finger.
Employee was pulling #27 shield over top of the shearer behind the headgate drum while standing at #26 shield when a piece of rock fell from between the shields and struck him on the calf of the left leg. He began missing work 3/5/09
EE was attempting to align the roof bolt coupler through the roof strap when the miner rocked as the operator sumped back causing coupler to bind in the strap and bend the roof bolt catching his R hand between the plant jack & roof bolt. EE was wearing West Chester Gloves. EE sustained lacerations to the index, middle & ring fingers on his right hand requiring sutures.
Employee was attempting to reposition the roof strap as he was raising the drill mast and his right hand was caught between the roof strap and ventilation tube causing a laceration to the palm of his right hand requiring 6 sutures. He was wearing Meta Miner Gloves.
Employee was walking toward the rib bolter in #2 entry, E-19 Section as the bolter operator was reeling in his trailing cable when the cable swung across the entry and struck him on his right shoulder knocking him to the mine floor onto his left shoulder and neck.
Employee was spot bolting along the belt line. He was lowering the head to insert an extension on the drill steel, and lowered the drill head onto his foot.
EE was raising the drill head to connect drill steels togather, when the srill steel caught the other steel and bent catching his left thumb between the plank jack and drill steel. The EE sustained a fracture to the left thumb.
Employee was raising a third drill steel up past the drill head to connect to steels in the hole when the drill head was lowered and caught the drill steel pushing it down on top of his right foot. Employee sustained a fracture to the 4th and 5th toes of his right foot.
Employee was helping crib the miner under the stab jack. When the stab jack was pressurized a board kicked out and struck him on the right arm.
Employee was installing a roof bolt. The bolt bent pinching his left hand between the bolt and mastjack.
Employee was using a drill steel and coupler conected to a bolt wrench to push up and tighten a bolt. The coupler broke letting the wrench fall striking him on forehead. (5 sutures)
EE lost suction on bolter after installing a 16' cable bolt. After checking auto dump for dust pre-collector to make sure it was working he shut the machine off. He then used a drill steel to hit the bottom rubber of dust box & started again. Still no suction. Shut power off & stuck hand in opening & caught fingers between flap & base. Lacerations to 3 fingers & Fracture to ring
Injured Employee was using a sinking hammer (drill) to drill holes for slope track anchors. He felt a pain in his abdomen. Dr. determined he has an abdominal hernia.
Employee was standing on #41 Shield, F-15 Longwall when a piece of rock 14" x 15" x 5" fell from above #42 shield and was thrown from the headgate drum of the shearer striking him on the nose and forehead. Employee sustained a fracture to his nose.
Employee was placing a bolt on top of an extension when the 12' rope bolt he was installing fell and caught his right little finger between the bolt and jaws of the Fletcher Bolter. Employee was wearing gloves.
Employee was operating shearer in F-15 longwall section at #160 shield when a piece of rock was thrown by the tailgate drum and struck him in the left upper lip. He received 1 suture.
Employee was standing at the E-15 longwall headgate area when a roof bolt broke/shot from the roof striking the employee in the right arm. Diagnosis as a contusion.
Employee experienced ringing in both ears from a loud pop during the cutting & burning of a rail at #2 wall, E-16 track entry. EE began missing work 6/4/07.
EE was cutting a top roller assembly with a torch to remove it from frame for the 3 north #5 head roller when the assembly fell catching his left hand between the assembly & frame resulting in fracture to left index finger. This injury later resulted in an amputation of the left index finger.
Employee was walking along the F-13 longwall face at #105 shield when a piece of sandstone was thrown from the headgate drum and struck the spray bar and caught his left little finger between spray bar and the handrail, causing a fracture to the left little finger. Employee was wearing gloves.
Employee was initiating a bank push of the panline at #4 shield, E-15 longwall section. The #3 shield slid back causing strain on the high pressure shield to shield hose, resulting in the manifold breaking off and he was struck in the left thigh by the 1 1/2" hose and coupler. The ee sustained a fracture to the left femur of the leg.
Employee inadvertantly stepped in front of #14 shield as it was advancing and caught his right foot between the shield toe and relay bar and fell forward jamming his left shoulder. Diagnosed with a hairline fracture to the 3rd metatarsal.
A longwall move, roof bolting the roof behind the shear, they were ready to set roof jack. A piece of rock fell off shield tip deflected rock from drum and cowl and it struck his left foot causing fracture.
8H longwall, injured was standing mid-shear when a piece of rock fell, hit top of shear and slid into walkway. Rock struck left foot causing fracture to left foot.
While rigging up a drill rig, derrick was in horizontal position suspended approx 1' via the gantry rigging. A gussett supporting a wire rope saddle failed allowing the line to slip off. The derrick fell the 1' onto crib blocks. EE was maneuvering the crib block at the time, intending to expose the derrick area in order to paint it. His hand was caught btwn the cribbing & derrick.
4 south right side, installing an 8' cable bolt, expander caught on strap. He attempted to free up the expander and his hand was pushed into bolter mast.
Employee was removing a pipefitting with a oxy/acetylene torch - piece fell as it was cut free, the falling piece struck the torch deflecting it into the employee's right wrist/lower arm
4 South RT. Section EE tied bolter cable up with a nylon rope.A rib strap was used to place rope behind rib strap & to tie bolter cable up. They activated the cable reel pickup the tension, cut the rope & the cable struck him in the mouth & cut his upper lip. Sutures to close wound.
