Struck by falling object

On the 002 Section in the #2 entry face, a rock fell out of the roof striking EE while he was roof bolting and knocked him to the ground. The crew was able to remove the rock and transport him to the surface while performing CPR. He was taken by ambulance to Ruby Memorial Hospital where he was pronounced dead at 1130 pm.

Accident type, without injuries

A brow in the #17 crosscut of the intake primary escapeway fell and rolled into the entry. The brow was 6' x 5' x 4' thick and impeded travel in the entry. Two (2) 4ft resin bolts were pulled out when the brow fell. After examination by a certified employee, floor to roof support, in the form of wooded cribs, were built to support the area and the rock was removed.

Employee was re-anchoring the shuttle car cable in the #4 entry between 203-204 crosscut on the MMU 002 working section when employee slipped, twisting and fracturing the right ankle.

Roof fall in 207/208 block, Number 2 entry. Fell on the Continuous Mining Machine when Miner operator was removing Miner from area. Miner had to retrieve with hydraulic retriever.

Struck against a moving object

While riding out on the mantrip, the driver hit a crib causing employee to strike nose on the mantrip.

While sounding and scaling the roof, employee drew hand back quickly to avoid a rock that was dislodged straining right shoulder.

Intersection in #1 heading fell inby the tail piece over anchorage over the weekend while no one was underground.

Struck against a moving object

While operating a rubber tire mantrip the steering wheel jumped out of employee's hand. Tried to stop wheel by grabbing it. The wheel hit right index finger breaking it.

Fall in return discovered in #8 heading 1/2 block out by spad 2100. Operator immediately ceased mining activity when the fall was discovered. This was an unattended area of the mine.

Intersection in #4 Entry at 199 block fell in. The fall extended into the #4-5 crosscut before the continuous miner was able to retreat from the mining area.

#90 crosscut 3-2 and straight of #3 Roof Fall in Primary Escape way. The fall is approximately 70' long, 5' high, and 17' wide. All approaches to the fall area were immediately timbered, and the escape way was rerouted. The fall will not be cleaned up.

While mining in the number two entry Cont. Miner off side panic button malfunctioned. Not allowing the machine to move. While immobile, the inby pillar cut fell in to the turn posts. No rock fell on the miner. After making electrical repairs, the miner was able to remove itself from the cut.

Accident type, without injuries

A roof fall occurred out-by the 002 section on #11 belt between #208 and #209 x-cuts. The fall was 16 feet wide by 15 feet high by 45 feet long. The fall was reported because it was above anchorage. Approaches were re-bolted and cable bolted. Fall was cleaned up and a square set was installed according to our roof control plan.

A roof fall occurred in the 2 tol x-cut at #260 block outby the working section. Both sides of the x-cut had been cribbed and intersections were bolted with additional cable bolts. The x-cut was deemed bad long before it fell. It was cribbed off already. It will not be cleaned up.

#8 Belt tore apart. EE and others were repairing the belt. While cutting the belt to make a splice, the knife slipped out of EE's hand and cut his right lower thigh. The foreman, EMT, administered first aid, and EE went to ER to have it checked. He got 3 stitches, but the doctor said next time he did not have to come in for a cut like this.

While shoveling the #14 belt tailpiece, employee pulled something in the middle of his back. He finished the shift and it began to hurt worse on the ride outside.

Accident type, without injuries

On the 1 North Mains in the #4 face of the 002 Section, the continuous miner cut into an uncharted borehole which was an 8"" pipe used as a power borehole to the former Humphrey mine. The pipe was sealed accordingly to plan and the area was made safe. There were no injuries.

A roof fall occurred in the #7 entry 1 block outby last open x-cut in an intersection. The fall was 18'x18'x9'high. No people or equipment was involved. All sides were cribbed and dangered off. The fall will not be cleaned up. We are now installing 6'/12' cable bolts in intersections until we reach x-cut #251.

Employee was running the miner making the initial cut in #2 to #1 x-cut. A small rock (less than 10 pounds) struck his hat then slid down his face cutting his cheek and lips on the left side of his face. The rock was very sharp. The area was fully roof bolted.

