A roof was installing a 6' roof bolt on a full face miner. He was torqueing the bolt when a 2' wrench broke and hit him above the eye.
A fully trained maintenance helper had turned a shield off. He was standing on the shield that he believed had fully functioned. He started the shield and it lowered on him. Another employee raised the shield. The employee was trained as a shieldman.
A fully trained miner was checking the suction on the bolter while the drill steel was in place the steel broke and struck him in the mouth the miner had on all required PPE but should not have had steel in place while checking suction
EE was cleaning the JSeal pump at end of shift and removed the grating of the hopper. Right hand came in contact with a moving auger. EE suffered traumatic amputation of middle and ring fingers on right hand.
While putting in a second drill steel the employee pressed the wrong lever and the stack steel did not meet. The steel then hit the roof bent the steel pushing the drill pot against the frame of the miner pinching his left hand.
While drilling a roof bolt hole the drill steel bent and struck his right hand.
Was backing a miner out of the face. Went to pull rib door in and pulled the wrong lever. Caught right thumb between door and rib. 4 stitches
The employee was explaining work to other miners with his back towards the water line and miner cable. When the miner cable and water line dislodged from the hanger threw the employee into the ATRS jack on the bolter.
employee was using a Jack-Hammer along the belt line to break up rock when he caught his left thumb between the belt structure and Jack Hammer causing a laceration. (required 6 stitches)
She is a trained rib bolter with 5 years experience. She was wearing all of her personal protective equipment. As she was trying to install a bolt into the drilled rib, she pinched her right wrist between the rib strap and the coal rib causing a radial, distal fracture. The cause of the incident was poor hand placement.
He was performing his regular job as a trained rib bolter, and was wearing all of his personal protective equipment. He had his left hand near the end of the rib bolting unit. The mining machine started to vibrate, and his hand was pinched between the rib bolting unit and the coal rib. The result was a laceration requiring 4 sutures to his left index finger.
While grinding a punch pin, the grinding wheel broke, a piece of the wheel struck and punctured his right upper thigh. He received three sutures to close the laceration.
He was placing a vertical pin on a longwall shield. He asked another employee to activate the relay bar to drop the pin into the hole. When the relay bar moved, it caught his left index finger between the pin and the panline causing a laceration which required 21 sutures.
He was hanging a chain for a ventilation tube and laid his hand on top of the rib protector door. The miner trammed forward and pinched his right pinky finger between the rib protector and the rib.
Employee was roof bolting on the miner. While pushing the bolt head into the chuck under pressure, his left middle finger became caught between the bolt head and the drill pot. The result of the injury was a fracture of the second phalange on his finger.
He was rib bolting on the right side of the continuous miner. He just finished drilling his hole when he lost his footing. In order to catch himself, he grabbed the slide on the rib drill with his right hand. His left hand (still on the lever controls) accidentally bumped the lever and the pinner retracted and pinched his right hand between the pinner and the stops.
He was observing his EE pulling a longwall face flight with a chain and shield. The flight broke free and struck him across the chest and chin. The result was a fracture to his jaw requiring surgery. He began missing work on 4/19/13.
He was bolting off the miner in the 12A miner section. He laid his right arm on the base of the bolter mast. He lowered the bolter mast using his left hand and the mast came down on top of his right arm. He began missing work on 4-19-13.
Employee was raising the rib bolter and felt pain in his right groin. Employee was diagnosed with an inguinal hernia
Employee tightening rib bolt w/hydraulic drill. Drill spun & hit employee in right cheek.
He was rockdusting outby the 6B miner section. He threw a rockdust hose over his shoulder and felt pain in his right lower back. *The operator disputes the allegations set forth in Citation #8049152 and submits this report solely to terminate the citation.
He was operating an air drill, the drill handle slipped out of his hands and struck him on the safety glassed. *The operator disputes the allegations set forth in Citation #8049153 and submits this report solely to terminate the citation.
Employee was assisting in bolting on a fall at 17-12. The drill operator pushed the bolt up with the drill, the bolt bent and kicked the drill sideways pinching his finger between the bolt and volcano plate.
Employee was jacking dollies back on track on the mainline track. Over a period of time throughout the shift he felt tightness in his back. He went to doctor and MRI appointments and began to miss work on 8/31/11.