Injured was helping another operator to raise bosco beams to roof. As the TRS was raising, the TRS pad with beam on it tipped causing the beam to slide off and hit his left forearm.
EE was pushing bolt up into the mine roof while bolting on the left side of the 3 north left miner. Roof bolt caught on roof strapor in bolt hole, causing the bolt to bend, catching his left middle finger between the bolt and atrs. EE sustained a fracture to the tip of his left middle finger. EE was wearing gloves.
Injured did not release pressure before removing staple while trying to add hydraulic line. It struck him in nose and left eye.
Employee was on #4 shield E-12 longwall face kneeling while pushing the panline out. The horizontal pin came out of the ram bar clevis allowing the clevis to roll catching his left lower leg between the clevis pin and clevis rail. EE sustained a fracture to the tibia and fibula of the left lower leg.
EMPLOYEE WAS OPERATING THE E-12 LONGWALL SHEARER CUTTING BOTTOM HEADGATE TO TAILGATE PASS WHEN THE SHIELD SRB WAS INITIATED THE WRONG DIRECTION. #20 SHIELD ADVANCED, KNOCKING HIM DOWN AND PUSHED HIM INTO THE PANLINE CATCHING HIS RIGHT LEG BETWEEN THE PANLINE AND SHIELD, CAUSING A FRACTURE TO THE TIBIA AND FIBULA OF THE LOWER RIGHT LEG.
Injured was helping to drill with a cobra drill when the chuck jammed. He was pulling on it when it came out. He was struck on the chin causing a laceration.
7H CAN LINE. WHILE TRYING TO GO THOUGH A MANDOOR AIR PRESSURE CLOSE DOOR ON HIS LEG CAUSING HIM TO FALL AND TWIST HIS LOWER LEG BACK. HE ALSO HAS HAD PRIOR BACK PROBLEMS.
WHILE EE WAS ATTEMPTING TO CLEAN THE SUCTION ON THE RIGHT SIDE BOLTER CHUCK OF THE CONTINUOUS MINER IN F-TAILGATE, HE WAS STRUCK IN THE RIGHT HAND BY A DRILL STEEL THAT WAS PREVIOUSLY STUCK IN THE ROOF. DIAGNOSED AS A FRACTURED RIGHT MIDDLE FINGER.
WAS DRILLING A HOLE INTO A STEEL BEAM, OVERHHEAD. HE DID HAVE SAFETY GLASSES ON.
employee operating backhoe, chain holding boom in place came off when employee moved backhoe - hitting employees mouth
7H LONGWALL WAS USING ZEE DRILL TO INSTALL SUPPLEMENTAL ROOF BOLTS. THE AUGER STUCK & DRILL LEG CAME UP, HE COULD NOT HOLD IT & HE FELL TO THE GROUND. RE-INJURED OLD INJURY TO HIS BACK & RT SHOULDER. LOST WORK DAYS BEGAN 8/23/05
Employee was on headgate end of shearer at #90 shield while the machine was backing up toward the headgate cutting up rock between the drums. The spray bar sprung out over the cable though and his right little finger was caught between the handrail and end of spray bar. Diagnosed as a fracture.
EMPLOYEE WAS OPERATING #4 TEREX PAN. THEY WERE STARTING A BENCH BY DROPPING REFUSE TO BUILD A NEW ROAD. AS HE STARTED TO UNLOAD REFUSE MATERIAL THE BACK TIRE OF THE PAN SLID OFF THE REFUSE & THE PAN ROLLED OVER.
Employee was helping install roof bolts along 7H belt for additional support. Co-worker pulled on slack hose causing the stoper drill to fall, injured tried to hold onto drill, but caught his left hand between the drill and the belts and bed rail.
INJURED WAS HELPING TO BOLT. WHILE CONNECTING DRILL STEELS, DRILL RAISED UP CATCHING HIS LEFT LITTLE FINGER BETWEEN THE DRILL AND ROOF CAUSING A LACERATION.
EMPLOYEE WAS JACKING AN OVERCAST BEAM UP TO CLEAR THE CONTINUOUS MINER DURING AN EQUIPMENT MOVE. THE JACK BAR SLIPPED OUT OF THE JACK CAUSING HIM TO LOOSE HIS BALANCE AND FALL TO THE MINE FLOOR JAMMING HIS RIGHT SHOULDER
EE WAS ROOF BOLTING IN THE LONGWALL RECOVERY CHUTE FOR F-13 IN THE LEFT 3 NORTH SECTION. WHILE POSITIONING THE CENTER BOLTER TO INSTALL A 12' ROPE BOLT ON THE LEFT SIDE OF ENTRY, HE HAD HIS LEFT HAND RESTING ON THE BOLTER CANOPY. AS HE TRAMMED THE BOLTER, HE CAUGHT HIS LEFT ARM BETWEEN THE RIB AND CANOPY.
EE WAS DRILLING BOLT HOLE & WAS HAVING TROUBLE. HE DECIDED TO CHANGE THE BIT. AS HE BEGAN TO DROP DRILL HEAD THE STEEL BECAME STUCK. 2 UPPER STEELS STAYED IN THE MINE ROOF & THE BOTTOM WAS OUT OF THE ROOF. USING THE BOLT WRENCH TO HIT THE DRILL STEEL HE HIT THE ROATION LEVER. STEEL SEPARATED, SPUN AROUND & STRUCK EE IN THE FOREARM.