EE was positioning the drill station on tram side of bolter when a rock fell from between 2 rows of bolts. His ATRS was set and his canopy was up. He had his left hand on the edge of the drill steel tray when a small rock fell and struck his left hand. He was wearing metacarpal gloves.

Struck by falling object

Employee installed his 1 & 2 roof bolt on operator side. When he lowered his canopy, he started to clean rock off of it. A small piece of rock fell from the roof and cut his left forearm.

Over-exertion in pulling or pushing objects

Two workers were pulling a cable off of buggy reel when EE felt a pop in his back. He did not begin to miss work until 1/21/13.

Flash burns (welding)

While welding, employee received a flash (arc) burn which did not affect him until later in the day. He is an experienced welder. He had no idea how it happened. There was no evidence of faulty equipment.

A roof fall occurred in the #2 entry at spad #3074. The mine was idle. The watchman discovered the power had gone off underground. Upon inspection the fall was discovered and reported. It was dangered off, cribbed 3 sides and posted on the front side. It will not be cleaned up. It was 30-35 feet long, both ways and 18-22 feet long ways. It was 16 feet high.

While drilling a hole for a 6' roof bolt, the off side bolter operator was struck in the face with a drill steel. It is not know if the steel broke or if it wasn't connected properly.

The #4 entry intersection 40' outby the face fell 18' X 18' X 12'. The #2 Roof Bolter was in by the last open X-cut when it fell entrapping the machine at the face. The bolter operators escaped without injury. There is no rock on the roof bolter. The fall will not be cleaned up. We will back cut the #4 entry to retrieve the roof bolter.

Roof bolters were going to bolt #4-5 x-cut. 2 rows outby the cut, they stopped to spot 2 or 3 bolts. After installing the first bolt near the rib, the left side operator swung head to spot another bolt. After drilling the hole, he was removing steels when a piece of rock hit the canopy, broke, & part of it came under canopy cutting his left arm with sharp edge.

Over-exertion in lifting objects

Employee did not report until 12/27/2011 - While assisting lifting covers of RRII roof bolter he thinks he hurt his lower back. It did not start bothering him until 12/25/2011. He has had previous back injuries with previous employers.

After scaling the roof in the #2 to #3 x-cut 1 block outby the 002 section tailpiece, the roof bolter operator trammed the bolter forward to spot bolt an already bolted area. He set up the ATRS, crawled under the operator side canopy and then a rock fell striking him on the back, neck, & shoulders.

The crew was moving the miner from #5 to #3. The buggy men were assisting with the miner cable. EE bent over to move the cable when a rock fell from between bolts knocking his hat off. A second rock fell striking him on the back of the head. He fell to the floor and was administered care after being moved from the spot where it happened.

In the #4 Entry 1 block outby spad #3018, a roof fall occurred that was 18' wide x 18' long x 7' high. The area was dangered off, cribbed on all 4 sides, and cable bolts were installed for 1 block outby and 1 block inby the fall area. The fall will not be cleaned up.

A roof fall occurred at spad # 2335 in a 3 way intersection. The area was bolted with 5' & 6' full grouted rebar. It was 5'-6' high X 16' wide X 40' long. It was dangered off and cribbed. It will not be cleaned up.

The injured EE was helping another EE get posts for wedges for 2nd mining. As EE was unloading wedges from a wagon the scoop being operated by the other EE accidentally bumped the wheel of the wagon which caused it to hit EE's left knee. The scoop operator thought he was far enough away from the wagon.

A roof fall occurred in the #2 entry 1 block inby spad #2942 in an intersection. The fall was 6' high x 17' long x 17' wide. There were no witnesses nor equipment involved. The area was breakered off on all 4 sides. It will not be cleaned up. This was outby the section tailpiece.

EE was helping crew un-hang 1 1/2"" water line hung up to roof bolt plates with basket hangers. When taking the hose out of the hanger, he cut his left ring finger on a roof bolt plate. He was not wearing his company provided meta carpal glove.

A x-cut fell 17'wide X 46'long X 6'high. It was already cribbed on each end. The fall was examined and dangered off and reported. No further cribbing needed. It will not be cleaned up. It was in #7 to #8 x-cut near spad #2049 and #2050. It was in an outby return.

