Employee stated was weed eating around a groundwater well when it slung some type of insect onto EE's hand. It bit the back of EE's left hand. No symptoms appeared until the 15th. Did not go to doctor until the 17th due to swelling/redness in the left hand/arm. Given antibiotics and no lost time was incurred.

Employee stated was replacing and wiring up a motor in the clean coal tunnel. Some rust/dust was blown into EE's right eye. Had to go to doctor to get removed. Removed foreign bodies from eye. Rx for antibiotic drops. Returned to work the next day with no lost time incurred.
Employee stated that EE was clearing out trees/brush for power line. EE's left foot slipped on the bank causing EE to fall onto the ground. EE hit the right side of EE's face against a stob causing 2 small lacerations above and below EE's right eye. This is a no lost time accident but reportable due to requiring sutures.

Employee stated that EE was getting off of dozer. The push beam was up on jacks. As EE stepped down off of push beam step, EE felt EE's right knee buckle when EE stepped onto the ground causing pain in EE's right knee aggravating a previous injury.

Employee stated EE was using a wood stick to dig mud out of loader bucket. As EE was jabbing the stick, a splinter embedded into EE's left hand between thumb and index finger. The area became infected, went to doctor on 6/25/20 to get checked. The splinter had to be surgically removed. Became reportable on 6/29/20 due to lost time and removal by doctor.
Employee was helping remove a discharge chute out of the plant. Using chain hoists to move toward doorway. Chain slipped causing the chute to shift/swing toward employee. It struck the right side of employee knocking them onto the floor resulting in pain to right shoulder and right knee.

Employee stated that they were going to fuel the hauler. They had put the fuel nozzle into the tank to fill. As they turned to go and start the pump, the nozzle came out and struck the employee's right wrist. Went to ER, x-rays negative, but put into sling and on restrictions.

Employee stated that EE was clearing out brush from underneath a power line right-of-way. As EE was pulling brush, a thorn punctured/embedded into right ring finger. Became infected and went to doctor on 3/29/17 to have checked. Doctor had to remove embedded object and treat infection.

Employee was getting down into wash box. Foot was on a step inside the wash box & left arm resting on top of wash box overflow. As EE stepped on down, EE drug arm down, and a sharp piece of metal on the edge of the box lacerated the underside of forearm. Required sutures.
Employee stated that was servicing a dozer. EE reached to loosen fuel filters and twisted left knee. Immediate pain but lessened with time. Kept working. Went to doctor on 5/23/16 to get checked. Never missed time/work until 7/15/16 when EE had orthoscopic surgery on knee and incurred lost time.

Employee stated that he was patching the supply pipe to the distribution box on the #70 screen. The ladder slipped causing the employee to lose his balance and fall onto the vibratory screen. This resulted in a right sprained ankle and bruising to the left leg/groin area.
Employee stated that he was bending down to pick up a coal sample. As he did, he felt pain in his lower back area. He continued to work but finally went to doctor on 6/19/2015 to get checked. Doctor has him on some restrictions. No lost days but reportable due to the restrictions.

Employee stated that he was putting a 10 inch pipe clamp on the return line to the heavy media cyclone sump. As he was installing the clamp, he was reaching overhead and in an awkward position when he felt pain go down his right leg. Went to Doctor to get checked. RTW with some restrictions.
Employee stated that he was driving a tandem coal truck hauling coal to the prep plant. As he was starting down a grade, he down shifted to a lower gear. He stated that it came out of gear & truck started to gain speed. As he proceeded downgrade, he applied brakes to slow the truck down but as he entered a curve, the truck turned over onto its right side causing multiple injuries.

Carrier bearing that holds drive line (shaft) broke allowing drive line (shaft) to move rupturing air line that goes to the brakes causing brakes to fail. At this time the truck (International Truck #15) begin rolling backwards. Employee jumped out of the truck landing on his back injuring shoulder (right) & his back.
Employee was standing behind foreman as he was cutting the seal/bearing off of shaft on magnetic separator. Something caused pressure behind the end cap on the separator. Before finishing the cutting, the end cap blew off, glanced off of a nearby pipe and struck the injured employee in the forehead resulting in a laceration and fractures to facial area.
Employee had been welding on the rear cross member of main frame on a hauler. He had finished welding and said he was in the process of getting down the ladder when his arm let go and he fell onto the ground striking his head against the ground incurring a concussion and also had shoulder pain.

