EE was changing drill steel. The up lever for the pod was activated instead of the down, pinching and cutting his left index finger.
EE was moving debris with drill pot, finger was pinched between feed jack and drill pot arm.
Employee was operating roof bolter when a hydraulic hose blew, spraying oil in his face (medical only).
Employee had his right hand on the drill pot while swinging the boom and accidently hit boom lift lever.
With the Fletcher drill pot raised, employee lowered the book resulting in wedging and bending back his middle finger on his right hand.
EE was operating the Roof Bolter on the off side. He moved the bolter arm into position and set his stabilizing jack. He then positioned himself to start bolting when he lowered the bolter pot onto his right leg.
While putting drill steel together in the process of drilling a test hole, a piece of rock fell striking the control lever causing drill pot to rise pushing drill steel into his left hand.
WHILE INSTALLING FLYBOARD HE WAS HOLDING BOTH STEELS AND MOVING POT BACK AND FORTH WHEN THE POT HIT A ROCK CAUSING THE TWO DRILL STEELS TO PINCH HIS FINGERS.
WHILE TRAMMING THE FLETCHER ROOF BOLTER, HE RAN OVER SOME UNEVEN BOTTOM CAUSING THE DECK TO RAISE AND CAUSING HIS HEAD TO BE ABLE TO COME IN CONTACT WITH THE MINE ROOF.
WHILE INSTALLING ROOF BOLT EE GOT FINGER CAUGHT BETWEEN MINE TOP & BOLTER HEAD (NOT SURE ON WHATPART OF BOLTER HEAD, POSSIBLY IT WAS THE BOLTER WRENCH).
WHILE HE WAS INSTALLING ROOF BOLT, HIS GLOVE GOTCAUGHT ON BOLT RESULTING IN SPRAIN & STRAIN OF HAND & FINGERS.
INJURED WAS HELPING LOOSEN A HUNG UP MINING MACHINE FROM MUD. HE WAS ATTEMPTING TO PLACE A CRIB BLOCK UNDER THE TREADS OF THE UNIT AND IT LUNGEDBEFORE HE COULD REMOVE HIS HAND. THE TRACKS CUT HIS RIGHT HAND. THE MINING UNIT ITSELF WAS NOT DAMAGED AND MUD COULD POSSIBLE BE A FACTOR. NO RULES OR SAFETY REGULATION VIOLATED BUT MORE AWARENESS FROM WORKERS COULD BE A FACTOR.
EE WAS INSTALLING A #5 REBAR 42" ROOFBOLT. WHILESTRAIGHTENING THE BOLT IN AN AWKWARD POSITION HEFELT SOMETHING IN HIS RIGHT SHOULDER POP.
INJURED WAS OPERATING A BELT CUTTER IN ORDER TO ACCOMPLISH MAKING SPLICE IN 36" BELT. WHILE PULLING ON THE UNIT HIS HAND SLIPPED AND FINGER SLID ACROSS THE BLADE. NO SAFETY RULES WERE VIOLATED AND MINE DOES NOT KNOW OF ANY SAFETY EQUIMENT THAT MIGHT HAVE PREVENTED THE ACCIDENT. THE INJURED DID NOT VIOLATE ANYTHING HE HAD RECEIVED IN TRAINING BUT HE COULD HAVE BEEN MORE ALERT TO AVO
EMPLOYEE WAS TRAMMING THE #2 BRIDGE AROUND CORNER AND HIS BRIDGE WAS PULLED TOWARDS THE RIB BY THE #3 BRIDGE OPERATOR, #2 BRIDGE JAMMED EMPLOYEE'S LEFT LEG INTO RIB.
DRAW ROCK FELL WHEN EMPLOYEE SET HIS ROOF SUPPORT JACK HITTING HIM ON THE RIGHT SHOULDER.
KETTLE BOTTOM/DRAW ROCK FELL BETWEEN T.R.S. AND CONTROL STATION WHILE HE WAS DRILLING HITTING EE ON HEAD, ARM AND HAND.