Struck by falling object

Helper side roof bolter was installing a new row of bolts. As he was swinging drill station toward the rib to install the #1 bolt, a piece of rock hit the canopy, then hit his back and right leg, knocking him to the floor.

In the #4 entry one block outby, face EE was assisting electricans with a cable splice. The razor knife slipped and cut him on the left knee.

While winding belt on a pull with scoop, the scoop bucket was swing toward this employee striking his left foot.

A continuation of the roof fall reported on 3/17/10 occurred. The fall started at previous fall and traveled 100' outby existing fall. The fall was 16 1/2 feet wide and 6' high. The area was bolted with 5/8 X 72"" fall grouted rebar with 6X6 plates and 2X2X12 cap boards. This area cribbed and dangered off.

A roof fall was discovered in the #0 entry in the #280 intersection. The fall was 40' long x 18' wide x 6' high. No persons or equipment involved. The 3 sides were cribbed and it will not be cleaned up. This entry was already stopped.

Struck by falling object

Foreman and roof bolter were scaling loose rock. A rock hit the panic strip. While trying to free it a piece of rock fell hitting the roof bolter operator on the shoulder and arm.

EE was completing a stopping in the #9 entry outyby the 002 section. He stumbled over some concrete blocks. While falling, he caught himself with his left hand. He completed the stopping and finished his shift.

Struck by falling object

While bolting, the operator had a rock fall from the roof, striking the canopy and then it hit his right foot just behind the metatarsal part of his boot. The rock weighed 25 lbs. He was operating the helper side of the RRII roof bolter.

The intersection in the #1 entry 1 block inby spad #2259 fell. 21' long x 18' wide x 6' high. Fall area was cribbed & dangered off. It will not be cleaned up.

EE section foreman was examining the working #5 face. He reached down to move the miner waterline and raised up striking his left ear on a cable bolt.

A roof fall was found in the 2-1 x-cut at #217 block near spad #2298. It fell between intersections. 50' long x 17' wide x 8' high. It was mined 3 months prior. It was bolted with 5/8 x 60"" & 72"" fully grouted bolts. The area was timbered & dangered off. It will not be cleaned up.

Intersection fell from the face which was extened 10' beyond the last open xcut to 5' outby the last open xcut, Fall was 30' lomg x 17' wide x 8' high. The area was bolted with 5/8x72 fully grouted rebar and 12x12 plates. The following was dangered off and cribbed. It will not be cleaned up, the fall occurred between 5 lifts.

Injured was found int he #5 entry on the 002 section in the last open x-cut intersection. He was unconsious. EMT and crew adminstered first aid and transported to surface. When he regained consciousness, He stated that passed out, falling to the mine floor and hurt his shoulder. It turnes out that he had a reaction to medication.

Employee was installing a 5/8 x 72"" full grouted roof bolt. As the bolt was going up the hole & spinning as it went, it caught and bowed striking his right forearm.

EE was loading 9X9 roof bolt plates onto the mantrip, when he stumbled over a 2X6X12 cap board and fell, injuring his left shoulder. He had previously injured this same shoulder. This was his second work related injury in two months.

While roof bolting, EE hit his knee on the drill head of the #1 roof bolter.

EE was operating the #1 scoop. He stopped, turned scoop off and left the operator's compartment. While getting out, he pulled something in his left knee.

Fall onto or against objects

Roof bolters arrived at bolter. Tram side operator got in tram deck to tram bolter. The opposite side bolter went to step over the cable when the operator turned the cable reel on. This caused him to fall which injured his leg.

Employee was lifting the rub rail on the miner. He pulled his back. He finished the shift. As he left the lamp house, he told his foreman he hurt his back. That is all the information he gave or has given.

A 3-Way Intersection fell 8' - 9' high by 18' long by 20' wide. the area was bolted with 5/8 x 72"" fully grouted rebar with 6 x 6 plates and 5' x 13' straps. The area had been inspected 2 hours before the fall. It started to bow and work. The foreman reported it at 4 pm. It was dangered off and cribbed. The fall will not be cleaned up. Bad geology was the apparent cause.

Employee was constructing a stopping with 8"" solid blocks. As he lifted one of the blocks, he felt something hurting in his back. He finished the shift. On 8/12, he went to the ER and was treated for a lower back injury. He is still undergoing treatment.