Employee stated that he was removing the track adjuster cylinder off of the track adjuster rod with a sledge hammer. The hammer glanced off the side of the cylinder and struck his left leg below the knee. Went to doctor and missed one shift work.
Employee stated that he was reaching up to open the crusher on the sample system at the strip dump. As he was pulling down on the lever to open it up, he turned his left ankle.
Employee alleges coal worker's pneumoconiosis. Employer was notified by letter dated 1/10/12 and was received on 1/12/12. The employee went off work in October 2011 on short term disability for medical reasons and is still currently off work.

Employee stated that he was removing the lower seal retaining cap from the upper hitch pin assembly. The retaining cap slid down the bar onto his right hand causing laceration to top of right hand between thumb and index finger requiring sutures and restricted days work.
Employee stated that he was climbing the ladder to get onto a coal loader so as to pull it out of the shop. As he reached the top, he felt a pop and burning sensation in his right shoulder.
The individual (self-employed) was not an employee of Red River Coal. He was in the process of salvaging scrap metal from the mine site. He complained of heartburn earlier and became nauseous and sat down. He fell over with an apparent heart attack. Transported to hospital where he was pronounced dead.
Employee stated that he was using a utility knife to cut the fan belts off of a loader. As he was cutting, the knife slipped causing him to lacerate his little finger (5th digit) on his right hand. Reportable due to requiring sutures.
Employee stated that he was holding the vibrator screen upright with his left hand while using a utility knife to cut the rubber tips off the side of the screen with his right hand. The screen slipped causing his left hand to move to the right and the knife came down and lacerated his left ring and small fingers. Reportable due to sutures.

Employee stated that he was mounting the left rear inside tire onto a mechanic's truck. While mounting the tire, the employee's left ring finger got caught between the tire rim and the hub severing the tip of his left ring finger.
Employee stated that he was helping to replace a screen on the #27 vibrator. He bent over to lift the screen out when he felt pain in his lower back area.
Employee stated that he was cranking the handle on the leveling jack on the utility trailer, so as to unhook it from the dump truck, when he felt pain in his lower back area. He went to doctor on 8-13-2010 and incurred lost workdays starting with that date.
Employee stated that he was working on unstopping a pipe from the thickener tank to the underflow pumps in the prep plant. He was pushing a hose with water pressure into the pipe so as to unstop it. He felt a burning sensation in his right groin area. Reportable when he went to doctor on 7/26/10 and diagnosed with a hernia.
Employee stated that he had just checked the oil levels in the motor grader and was starting to get into the cab when his foot slipped off of the tire and he fell approximately 5 feet onto the ground injuring his right shoulder.
Employee stated that he was using a chain hoist to reposition the flop gate on the main conveyor belt. As he was pulling on the chain hoist, he felt a pop in his right shoulder. Went to doctor on 3/17/10. Continued working but became reportable when lost time was incurred on 6/9/10 due to having surgical repair on right rotator cuff.

Installing powertrain oil cooler on dozer and felt burning pain in right shoulder. Right shoulder strain.
Employee stated that he was helping remove a bogie pin from a dozer roller frame. He was holding a bar while another mechanic was using a hammer to knock out the pin. The bar bounced off and caught his left little finger between the bar and the roller frame, resulting in a fracture to the tip of his little finger.
Employee stated that he was rebuilding a dozer blade. He had been grinding on, welding on, and air arcing on the blade during the course of the work shift. He noticed a foreign body in his left eye toward the end of the shift. Had to get removed and missed next two work shifts.
Employee stated that he was picking up a five gallon bucket of oil. The bucket was frozen to the ground. He pryed it loose, picked it up and twisted as he put it in the pickup. He felt pain in his lower back area.

Employee stated that he was getting up onto the road grader. He grabbed hold of the hand rails to pull himself up the ladder. As he did, he felt a pop and had pain in his right shoulder.
Employee stated that he was working inside the raw coal screen. As he positioned himself to install a piece of rubber belting on the back of the screen, he twisted his right knee and felt it pop as he knelt down. Reportable effective 2/18/08 due to restricted work. Lost time on 2/22/08.

