Employee was knocking down fly pad material with a hammer, right hand struck a nail, cut hand between little and ring finger.

EE bent over to read electrical meter and had abdomen pain.

Employee was taking water line apart, took coupler off and water line sprung upward hitting him in the face.

Employee bent over to pick up hand grinder, felt back pain.

Employee was tramming a 3-wheeler along #4 belt, the front wheel hit a rut causing steering wheel to spin and the knob struck his hand.

Employee was tramming a scoop to section, the brake head over heated and caught on fire. He put the fire out with rock dust and inhaled smoke in the process.

Employee filed an ALLEGED Carpal Tunnel claim on 6/6/06. Received notice on 9/8/06 that he had surgery for Claim No. 06-841264. Had surgery on his other hand 9/20/06.

Employee was pulling belt with chain hoist, belt rail pulled out striking his face.

Employee was changing out planetary on bridge and mashed finger between planetary and bridge frame.

Employee was operating the #4 Fairchild Bridge, the crawler unit turned without his knowledge and when he started to tram, the machine trammed onto his foot.

With the Fletcher drill pot raised, employee lowered the book resulting in wedging and bending back his middle finger on his right hand.

Employee was operating a mantrip when he hit a saw horse that was holding a continuous miner swing jack. The jack fell and landed on his right foot. Amendment: He did not miss any work until 10/24/05. This accident was previously reported on 10/7/05 as a no lost time accident on E-Document 242996.

Employee was removing cover to replace light bulb on scoop. He cut his finger on a piece of metal.

EE was operating the Roof Bolter on the off side. He moved the bolter arm into position and set his stabilizing jack. He then positioned himself to start bolting when he lowered the bolter pot onto his right leg.

EE injured his lower back when he lifted a bottom roller onto the stand during belt move.

Ee was making the fire boss examination when his ride got stuck. After realizing there was no jack on the ride, he pushed the mantrip and strained his back.

While putting drill steel together in the process of drilling a test hole, a piece of rock fell striking the control lever causing drill pot to rise pushing drill steel into his left hand.

Employee was tramming #3 Bridge into the next cut, he had his fingers on top of bridge when it rocked sideways, pinching his left middle finger between bridge and metal guard.

Employee was pulling on a belt conveyor stand when he felt a pain in his right shoulder and arm. His first day of work missed and doctor visit was on 05/17/2004.

WHILE CHANGING SCOOP BATTERIES AT THE UNDERGROUND SCOOP CHARGER, A PIECE OF ROCK FELL STRIKING HIM ON HIS HEAD.

WHILE PICKING UP MINER CABLE AND BACKING MINER UP OUT OF #2 ENTRY HE INJURED HIS LOWER BACK.

EMPLOYEE WAS INSTALLING MINER BITS INTO THE BIT LUG WHEN THE HAMMER HE WAS USING GLANCED OFF ONE BIT CRUSHEING HIS FINGER BETWEEN HAMMER & ANOTHER BIT.

WHILE OPERATING SCOOP HE JAMMED HIS NECK AGAINST MINE ROOF.

STARTER FROM LOADER FELL OFF BUMPER ONTO HIS RIGHT FOOT.

OPERATING #2 MOBILE BRIDGE FROM #4 ENTRY TO #3 ENTRY THE BRIDGE ROLLED UP CAUSING HIM TO HIT HIS ELBOW ON ROOF

WHILE CREW MEMBERS WERE INSTALLING A RAPID HAUL BOLT WITH A HAMMER, SOME TYPE OF MATERIAL ENTERED HIS EYE SCRATCHING THE RIGHT RETINA.

WHILE HANGING VENTILATION CURTAIN, THE NAIL HE WAS PUSHING THROUGH THE CURTAIN SLIPPED RESULTINGIN A LACERATION ON HIS RIGHT INDEX FINGER. HE DID NOT SEE A DOCTOR UNTIL 9-2-03.

EMPLOYEE WAS GETTING OFF OF THE MANTRIP WHEN HE FELT SOMETHING PINCH IN HIS RIGHT LEG.

EE WAS CHANGING MINER BITS WHEN A PIECE OF METAL STRUCK HIM IN THE EYE. STATED THAT HIS SAFETY GLASSES SLIPPED DOWN ALLOWING THE METAL TO ENTER THE EYE.

THE EE WAS COMING OUT OF THE MINE WHEN SOME DUST GOT IN HIS EYE. HE RETURNED TO WORK THE FOLLOWING DAY AND WORKED THE 9TH, 11TH AND 12TH. HE LEFT EARLY ON THE 12TH TO RECEIVE MEDICAL TREATMENT .

EE WAS ATTEMPTING TO CROSS THE HAULAGE SYSTEM DURING A BELT PROVE. THE MINER WAS ATTACHED TO THE HAULAGE SYSTEM SO THAT THEY COULD MOVE IT. THE MINER OPERATOR DID NOT HEAR EE TELL HIM NO TO GO . WHEN EE GOT ON TOP OF THE HAULAGE THE MINER OPERATOR TRAMMED FORWARD CAUSING THE SYSTEM TO RAISE UP SMASHING EE IN THE TOP.

WHILE INSTALLING ROOF BOLT EE GOT FINGER CAUGHT BETWEEN MINE TOP & BOLTER HEAD (NOT SURE ON WHATPART OF BOLTER HEAD, POSSIBLY IT WAS THE BOLTER WRENCH).

EE WAS PUMPING GREASE INTO A HOT BEARING, ATTEMPTING TO COOL IT, WHEN HOT GREASE SPLASHED INTO HIS RIGHT EYE.
