Employee stated that EE was using a cut off wheel to cut off a section of an exhaust pipe. The wheel broke causing a piece of the wheel to fly off and strike the employee's chin. No lost time incurred but reportable due to requiring sutures.

Employee stated that he was rebuilding a dozer blade. He had been grinding on, welding on, and air arcing on the blade during the course of the work shift. He noticed a foreign body in his left eye toward the end of the shift. Had to get removed and missed next two work shifts.

Employee stated that he had repaired the air compressor. He turned it on so as to build up the air pressure. The pop off valve released air pressure which blew something out and into the employee's right eye. Went to doctor, nothing found but prescription given for antibiotics for eye.

EE STATED THAT HE WAS USING A TORCH TO CUT OFF A BELT PIN ON AN OVERHEAD CONVEYOR BELT. AS HE WAS CUTTING THE PIN, A PIECE OF METAL FLEW INTO HIS LEFT EYE. WENT TO DR. ON 2/22/03 TO HAVE FOREI GN OBJECT REMOVED FROM HIS LEFT EYE. NO LOST WORK DAYS INCURRED BUT REPORTABLE DUE TO EMBEDDED OBJECT REMOVED BY DR.

EE WAS HELPING REPAIR CLEAN COAL BELT. HE WENT TO CUT OFF A FEW PIECES OF ANGLE IRON. WHILE USING CUTTING TORCHES TO CUT THE ANGLE IRON SPARKS FLEW UP INTO HIS FACE. A FOREIGN BODY LODGED IN H IS LEFT EYE. HE WENT TO THE DOCTOR TO HAVE IT REMOVED & HAD ANTIBIOTIC OINTMENT ADMINISTERED.