A roof fall occurred in the #9 entry at spad # 453 which was a 3-way intersection. The fall was 20' long x 17' wide x 5' high. The area was bolted with 4' & 5' full grouted roof bolts. The fall was dangered off & timbered all sides. It will not be cleaned up. No persons or equipment involved.

Crew was setting posts in preparation for new bleeder system. Employee said sometime during the night he pulled something in his neck or back. He finished the shift and reported it to the shift foreman. He has not worked since 4/23/07.

A roof fall occurred in the #10 entry inby spad #1866 which is outby the working section. The fall was 80'long x 17' wide x 8' high. The area had been dangered off prior to the fall. The area was bolted with 5/8 x 6' full grouted rebar and spot bolter with 12' cable bolts. The area was timbered off on all sides and dangered off.

A roof fall occurred at spad #1868 in the #7 entry outby the working section. The fall was 8' high x 25' long x 20' wide. It was bolted with 5/8 x 72"" fully grouted rebar and spot bolted with 12' cable bolts. The area was dangered off and all sides timbered off.

EE and Another ee were at the face. He was having trouble with the miner remote, while they were looking at it, a piece of rock lit his hat. It was 1'x1'x2"". The rock fell between roof bolts. Later in the shift he said his neck was hurting.

EE was bolting on the offside. After drilling the hole, he installed a 5/8 x 6' rebar with resin 2x6x12 board & 6x6 plate. As the bolt was being shoved into the hole, it caught on something and the bolt plate struck EE's right wrist. The bolter was not rotating.

A roof fall occurred in the #10 entry back cut measuring 30' long X 17.5' wide X 8' high. The face had a 12' cut unbolted. When it fell it caused the intersection to fall also. The intersection had 5/8 x 60"" bolts. The area was cribbed off and permanently dangered off.

In the #9 entry 1 blk inby spad #5148, a roof fall 8'h x 25'lx17'w occured. The area was mined 4 years ago, it is not a normal travelway. The mine examiner noted a chg there week prior to the fall and set breaker posts & dangered it off expecting a fall soon. The area was bolted with 5/8 x60"" resin bolts with 8x8 plates.

A roof fall occurred at #197 intersection in the #2 entry 1 block outby spad#1674.Foreman observed intersection was working. He dangered the area off, moved life line to adjacent outby, built crib as breakers & Cribbed adjacent interesection The area was bolted with 5/8 x 72"" full grouted rebar using 6 X 6 plates & 2 X 6 X 14 boards. The fall was 50' long x 17' wide x 6' high

Employee was walking in the last open x-cut when he slipped on wet pavement and twisted his left ankle. He did not seek first aid. He drove himself out of the mine and went to the ER with his wife driving.

On the 002 section, in the #3 entry, one block inby spad # 1764, an intersection fell. 18'wide X 35'long X 7.5'high. The area was bolted with 3/4 X 72"" fully grouted bolts. The roof was laminated slate & sandstone. This fall occurred over an idle weekend. The area was dangered off & will not be cleaned up.

Roof fall 35' long x 12' wide x 6' high in the #7 entry at speed #5157. This area was mined 4-5 years ago. The roof had drippers and had been olted with 5' resin bolts and 9' instal II bolts. The area was timbered and permeanently dangered off.

The #12 entry had streaks of sandstone & shale in the roof. The area was bolted with 5/8 x 72"" fully grouted rebar & boards. The entry has been discontinued. The fall area is dangered off & timbered.

Struck by flying object

Roof bolter operator was drilling roof in x-cut #2 to #1. The steels clogged up so he removed them from the roof. He struck the starter steel on the boom of the bolter. The mud in the steel blew out the back end of the steel striking him in the left eye with mud. He was wearing safety glasses. We washed his eye & transported him to ER for further treatment.

Miner operator had finished loading shuttle car. After the buggy went around the turn the miner operator stood up & walked backwards. While doing this, he fell backwards stumbling over a lump of coal. He was given first aid by EMTs and transported to surface. He was then taken by ambulance to Ruby Mtn. Hospital.

The intersection one bock inby spad #1080 in #8 entry fell 5' high x 18' long x 20' wide. The area was bolted with 5/8 x 60"" full grouted rebar. No known remove for this fall. Area dangered off, set breakers, and will not be cleaned up.

