Showing all 5The employee was installing a roof bolt in the face of the No. 2 heading when EE inadvertently applied rotation to the drill causing EE's left had to twist around the roof bolt being installed.
Failed to completely get cable bolt into hole hew as attempting to bend remainder of bolt with drill head. The drill slipped on the bolt and caught his right index finger. The finger was mashed and cut.
Employee was holding a section of drill steel in the hole while he was swinging the drill head back and forth to clear loose coal from underneath the drill head. The drill head was moved too close to his wrist inadvertently causing the canopy to strike his wrist causing the bruising.
FLEXIBLE 2" WATER LINE FROZE GOING INTO THE MINE. IT WAS BROUGHT TO THE SHOP TO UNFREEZE. WHEN ICE STARTED MELTING, AIR PRESSURE WAS APPLIED TO THE HOSE TO HELP REMOVE THE ICE. WHEN THE LAST O F THE ICE CAME OUT IT RELEASED THE AIR PRESSURE WHICH MADE THE HOSE KICK BACK HITTING EE IN THE FOREHEAD AND NOSE. BROKE NOSE AND SEVERE BRUISING.
