Showing all 23Employee was in the process of installing a 6' bolt. As he was pushing the bolt up, the bolt started to bend and he hit his rotation causing the bolt wrench to strike him in the nose.
A roof bolter operator was injured while preparing to install a roof bolt in the crosscut between the No6&No7 entries. operator started to position the roof bolter machine to install the roof bolt, the offside operator started around the back of machine. Offside operator was pinched between the coal rib and roof bolting machine. (MSHA immediately notified of accident)
Employee was attempting to remove a piece of steel that had gotten stuck in a hole. As he was rotating the steel, the pusher came loose from the finisher striking him on the right cheek causing a laceration that required 6 stitches.
Employee was loading glue on roof bolter when operator moved bolter and ran over his foot. Glue was being moved to the operator tray.
EE was cleaning out the cyclone on roof bolter- another operator set stab jack while ee had his arm in cyclone
The individual was backing the roof bolter out of #4 Left and his right hand was caught between the operator deck and the coal rib. He did not start missing work until 2-11-11.
The individual was roof bolting and had finished drilling the hole when the drill steel fell out of the hole hitting his left hand causing a fracture to the ring finger.
While operating the continuous miner, a rock was thrown by the cutter striking the individual in the head above the right eye causing a contusion.
The individual was tramming the continuous miner over a piece of coal. The coal burst and a piece flew up and hit the individual in the mouth breaking a tooth. This is a medical only with no lost or restricted work days.
The individual was operating a roof bolting machine. He had his hand positioned on the boom lift of the roof bolting machine. As he raised the drill head on the machine, he caught the tip of his right ring finger. The tip of his finger was surgically removed by the physician.
The injured was hanging the electrial cable to the roof bolting machine. The machine operator engaged the cable reel and caused the cable to knock EE down to the mine floor.
The continuous mining machine finished a left run out of the entry while pillaring. The injured was preparing to reposition the mobile roof supports when the mining machine accidently struck a piece of metal on the MRS. The metal shot off unexpectedly striking him on th right forearm. This resulted in a laceration which required sutures.
The injured was loading coal into a shuttle car with a continuous mining machine when a rock flipped off the machine boom and struck the individual on his left leg. This resulted in a fracture of his lower leg.
ON 12/4/02 THE HYDRAULIC DRILL WAS USED TO DRILL THE BOTTOM. THE NEXT DAY THE EMPLOYEE CALLED & SAID HIS SHOULDER WAS BOTHERING HIM FROM DRILLING THE PREVIOUS DAY.
EMPLOYEE WAS LETTING DRILL HEAD OF PINNER DOWN WHEN GLOVE GOT CAUGHT BETWEEN ARM AND DRILL HEAD.PULLING HIS HAND IN IT. IT CUT THE TIP OF HIS LEFT RING FINGER. NOT AN AMPUTATION, JUST A BAD CU T.
EE WAS BOLTING TOP WHEN ROCK FELL HITTING HIM ON THE LEFT ARM REQUIRED STITCHES.
EE WAS DRILLING A HOLE AND DRILL STEEL FELL OUT OF THE HOLE AND HIT EE ON THE LEFT HAND.
EMPLOYEE WAS BOLTING TOP WHEN A BOLT WRENCH CAME OFF THE TOP OF THE PINNER HEAD & HIT HIM IN THE MOUTH, BREAKING HIS TOOTH OFF.
EE WAS DRILLING A HOLE, AND A ROCK FELL STRIKINGHIM IN THE NOSE.
EMPLOYEE WAS BOLTING TOP WHEN A ROCK FELL AND KNOCKED HIM INTO THE PINNER.
EE WAS MAKING SPLICE ON THE END OF A BELT, HE TORE HIS LEFT THUMB NAIL OFF AND FRACTURED THE TIP OF HIS FINGER.
EMPLOYEE WAS INSTALLING ROOF BOLTS WHEN A PIECE OF ROCK FELL FROM THE MINE ROOF AND STRUCK HIS LEFT MIDDLE FINGER. THIS RESULTED IN A FRACTURE OF THE TIP OF HIS FINGER. THE EMPLOYEE DID NOT GO TO A PHYSICIAN UNTIL 7-14-00.