Employee stated that he was doing various jobs during the day. He wiped his eye area with his shirt sleeve throughout the day. He noticed a foreign body in his left eye that night. Tried to remove but couldn't. Went to ER doctor to have removed. Removed imbedded object and used antibiotic drops.
Employee stated he was removing belt wiper from conveyor belt. As he was pulling out one end of the belt wiper, the other end fell causing his end to pin his right arm between the belt wiper pipe and the brake rollback arm. Resulted in mashing type injury with a laceration (sutures).
Employee stated that he reached down to move a box with a hydraulic pump inside. The box slipped off the pump causing employee's left middle finger to slide against the flange on the pump lacerating top side of left middle finger (sutures).
Employee stated that he was replacing steel plate on back of wash box. As he stepped up onto a steel beam, he hit the top of his head on a 3 inch steel pipe. This pushed his hard hat down onto his glasses cutting his nose, bruising his right eye, and causing mild concussion.

Employee stated that he was delivering parts to the job site in the parts truck. The truck hit a pothole in the haul road causing the employee to be jolted in the truck. He said he felt pain in lower back area and down his right leg.
Employee stated that he was trying to dislodge rocks in the crusher so as to unclog the crusher. He placed his left hand on the top of the chute to the crusher for balance. A large rock came loose and struck his left thumb as it laid on the metal chute, severing the tip of his left thumb.
Employee stated that he had delivered parts to the job. He was coming down the road from the job when his parts truck hit a pot hole in the road. The truck jarred him causing low back pain. Reportable on 7-31-06 when lost time was incurred.
Employee stated that he had repaired the air compressor. He turned it on so as to build up the air pressure. The pop off valve released air pressure which blew something out and into the employee's right eye. Went to doctor, nothing found but prescription given for antibiotics for eye.
Employee stated that he was bent over the fine refuse discharge chute attempting to pull out a vibrating screen panel. As he was bent over, he attempted to lift out the screen. When he did, he felt pain in the lower back area.
EE STATED THAT HE OPENED THE DOOR ON THE HEAVY MEDIA CYCLONE UNDERFLOW BOX TO ADD SOME CERAMIC BEADS. WHEN HE DID, HE FELT A SUDDEN SHARP PAIN IN HIS LOWER BACK WHICH RADIATED DOWN HIS LEFT LEG (STRAINED BACK).

EE STATED THAT HE WAS ASSISTING IN SETTING AN ENGINE BACK INTO A DOZER. HE HAD HIS HAND ON THESPREADER CHAIN WHICH HELD THE ENGINE ONTO THE BOOM OF THE TRUCK. AS HE WAS GUIDING THE ENGINE IN T WHICH CAUGHT AND TWISTED THE EE HAND CAUSING FRACTURE TO LEFT THUMB.
EE STATED THAT HE WAS USING A TORCH TO CUT OFF A BELT PIN ON AN OVERHEAD CONVEYOR BELT. AS HE WAS CUTTING THE PIN, A PIECE OF METAL FLEW INTO HIS LEFT EYE. WENT TO DR. EE STATED THAT HE PICKED UP A WIRE ROP SLING OUT OF THE FLOOR AND WAS USING METAL ROD TO HANG IT ON THE WALL IN THE SHOP AREA. THE WIRE ROPE SLID DOWN THE ROD, SRIKING THE RIGHT MIDDLE FINGER WITH A WIRE STRAND EMBEDDED IN FINGER. DR REMOVED WIRE STRANDS AND SUTURED WOUND. NO LOST TIME BUT REPORTABLE.

EE STATED THAT HE WAS CHANGING OUT THE TOP SCREEN ON THE VIBRATOR DECK. HIS FEET SLIPPED OUT FROM UNDER HIM CAUSING HIM TO FALL/SLIDE DOWN OFF OF THE SCREENS. HE LANDED ON HIS LEFT KNEE. NO LO ST TIME WAS INCURRED BUT IS REPORTABLE DUE TO BEING ONSOME RESTRICTED WORK.
EE STATED THAT HE WAS HELPING INSTALL A SET OF TIRE CHAINS ON THE FRONT WHEELS OF A FRONT END LOADER. WHILE DRIVING A PIN INTO THE TIRE CHAINS WITH A HAMMER, A SMALL PIECE OF METAL FLEW OFF ST RIKING AND BECOMEING EMBEDDED IN THE EE'S LEFT LEG, METAL WAS REMOVED FROM HIS LEG BY A PHYSICIAN.

