Crew was going thru airlock when door closed back toward ride, when employee kicked door & broke foot while door was closing.

The employee was attempting to scale a piece of draw rock with a slate bar. When the piece EE was prying on came down and struck EE on the head and neck. EE was transported to the Medical Center for head and back pain. Tests were negative for fractures and Ee was diagnosed with soft tissue bruising.

EE was crawling from the rear of the roof bolter to EE's drill station. A piece of rock got between EE's knee and knee pad when EE put knee down on the rock, the result was severe pain to left knee. EE was transported to the surface then to medical center. EE was examined and released and told to pursue additional medical examinations.
EE was replacing the ripper head jack on the continuous miner, the excessive lifting and pushing resulted in a burning sensation in EE's left shoulder. EE reported that EE's shoulder was burning. EE continued working until 9/27/19 which time EE went to a doctor. EE performed regular job duties until shoulder surgery on 11/21/19.
EE was on the left side of the No. 6 entry approx. 10 ft. outby the last open crosscut performing repairs to the offside of the continuous miner when a piece of draw rock measuring approx. 4' L x 28"" W & 4-5"" thick fell striking EE on left shoulder. A large lump immediately popped up on EE's left shoulder. EE was transported to the surface by rail mounted vehicle.

EE was assisting to lift a roof bolter drill head approx. 24 inches off the floor into a personnel carrier so that it could be transported to the surface when EE experienced extreme pain in EE's lower back. The drill head weighed approx. 140 lbs. The injury occurred at the end of the section supply track way.
Roof fall at break #201 in the #5 entry of intake intersection fell in. Measuring about 30' x 30"" x 9'. No injuries or deaths. Did not change the course of any travel.
While bolting in #8 heading a piece of rock rolled out from the face catching EE on the lower part of EE's right leg
Was bolting in #4 Right when a piece of draw rock fell striking EE on the right arm and cutting EE requiring 3 stitches.

The employee was traveling through the crosscut from No.2 entry toward the No.1 entry. While rounding the inby corner into the No.1 entry, EE contacted that corner with right shoulder and body. EE fell to the ground and started experiencing severe pain in the middle portion of EE's back.
The employee was physically pushing ½ of a 16 foot airlock door to open it when severe pain occurred in their lower back area. The metal airlock doors are located at 12 crosscuts, inby the no. 5 return air portal and separate the main intake and return air courses. The doors are 16 ft. wide. A lot of air pressure is present at this location.
EE was attempting to change a conveyor torque shaft on the Joy continuous miner, when the shafts cap was loosened. The cap blew out spraying EE on the hands and the right side of face with hydraulic oil. EE received 1st & 2nd degree burns to the top side of both hands and right side of the face.
EE was attempting to remove some draw rock off top of cont. miner. EE grabbed a piece of rock measuring 30""L x 24""W x 5"" thick & pulled it toward body. When rock reached edge of machine EE lost grip, rock dropped down & caught front part of ring finger on rt hand against edge of machine severing most of the flesh toward the bone. EE was working in no. 6 entry w/2 co-workers.
EE was pushing a rock, on the side of the no. 4 belt tail piece, with a shovel, back onto belt when rock hit moving belt it spun around, bounced back to the side of the tail piece, struck EE's legs, causing EE to lose balance, fall onto belt conveyor. EE carried underneath belt crossover and down belt when EE grabbed a belt support chain and fell off belt. EE travelled about 40ft

EE was using the utility knife to strip the insulation from the leads on 2/0 high voltage cable, the knife slipped and cut hand at the base of right ring finger. EE drove self to the Whitesburg KY Appalachia Regional Hospital where ee received 3 stitches.
When the drill bit contacted the immediate roof a layer of draw rock measuring 17 inches X 17 inches x feather edge to 1 inch thick, fell striking EE on EE's right thumb and forefinger resulting in 2 broken bones and 1 fractured bone.
During the process of inserting a roof bolt through a steel strap into the drilled hole a small piece of draw rock was dislodged and struck EE on EE's left forearm resulting in a laceration.
The employee was changing belt conveyor top rollers which requires heavy lifting and pulling, while in this process ee suffered severe lower back pain

EE was installing roof bolts and steel straps a piece of draw rock dropped out in the area of the T-bar, it hit the loose end of the straps sliding down the straps to the mine floor. When the rock measuring 4'x4'x3"" thick contacted the mine floor flipped back on to ee right leg resulting in a bulging fracture to the ankle area.
While twisting and turning on his knees swelling and severe pain occurred.
While maneuvering the 4 wheel personnel carrier in to tight area the front wheel contacted the coal rib causing the steering wheel to spin uncontrollable striking EE on the right index finger.

The continuous miner was mining in the third lift of a left pillar when the roof fell causing the cont. miner to be stuck. The roof fall extended from the front of the miner to the back of the main frame.

Failed to completely get cable bolt into hole hew as attempting to bend remainder of bolt with drill head. The drill slipped on the bolt and caught his right index finger. The finger was mashed and cut.
Employee bumped his right elbow on the corner of the center control panel while sitting down on the Johnson Ind. 4 wheeler. Resulting in swelling and bruising to the elbow.

EE was changing battery on Mac 8 mantrip. Battery arced and caused battery to burst.

Standing at the back of the miner machine the EE started the miner raised the ripper head and the machine tilted, then the back bumper sat down on the left toe of boot and broke three toes.
