While welding, employee received a flash (arc) burn which did not affect him until later in the day. He is an experienced welder. He had no idea how it happened. There was no evidence of faulty equipment.

While drilling a hole for a 6' roof bolt, the off side bolter operator was struck in the face with a drill steel. It is not know if the steel broke or if it wasn't connected properly.

Employee was installing a 5/8 x 72" full grouted roof bolt. As the bolt was going up the hole & spinning as it went, it caught and bowed striking his right forearm.

Roof bolters arrived at bolter. Tram side operator got in tram deck to tram bolter. The opposite side bolter went to step over the cable when the operator turned the cable reel on. This caused him to fall which injured his leg.

EE was bolting on the offside. After drilling the hole, he installed a 5/8 x 6' rebar with resin 2x6x12 board & 6x6 plate. As the bolt was being shoved into the hole, it caught on something and the bolt plate struck EE's right wrist. The bolter was not rotating.

Roof bolter operator was drilling roof in x-cut #2 to #1. The steels clogged up so he removed them from the roof. He struck the starter steel on the boom of the bolter. The mud in the steel blew out the back end of the steel striking him in the left eye with mud. He was wearing safety glasses. We washed his eye & transported him to ER for further treatment.

Roof bolter operator drilled 5' hole while putting steels out of the roof, the bottom steel stuck in the head (chuck). The steel stayed in the roof, he lowered the head and pulled on the steel in the chuck with right hand. His left hand was resting on the boom. When the steel came out of the chuck, it fell striking left middle finger.

EE WAS ROOF BOLTING ON THE FLECTHER BOLTER RIGHT HAND SIDE. HE WAS RAISING THE BOOM WITH THE STARTER STEEL IN THE DRILL CHUCK. HE HAD HIS HAND ON THE STEEL AS IT TOUCHED THE ROOF. THE STEEL WA S NOT ROTATING. A PIECE OF GREY SHALE FELL & HIT HIS RIGHT THUMB. HE FINISHED THE SHIFT WITHOUT ANY PROBLEM. AFTERWARD HE WENT TO THE E.R. WHERE X-RAY REVEALED A BROKEN THUMB.

EE WAS SPOT BOLTING AN OUTBY ENTRY USING A SINGLE BOOM ROOF BOLTER WITH ATRS. HE WAS HOLDING A 1 1/8" X 36" DRILL STEEL IN HIS LEFT HAND. THE DRILL STEEL WAS APPARENTLY RESTING ON HIS LEFT FOO T. WHILE LOWERING THE DRILL BOOM THE DRILL STEEL IN HIS LEFT HAND WAS STRUCK BY THE BOOM AND PUNCTURED HIS LEFT FOOT.