HE CAUGHT HIS RIGHT MIDDLE FINGER BETWEEN THE ENGINE STAND AND ENGINE MOUNT CAUS ING FRACTURE TO RIGHT MIDDLE FINGERTIP. NO LOST TIME INCURRED BUT EE IS ON SOME WORK RESTRICTIONS LEFT HAND WORK.

KNEE STARTED SWELL8ING ABOUT 3 WEEKS LATER. WENT TO DR. ON 1-3-2001. NOT REPORTABLE UNTIL LOST TIME INCURRED ON 1-31-2001.
EE STATED THAT HE WAS REMOVING A CERAMIC LINER FROM A CYLANE SECTION. HE WAS USING A SLEDGE HAMMER TO KNOCK OUT THE LINER. AS HE WAS STRIKING THE LINER WITH THE HAMMER, HE MISSED THE LINER WIT H THE HAMMER AND STRUCK HIS RIGHT MIDDLE FINGER AGAINST THE LINER. EE STATED THAT HE STEPPED ON A ROUND BOLT LAYING ON THE FLOOR OF THE SHOP BUILDING. HIS LEFT FOOT SLIPPED ON THE BOLT AS IT ROLLED CAUSING HIM TO FALL. HE CAUGHT HIMSELF WITH HIS HANDS. HOWEVE R, AS HE DID, THIS CAUSED HIM TO TWIST HIS BACK RESULTING IN LOWER BACK PAIN, DID NOT GO TO DR. UNTIL 9-20-00 WHICH RESULTED IN LOST WORK DAY AND RESTRICTED WORK.
EMPLOYEE STATED THAT HE WAS ASSISTING THE MECHANIC IN REPLACING A HEEL PLATE ON THE LOADER BUCKET; A LARGE WASHER WELDED ONTO THE HEEL PLATE, WHICH WAS BEING USED TO LIFT THE PLATE INTO PLACE, BROKE OFF AS THEY WERE PRYING ON THE PLATE TO HINSTALL IT; THE METAL PLATE FELL OFF, STRIKING HIS RIGHT FOOT BEHIND THE HARD TOE AREA. MISSED WORK ONE SHIFT AND THEN LIGHT DUTY.
EE STATED THAT HE WAS ATTEMPTING TO REMOVE A ROOF BOLT CAUGHT AT THE HEAD ROLLER ON THE RAW COAL CONVEYOR BELT. THE BELT WAS TURNED OFF BUT HAD NOT COMPLETELY STOPPED. EE GRABBED HOLE OF ROOF BOLT AND THE BOLT JERKED HIS HAND AGAINST THE HEAD ROLLER SIDE CAUSING LACERATION TO TWO FINGERS. REPORTABLE DUE TO REQUIRING SUTURES.
A DOZER OPRATOR WAS WORKING/PUSHING COAL TOWARD A LOADOUT SPOT FOR THE FRONT END LOADER WHERE IT COULD BE PICKED UP AND DUMPED INTO THE HOPPER FOR THE PREP PLANT. AS HE WAS PUSHIGN COAL, HE N OTICED STEAM/SMOKE COMING FROM THE COAL PILE. AT APPROX 11:45 PART OF THE COAL PILE WHICH HAD BEEN OPENED UP AND EXPOSED TO AIR BY THE DOZER, COMBUSTED INTO FLAMES. THE DOZER OPERATOR PROCEE
EE STATED THAT HE WAS DISMOUNTING THE DOZER. HE STEPPED FROM THE DOZER ONTO THE PUSH ARM AND THEN FROM THE PUSH ARM ONTO THE GROUND. AS HE STEPPED ONTO THE GROUND, HIS LEFT FOOT STEPPED ONTO F ROZEN MUD WHICH CAUSED HIM TO TWIST HIS ANKLE AS HE WAS STEPPING DOWN.