The intersection 1 block inby spad #1677 fell 18' long x 40' wide x 12' high due to wet roof bad geology & subsidence. The area was bolted with 68x72"" rebar with 6x6 plates + 2x8x14"" boards. The area had been spot bolted with 12' cable bolts. The fall will not be cleaned up. All sides cribbed & permanently dangered off.

The intersection at #195 block just inby spad # 1657 fell. 18' long x 18' wide x 7' high. It was bolted with 5/8 x 72"" full grouted rebar bolts with 6"" x 6"" plates & 2"" x 6"" x 16"" boards. Bad geology & subsidence caused this fall. It is permanently dangered off & timbered. It will not be cleaned up.

#8 entry intersection 1 block outby spad #1657 fell 18' wide x 16' long by 5' high. The area had fallen out some on initial mining previously. The area was dangered off & cribbed. It will not be cleaned up.

#191 X-cut between #4 and #5 entries fell. 16 1/2' wide X 40' long and at leat 5' high. The area had bad geologic conditions. The X-cut had been cribbed at each end and dangered off before it fell. It was bolted with 5/8 X 72"" full grouted resin bolts. This will not be cleaned up.

Roof bolter offside operator on the offside was installing 1st bolt in the row they were bolting. An overhanging brow inby his row fell and struck his right foot. It cut his boot, but he kept bolting. He decided to go to ER to have it checked out.

Initial mining was OK. The area (#155 x-cut 5 to 6) was bolted with 5' resin bolts & cap boards. A few months later, we timbered the edge of each side of the x-cut. We dangered off the entrances to the x-cut. There was no persons or equipment involved. The area was 40' long, 16' - 17' wide and 4' - 5' high. The fall will not be cleaned up.

Employee was shoveling #1 beltdrive to transfer area when he pushed the shovel under the belt to shovel, he felt something in lis lower back glue on strain. he finished the lift, went home, then went to er.

While examining returns & bleeders, I found a roof fall on the 5 north 1 left panel in the x-cut from #6 to #12 entry at the end of this panel. The fall is 400' long, 16-18' wide, 8'10' high. The area has been dangered off and breakers set. It will not be cleaned up and is not obstructing air or travel.

Xcut #9 to #10 had a cutter in the middle of entry. It was bolted with 6' fully grouted resin bolts & 12' cable bolts. The roof was wet. We dangered it of on 10/11/05 & built cribs at the edge of each side. When it fell, it was 62' long x 17' wide x 8' high. No one or no equipment was involved. It will not be cleaned up.

The #5 entry was driven in approx. 90'. Last 15' to 20' was unbolted. Midnight shift had dangered it off & reported that add'l bolting needed to be done outby the cut. Day shift was installing 10' bolts with straps 30' from fall area before it started working. Entire entry had been bolted with 5' & 6' full grouted 5/8 resin rods. Fall was approx 60' long x 10' high x 18' wide.

Employee was shoveling #10 belt tailpiece. He turned belt off to cross over tailpiece. As he put his right leg up on tailpiece, his left knee twisted. This employee has a history of unusual injuries.

DISCOVERED A ROOF FALL IN #6 ENTRY, ONE BLOCK FROM THE FACE ON THE 002 SECTION. THE FALL IS 9'H X 17'W X 20'LONG. IT WAS A 3WAY INTERSECTION. IT HAD BEEN BOLTED WITH 6'RESIN BOLTS & SPOT BOLTED WITH 9'INSTALL BOLTS. THE AREA WAS DANGERED OFF, BREAKERS SET & WILL NOT BE CLEANED UP. NO PERSONS OR EQUIPMENT WERE INVOLVED.

x-cut 5 to 6 led cut thru the cut was a 10' cut. the roof had fallen out 2' above the coal seam. bolter were installing 6' resin bolts with boards. right side bolter operator was prying a borw in his side with a slate bar. the section forman was beside him. when the piece he was prying fell, more rock and coal fall from the rib. a small strippies of rock struck left forearm

Scoop operator was transporting supplies from surface to section. At #134 x-cut in intake #3 entry a rock fell between roof bolts striking the scoop operator's left leg. He was running the scoop in forward direction.