10A Longwall at 150 shield; employee was operating the tail gate end of the shearer and cutting toward the head. His right hand was resting on the handrail when a rock came off the shearer drum and struck his right hand causing a laceration.
He was operating the shearer when a piece of rock came off the headgate drum and struck him on the right hand. *We dispute the allegations in Citation # 8049157 and submit this report solely to terminate the citation.
Based on the reports, it appears that the EE backed the dozer he was operating down a hill so that it was in front of a water truck that was stuck and needed to be pulled up the hill. The EE exited the dozer to help with the dozer's winch and was between the dozer and the truck when one or both vehicles moved pinning the EE causing his fatal injuries. (See Attached Letter)
EE was drilling a left hole with pauline bolter, when drill steel got hung up, EE grabbed steels with left hand and checked bolter suction with right hand. The steel that was hung up in the hole fell, striking the steels he was holding, causing the drill steel to strike his right hand. Resulted in a fracture to the right ring finger.
EE was raising the head of the center bolter when a drill steel fell behind the head. The drill steel sprung back and struck the EE in the lower jaw. This resulted in a broken mandible and laceration.
Employee was running the headgate side of the shearer, when a baseball size rock came off the drum and struck him in the mouth.
Crew was spraying gunite on an overcast, when they had a plugged line. The crew located the plug, shut off the air to the hose and began to break the line. When the employee took the line apart, air and gunite shot out of the end strucking the injured in the face.
EE was swamping out continuous miner when vibrations caused unused rib straps to fall off storage area on boom of miner. EE reacted to grab the straps and caught right middle and ring fingers between straps and bumper of miner. Laceration on both fingers.
While pushing a 16' cable bolt into the top with a gopher drill, the bolt slipped off causing the drill to rise quickly. The employee's hand was in a pinch point which caused multiple fractures and two lacerations to his right middle finger.
While setting a shield on the 7A long wall set up face, the injured activated the relay bar function in order to align the horizontal pin. This action caused unexpected movement of the shield pontoon which cam into contact with the employee's right foot causing multiple fractures.
WHILE DRILLING METAL ON A CATWALK, DRILL GOT CAUGHT ON METAL AND TWISTED EE'S RING FINGER ON HIS RIGHT HAND, STOVING IT.
Employee moved cable that he thought was bolter cable. When the bolter was turned on, the cable jerked and threw the employee to the ground. When he fell he landed on his left wrist resulting in a fracture.
While grinding on handrail a piece of steel bounced off face shield and into left eye.
While instralling a rib bolt with a breast drill, the employees lost their grip on the drill which allowed the drill handle to rotate up and strike the employee on the right ear causing a laceration requiring sutures.
Employee was caught up & tripped by the entry cable when center bolter was turned on, causing him to fall.
While pulling shields at 1 BHG, shield shifted striking the duke jack. The duke jack in turn struck the employee in the lower leg.
While torqueing bolt, coupler broke causing steel to strike employee above right ear causing laceration.
As employee finished using his starter steel, he began dropping the drill unit with the rotation on. He felt the steel bind as the bit cleared the plank; the bottom of the steel was broken which flew out, striking him in the nose.
Emloyee was center bolting when a piece of the weiner skin got caught in the rotating drill steel of a center bolter pulling and wrapping the weiner skin around. His hand, injuring his little left finger.
EE was walking in #2 entry. center bolter picked up the slack on his cable reel, the cable came across the entry and tripped him causing him to roll his right ankle.
Reaching for drill steel and cut arm on bolt plate while bolting on miner.
Employee was welding in shaker chute when employee experienced what appeared to be heat exhaustion. Employee was treated for a burn to the leg.
Employee installed roof bolt on left side of miner and turned to speak with rib pinner while the head was sumping. A bolt from the bolt tray was caught by moving boom and struck Employee in left upper lip.
EE pinched right little finger between drill steel & side of miner. Rib pinner hoses caught drill steel while being retracted, pinched EE's finger between steel & miner.
Employee was center bolting in the 7 left 5 south section when a 5 foot section of auger stuck in the hole he was drilling. He started to install the second steel and the stuck auger fell from the hole pinching finger between the steels.
Backing miner when canopy caught on roof strap raising canopy off guides. Moved miner forward and canopy fell hitting miner on shoulder.
