Employee was hanging curtain at the roof bolt machine when they were struck by a rock measuring approximately 32" wide by 65" long by 1-4" thick.
Employee was operating a continuous miner on 2 Gas Mainline 3 section when they were struck in the left foot by a rib roll measuring 7' long by 15" wide by 3" thick. Employee returned to work after the accident but was placed off work on 11/5/2025.
Employee was installing a roof bolt when EE stated EE's back was hurting. Employee went to the hospital and was released and returned to work. Employee went to EE's primary care physician on 6/9/2025 where an MRI was conducted and produced negative results and employee returned to work. Employee returned to EE's primary care physician on 7/9/2025 where EE was placed off work.
Employee was operating continuous miner when they were struck in the left foot by a rock that came off the boom of the miner.
Employee was standing next to the feeder when a rock struck the skirt board and broke the weld causing the skirt board to strike the employee in the left arm.
While ee was using a drill press in the shop, they cut two of their left hand fingers requiring stitches.
Individual was bent over, cutting a piece of metal out from between crusher rolls, when a piece of mud that was stuck to the side of the chute fell, struck the employee, knocking EE into the side of the chute.
While operating the shearer, injured placed hand on the pan line and a rock hit left ring finger, resulting in a laceration that required 8 stitches.
Employee was struck on the hand by a link from a broken chain. The employee was standing 25 feet from the continuous miner as the CM backed out of #3. A chain, that had been of the miner, got wrapped around the offside spray block chassis and the other end was run over by the miner, putting tension on the chain causing it to break.
Injured was drilling a hole for a 12' super bolt when the insert for his third piece of steel broke, causing the steel to bend near the chuck. As the steel rotated, it struck the injured on the lower right arm, causing two fractures to the lower right arm.
While removing a rock jammed in crusher, right thumb smashed and forearm cut
EE was operating the tail side of the shearer and crossed out of the 6th shield zone to confirm that the shield was against the top. When EE stepped on the 7th shield the shield advanced and caught EE's leg between the relay bar and the shields pontoon.
A pump repair was in progress. The flywheel came off and struck the employee in the abdomen, chest, and face. The abdomen and chest were bruised, the chin was cut, and two teeth were chipped.
Employee stated that in an attempt to change out a drill chuck on a roof bolt machine he had fouled a wrench on the old chuck and was going to use the drill's rotation to break it free. The employee mistakenly engaged the boom raise lever, instead of the rotation, and pinched his fingers between the wrench and boom.
The employee stated he was attempting to place the miner cable out of the area so as not to get the continuous miner on the cable, when he tripped and fell hitting the tram levers causing the machine to pinch him between the machine and the rib.
A crew was in the process of changing out a transfer belt when a damaged idler became dislodged from the frame assembly and struck the EE in the upper right arm causing a compound fracture of the humerus, a torn tricep muscle and a fractured pelvis.
The EE was cutting a 2" X 4" board with a curcular saw, when the saw kicked back and cut the EE on the right leg, requiring sutures.
Injured was in the process of removing drill steel from drill hole when his left middle finger was caught between the bottom and top extension joint, resulting in a laceration requiring sutures.
Injured was replacing a jack pin on the #48 shield when he caught his finger between the jack and pan.
Employee was mixing concrete with an electric mixer. He was emptying mix out of the mixer with his hand on the wheel, the wheel got loose and began to spin striking him on top of his hand.
Co-worker and injured were instructed to drill a test hole to evaluate the roof. The injured was unaware the co-worker was ready to tram the roof bolt machine. The injured was reaching for the drill steel stored on top of the machine when the co-worker trammed the machine forward. The injured sustained a contusion to the top of his right foot.
The employee was hooking a chain around a rock when the shear machine moved and tightened up the slack in the chain pinching the ring finger of the right hand between the chain and the rock.
Injured was in the process of installing a 4 ft strap to the mine roof. As the bolt was being installed, the bit caught the strap. This resulted in the strap rotating. The strap contacted the employee on the left ear.
Injured and 2 other EE's had finished supplying the roof bolter. The injured got into operations check and began to tram the roof bolter, when he turned the machine, he caught his hand between the roof bolter and the fork lift that was being used to supply the machine.
Employee and a co-worker were in the process of drilling holes into the mine floor to install anchor pins for a new belt drive. As the employee was attempting to re-position the drill, his right index finger was caught between the drill steel and the chuck of the drill, resulting in a laceration requiring 7 sutures to close.
Employee was holding a 6" x 16" roof plate in line and caught his right index finger between plate and the roof.
He was getting plates off the bolter for the roof bolts when the other operator moved the machine and ran over his left foot.
Employee was using a cutting torch - a piece of slag entered the employee's LEFT eye. The employee was wearing eye protection.
The employee unhooked a load that was placed by a crane, a coworker did not maintain control of the headache ball allowing it to swing into the injured employee - striking him in the head.
