Employee was cutting out a blade liner on a D-11R dozer. The liner broke loose pushing him to the ground. His foot was under the blade and his foot was fouled when he began to move causing a break to his lower left leg (blade was on the ground; the ground was uneven where the work was being done).

Showing all 6Employee was pushing in a shot. When he started to back up a rock came from the spoil and he backed up on it. When he started to tram off, the dozer slipped off causing a jolting action resulting in lower back pain. He continued to work until 10/21/06 at which time he bagan to miss work.

EE WAS DIAGNOSED WITH A MILD BULGING DISC & DEG. DISC DISEASE.