Roof bolter operator drilled 5' hole while putting steels out of the roof, the bottom steel stuck in the head (chuck). The steel stayed in the roof, he lowered the head and pulled on the steel in the chuck with right hand. His left hand was resting on the boom. When the steel came out of the chuck, it fell striking left middle finger.

EE WAS GOING FROM CROSSCUT TO CROSSCUT GATHERING SUPPLIES. HE WAS DRIVING A 3-WHEEL MANTRIP. WHEN HE PULLED TOWARD THE INTAKE STOPPING, HE OBSERVED A ROLL OF BELT WITH A METAL TUBE STICKING OUT. THE INJURED PERSON WAS ASSISTING THE SKID STEER OPERATOR WITH MOVING PALLETS. WHILE MOVING THE LAST PALLET, HE PLACED HIS HAND ON THE PALLET THE DAE WOO OPERATOR WAS SETTING DOWN ON THE PAVEMENT. HE CAUGHT HIS LEFT INDEX FINGER BETWEEN THE PALLET AND THE FORK ON THE DAE WOO.

The intersection of #2 entry at #172 block (1 block inby spad #1136) fell. 18 x 18 x 6' high. the area was bolted with 5' & 6' super twist bolts. It also had 2-4 10' cable bolts in it.

ROOF BOLTERS AND SUPPLYMAN WERE LOADING SUPPLIES ON ROOF BOLTER. THE OFF SIDE OPERATOR WAS PUTTING BOLTS FROM SUPPLY CART ONTO HIS SIDE OF THE MACHINE WHEN HE FELT A TWINGE IN HIS BACK. HE STATED THE AREA WAS LOW AND HE WAS LIFTING NORMAL WEIGHT / LOAD. THE EMPLOYEE FINISHED THE SHIFT AS USUAL.

SHUTTLE CAR OPERATOR WAS LEAVING THE MINER WITH HIS BUGGY LOADED WITH COAL. AS HE MADE A LEFT TURN, THE WHEEL UNIT ON THE DUMPING END, REEL SIDE OF SHUTTLE CAR BROKE OFF. THE BUGGY STOPPED ABRUPTLY CAUSING THE OPERATOR TO PITCH FORWARD & TO THE LEFT. THIS CAUSED HIM TO STRIKE THE LEFT SIDE OF HIS HEAD INTO THE CONTROL AREA OF THE SHUTTLE CAR.

AREA WAS FULLY BOLTED WITH 4', 5', 6' RESIN BOLTS & A FEW 9' BOLTS. THIS AREA IS ABANDONED AND TRAVELED ONLY BY MINE FOREMAN/FIREBOSS.

HEAD ROLLER OF DIRTY COAL STACKER BELT HAD MUD BUILDUP ON IT. EMPLOYEE WAS INSTRUCTED TO CLEAN ROLLER WITH BELT OFF. HE DID SO FOR A FEW MINUTES THEN TURNED THE BELT ON AND USED A SLATE BAR TO SCRAPE THE ROLLER. THE BAR CAUGHT IN THE ROLLER AND STRUCK HIM ON THE CHIN.

EE WAS MAKING UP BELT SADDLES WITH SPLICES ON BOTH ENDS. HE WAS USING A HAMMER TO SRIKE THE SPLICING TOOL, WHEN HE STRUCK THE SPLICING TOOL WITHA HAMMER A PIECE OF STEEL CAME OFF THE HAMMER HE AD AND ENTERED HIS ARM 2"" ABOVE THE WRIST. THE PIECE OF STEEL WAS VERY SMALL.

EE WAS ROOF BOLTING ON THE FLECTHER BOLTER RIGHT HAND SIDE. HE WAS RAISING THE BOOM WITH THE STARTER STEEL IN THE DRILL CHUCK. HE HAD HIS HAND ON THE STEEL AS IT TOUCHED THE ROOF. THE STEEL WA S NOT ROTATING. A PIECE OF GREY SHALE FELL & HIT HIS RIGHT THUMB. HE FINISHED THE SHIFT WITHOUT ANY PROBLEM. AFTERWARD HE WENT TO THE E.R. WHERE X-RAY REVEALED A BROKEN THUMB.