Employee was drilling with sinking hammer. He strained lower back while pulling drill out of hole.
EMPLOYEE WAS PINNING TOP, DRILL BIT BROKE. DRILL STARTED SPINNING, STRAINING RIGHT SHOULDER. STARTED LOSING TIME (OFF) 10-31-2005.
While installing 6ft resin bolt using a RRII Fletcher bolter, the bolt came out of drill pot on machine striking employee in the left arm and causing a contusion on the left arm.
Emp. was getting ready to change bit on hammer. Driller asked if his hands were clear of connection point. Emp said yes. Driller proceeded to begin connection which pinched end of emp's middle finger of left hand between hammer and bit.
TWISTING HOSE COUPLING TRYING TO ALIGN HOSE TO PUT IN SAFETY PIN. WHILE AIR COMPRESSOR WAS RUNNING THE HOSE BLEW APART SHOOTING DIRT INTO EE'S EYE. DIRT REMOVED AT HOSPITAL (OUTPATIENT).
The employee was struck on the right leg by a piece of 65 lb track rail being loaded on a low-boy which was moved by a continuous miner.
THE VICTIM WAS BOLTING THE ROOF, PREPARING FOR LONGWALL RECOVERY, WITH A MICON DRILL. HE TURNED TOWARD THE SHIELDS & WAS STRUCK IN THE RT.LOWER BACK & HIP BY A 3'X 9'X 3-18"ROCK. INJURED DID NOT CEASE WORK UNTIL 5/7/04.
AS THE VICTIM WAS ATTEMPTING TO BACK THE ROOF BOLTER FROM UNDERNEATH A ROCK WHICH HAD LODGED THE ATRS, A PIECE OF 4' DRILL STEEL WAS LODGED IN A HOLE FLEW BACK AND STRUCK THE VICTIM ON THE LEFT SHOULDER.
OPERATOR WAS INSTALLING ROOF BOLTS AND MASHED FINGER IN BETWEEN TOP AND ROOF BOLT PLATE.
EE STATED WHILE INSTALLING 6 FT ROOF BOLT, HE PUSHED BOLT INTO HOLE, HURTING HIS BACK.
VICTIM WAS INSERTING A GLUE TUBE AND BOLT INTO A HOLE WHEN A PIECE OF ROCK FELL FROM THE TOP, AND STRUCK HIM ON THE RT FOREARM.
INJURED WAS OPERATING THE HEAD SIDE CUTTER DRUM ON LONGWALL SECTION. AT APPROX 70 SHIELD HE NOTICED A SHIELD TILTED DOWN. HE WAS POSITIONED 10' TO 12' OUT BY DRUM (TOWARD HEADGATE) HE STATED T HAT HE COULDN'T SEE WHAT HE WAS CUTTING. A ROCK FLEW OUT FROM THE DRUM AND HIT HIM IN THE LEFT EYE.
THE VICTIM WAS OPERATING THE TAILSIDE OF SHEARERDRUM WHEN A HAULAGE BAR BOLT BROKE & STRUCK THE EE ON THE LEFT WRIST & RIBS
THE INJURED WAS USING A BENCH PRESS GRINDER TO REMOVE BURRS FROM A 3"WASHER. THE GRINDING WHEELSNAGGED THE WASHER AND PINCHED HIS RT. THUMB AGAINST THE GRINDER REST.
EE RAN HEAD INTO LIGHT BOX POWER SUPPLY CAUSING DAMAGE TO DISKS IN HIS NECK. EE DID NOT MISS WORK OR RECEIVE TREATMENT UNTIL 5-5-03.
THE EMPLOYEE WAS OPERATING THE CONTINUOUS MINER WHEN A PIECE OF ROCK ~2' LONG & 16" THICK FELL FROM THE RIB & STRUCK THE TOP OF HIS RIGHT FOOT.
THE MINER WAS ON HIS KNEES, BENDING ROOF BOLTS WHILE TRYING TO INSERT THEM INTO THE HOLES WHEN HE FELT A PAIN IN HIS LOWER BACK.
EE WAS CUTTING WITH TORCH UNDER BOOM OF MINER WHEN A PROOP OF HOT SLAG BLEW BACK INTO HIS THROAT.
WHILE UNLOADING ROCK FROM SHOT O-CAST HOLE, THE CUTTER HEAD SLUNG A SMALL PIECE OF ROCK THAT STRUCK HIM IN THE BACK OF THE HEAD.
A HYDRAULIC HOSE WAS CAUGHT BY THE STAB JACK, BUSTING THE HOSE.
WHILE INSTALLING ROOF BOLT PLATE AGAINST TOP, LEFT MIDDLE FINGER WAS CAUGHT ON TOP OF PLATE, CUT TIP OF FINGER. EE WAS NOT MADE AWARE OF LOST TIME UNTIL 4-29-02.
