With the Fletcher drill pot raised, employee lowered the book resulting in wedging and bending back his middle finger on his right hand.

Employee was operating a mantrip when he hit a saw horse that was holding a continuous miner swing jack. The jack fell and landed on his right foot. Amendment: He did not miss any work until 10/24/05. This accident was previously reported on 10/7/05 as a no lost time accident on E-Document 242996.

Employee was removing cover to replace light bulb on scoop. He cut his finger on a piece of metal.

EE was operating the Roof Bolter on the off side. He moved the bolter arm into position and set his stabilizing jack. He then positioned himself to start bolting when he lowered the bolter pot onto his right leg.

Ee was making the fire boss examination when his ride got stuck. After realizing there was no jack on the ride, he pushed the mantrip and strained his back.

While putting drill steel together in the process of drilling a test hole, a piece of rock fell striking the control lever causing drill pot to rise pushing drill steel into his left hand.

THE EE WAS COMING OUT OF THE MINE WHEN SOME DUST GOT IN HIS EYE. HE RETURNED TO WORK THE FOLLOWING DAY AND WORKED THE 9TH, 11TH AND 12TH. HE LEFT EARLY ON THE 12TH TO RECEIVE MEDICAL TREATMENT .

EE WAS ATTEMPTING TO CROSS THE HAULAGE SYSTEM DURING A BELT PROVE. THE MINER WAS ATTACHED TO THE HAULAGE SYSTEM SO THAT THEY COULD MOVE IT. THE MINER OPERATOR DID NOT HEAR EE TELL HIM NO TO GO . WHEN EE GOT ON TOP OF THE HAULAGE THE MINER OPERATOR TRAMMED FORWARD CAUSING THE SYSTEM TO RAISE UP SMASHING EE IN THE TOP.
