An EE was using the joystick on a roof bolter to install a roof bolt, the employee felt left wrist pop. The employee received stitches in a procedure to fix the ligament in wrist.
The injured EE went to hang ventilation curtain, while his operator put up a corner bolt. The EE hanging curtain went back to get nails. The Operator started repositioned the bolter, not realizing his helper came back. When he started moving the bolter he bumped his helpers hip.
Employee stated he lowered the drill pot on his left knee. The Employee had no broke bones the Dr. placed him on light duty for a few days.
After helping his roof bolt partner straighten out a bent cable bolt, he was walking back to his side when he slipped and as he attempted to catch himself he placed his hand on the idle arm as his partner was running the drill pot up. At this time his hand got caught in the mechanism causing a crushing injury to his left index, middle and ring finger.
Employee was roof bolting and got his finger caught between leveling arms and the canopy post. (fractured finger)
Employee stated that he aligned his screen on the roof, his steel was started on a strand of the screen. When he rotated the drill steel to drill his hole it wrapped the screen around the tip of his finger pinching the tip of his left index finger off.
The employee stated that he placed his drill steel in the chuck of the drill pot on a Fletcher bolter, when he went to use his up pressure the steel popped out of the chuck and hit him between his left thumb and left index finger. The pressure from the hit caused an open wound that resulted in three stitches.
Employee was exiting the operators deck when his feet hit the tram pedal and lever, sliding the operators canopy of the roof bolter into rib. Employee's fingers were between canopy and rib resulting in right hand index finger to be cut off.
The operator thought his helper motioned him to advance forward, when the operator dropped the ATRS on the bolter the front end of the roof bolter dropped and the drill station canopy hit the helper on the head.
Employee was drilling a hole with a piece of drill steel, the drill steel broke and a piece of the drill steel come back and hit him in the left shoulder causing a laceration.
Employee was helping bolter operators, when he reached over the bolter to mark an area on the roof with paint. The bolter operator on the opposite side moved the bolter inby to the next row to be installed and the helper had his foot in front of the bolter tire and the tire rolled on his foot.
Employee was installing roof bolts when he had his right thumb caught between the drill chuck and the mine roof. The employee received a laceration.
Employee cut left thumb installing metal strap in roof. Employee received two sutures. Employee was wearing metalcarpal gloves.
Continuous miner operator was tramming the miner from place to place when the miner left track started to spin and kicked the conveyor boom against the miner operator. The operator received 3 broken ribs
A small piece of coal fell from the mine roof and bounced off the roof bolting machine and struck employee in the nose.
The employee was pulling a drill steel from the mine roof and his hand was on bottom of steel. When the steel came loose from the roof the employee's right pinky finger was smashed between the bottom of the steel and the roof bolter drill head. The employee's received a laceration which required 6 sutures, and a broken 5th Distal Phalanx.
Injury was drilling the roof when I needed to change to the 2nd still. the first fell out from the roof striking my finger on the chuck of the roof bolter. This injury did not involve any aspect of compliances with rules or regulations, job skills or attitude.
Employee got a metal fleck in his eye while grinding.
Employee was moving Fletcher roof bolter in outby area of mine. The canopy of the bolting machine caught a roof bolt & a small piece of metal projected into his left eye. Employee had safety glasses in his possession, but was not wearing them.
EMPLOYEE WAS PUTTING THE WRENCH UP TO INSTALL THE ROOF BOLT AND PINCHED HIS LEFT INDEX FINGER ON THE BOLTER ARM (9 STITCHES IN FINGER). THIS INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS OR ATTITUDE.
I WAS WELDING ON STEEL STRUCTURE WHEN IT FELL. MY WELD DID NOT HOLD. I DID NOT DO IT PROPERLY. I AM A CERTIFIED WELDER. THIS INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS OR ATTITUDE.
EE WAS OPERATING ROOF BOLTING MACHINE. WHEN PRESSURE WAS APPLIED TO DRILL WRENCH, IT SLIPPED AND STRUCK EE'S LEFT INDEX FINGER. LACERATION - 11 STITCHES. THIS INJURY DID NOT INVOLVE ANY ASPECT OF RULES OR REGULATIONS, JOB SKILLS OR ATTITUDE.
I WAS DRILLING A HOLE TO INSTALL A ROOF BOLT-AFTER DRILLING FIRST SECTION I LOWERED DRILL HEAD TO INSERT EXTENSION, FIRST STILL WAS IN THE ROOF, AS I PUT EXTENSION INTO DRILL HEAD THE FIRST ST ILL FELL FROM ROOF STRIKING MY RIGHT HAND, SMALL FINGER. THE INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS, ATTITUDE OR PROTECTIVE ITEMS. GLOVES WERE W
EE WAS OPERATING ROOF BOLT MACHINE HE WAS SWINGING BOOM IN TO DRILL HOLE. WHEN BOOM WAS COMING IN, IT STRUCK A PIECE OF DRILL STILL THAT HAD FALLEN. WHEN IT STRUCK THIS, THE PIECE FLIPPED INTO THE AIR AND STRUCK EE IN THE RIGHT CHEEKBONE AND SIDE OF FACE INCLUDING THE EYE AREA AS IT FLIPPED UP. THIS ACCIDENT DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS.
WAS OPERATING ROOF BOLTER, SMALL ROCK FELL & STRUCK EE ON HARD HAT AND LT. SHOULDER. THIS INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS OR ATTITUD E ON PROTECTION ITEMS. THIS ACCIDENT WAS NOT RELATED TO OPERATION OF EQUIPMENT.
I WAS OPERATING A ROOF BOLT MACHINE, I HAD INSERTED THE RESIN & THE BOLT AND HAD TURNED BACK TO THE CONTROLS WHEN A FLAKE (6"X1 1/2" THICK) OF SHALE STRUCK MY HARD HAT AND MY FACE. THE INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH RULES OR REGULATIONS, JOB SKILLS, TRAINING OR ATTITUDE OR PROTECTIVE ITEMS.
WAS OPERATING FLETCHER ROOF BOLTER. WAS REACHING FOR DRILL STEEL THAT WAS IN ROOF -STEEL FELL UNEXPECTINGLY STRIKING EE IN TOP OF THE RIGHT HAND. THE STEEL CONNECTOR THAT WAS IN THE STEEL CUT EE'S HAND. THE INJURY DID NOT INVOLVE ANY ASPECT OF COMPLIANCE WITH REULES OR REGULATIONS, JOB SKILLS OR ATTITUDES.
MINER OPERATOR WAS STRUCK IN FOREHEAD WITH A SMALL PIECE OF COAL THROWN FROM THE HEAD OF THE CONTINUOUS MINER.
CHANGING CHUCK IN DRILL HEAD ON BOLTER. ROCK FELL OU6T STRIKING EE ON THE LEFT SIDE OF HIS EAR AND HEAD. THE ROCK WAS APPROX 8" X 1-1 1/2" THICK. 5 STITCHES AT EMERGENCY ROOM.
