While aligning a shield, shear operator stopped advancing the shear, during the stop a rock fell from the top and glanced off the spinning drum and hit the employee in the side of the face.

EE was assisting with retriever operation, when a clevis broke, the retriever lunged and the hydraulic lines caught the EE foot pulling into into a post striking their head.

Injured was working on a valve bank while pan line was pushing, toe of boot was down in the connector for the relay bar to the pan. Pinched metatarsal on boot and pinched EE's big toe.

While bolting, the injured individual began drilling a hole. After penetrating the first layer, a section of rock loosened and fell down the drill steel. When it hit the drill pot, it broke, causing a 3-foot piece to drop onto the platform and land on EE's right foot, resulting in a diagnosis of a fractured small toe.

While tramming C/M with the boom angled towards them, injured walked up to the boom to adjust the cable when the miner came towards them pinning them between the boom and the rib, Injured was taken to medical facility where they were diagnosed with a fractured pelvis.

A Miner was dumping Muck Buckets on the Surface. When the bucket was flipping back over after dumping the miner's finger was caught between the bucket bell and bottom bell stop.

Employee was in the process of dumping a muck bucket when left forearm was caught in the counter weight area and pinched.

On February 2, Employee was assisting the unloading of a motor out of a supply car when EE grabbed the rope at the sheave wheel as another employee began operating the winch, 'bumping' the control. After the control was 'bumped', EE yelled stop, & indicated to the other person that a finger was pinched. Resulting as an amputation to the first knuckle of the index finger.

Head side shearer operator was cutting sandstone top traveling to the head gate. Injured was standing approximately 1-2 shields ahead of the head drum, leaning over the pan line monitoring the cut that was in progress. During this process, EE was struck in the neck (right side) by a piece of the metal from the drum.

Injured was operating Head Side of Shear on Longwall. Injured was leaning with left hand on equipment. A rock was thrown from the Head Drum approx. 15 ft and struck the injured's left pinky finger. Finger was fractured and lacerated.

Injured was bolting. When second steel added to steel already in roof. Injured did not get second steel seated into drill chuck. When up pressure was applied, drill steel slipped off side of chuck striking injured in the rt index finger. Injured continued to work until 1/27/22 when EE was taken to Broaddus Hosp for an x-ray and showed avulsion fracture.

Employee was clearing rock out of the walkway of the longwall. While they were performing the task, a rock fell from on top of a shield and made contact with hardhat then landed on right thumb, causing it to be fractured.

Employee was positioned on #25 shield when the shield began to lower and pull in. The injured was caught between the hoses on the shield and the pan line. The employee was examined at a hospital and diagnosed with a fractured pelvis.

After changing a roof drill bit on the starter steel, the EE had the steel resting on top of EE's right foot. As EE lowered the drill pot, the pot caught the drill steel as it was coming down and pushed the steel down on top of EE's right foot resulting in a fracture to the 2nd metatarsal. The employee was taken off work after being examined by specialist on 5/28/21.

The employee's left little finger was mashed between a water line coupling and the roof bolter bumper. EE was treated at Broaddus Hospital where EE received two sutures to the little finger. The employee was released to work full duty and returned to work on EE's next scheduled shift 7/25/20.

While tightening lug nuts on a mantrip, the wheel stud broke causing the employee to lose balance and to fall rolling the right ankle underneath EE. EE finished the shift and was taken to the hospital on 7/24/2020 where EE was referred to an orthopedic specialist on 7/28/2020. There EE was diagnosed with a distal fibula fracture and was taken off work.

While roof bolting in high top, EE was lowering the drill head to reposition the drill steel. The bit on the drill steel became caught on the roof mesh causing the drill steel to pull out of the drill chuck. The steel glanced off the machine canopy & struck EE on the upper lip resulting in a laceration that required four sutures to close. EE was released to work full duty.

While unhooking the cable pull rope from the continuous miner pan, the employee's left forearm became pinched between the cutter head stop and the side of the pan. The employee was transported to a hospital in Morgantown, WV where they were examined. EE was admitted later that day to remove a portion of dead skin caused by the pinch. Prepping for graft.

While using a cutting torch to cut a bent flight out of a shuttle car chain, a piece of hot slag or hot spark got into the employee's left ear. The employee continued to work until 6/26/19 at which time EE went to the hospital for medical procedure to repair perforation to the left ear drum.

EE was repositioning drill station of the twin head roof bolter. EE swung the drill station ee had gloved right hand resting on the hand hold on the underside of the operators canopy. Canopy came in contact with a roof bolt plate that was not flat against the roof due to uneven roof and the plate cut the 3rd finger of right hand requiring 3 sutures to close the laceration.

EE was in the process of installing belt hangers using the roof bolting machine and 5 foot long roof bolts to secure the belt hangers to the roof when the 4th finger on ee right hand was caught between the drill pot and the ATRS. This resulted in an open fracture and tissue loss to this finger.

While using a hand held grinder to remove a weld from a piece of metal the grinder disc caught and the grinder bounced back allowing the rotating disc to strike the injured on the chin resulting in a laceration requiring sutures to close.

Principal injured was attempting to remove a stuck drill steel from the mine roof by raising the drill head to the roof to align the wrench with the stuck drill steel when employee's right thumb was caught between the wrench and the T-3 channel strap employee was bolting to the mine roof.

EE had drilled into the mine roof 5.5 feet with 2 stackable drill steels, EE was attempting to place the third stackable steel into the roof bolter drill pot when the 2 that were in the hole dropped out striking and pinching the little finger on EE's left hand between the steel EE was holding and the ones that dropped out of the roof. Resulted in a laceration requiring 4 sutures.

Employee's right arm was caught between the feeder and the feeder trailing cable when he was attempting to trip the machine's service breaker. The cable had been caught by a rock in the feeder and became tight at the point he reached for the breaker switch. The injury resulted in a fracture to the radius and ulna bones.

While operating the continuous miner in the #3 entry on the #3 section; employee, was struck in the left side of the neck by a piece of metal that appears to have come from a bit lug or a miner bit.

Individual was operating the continuous miner in the cross cut between number 1 and number 11 headings when a piece of metal 1 inch long by 1/2 inch wide by 1/4 inch thick flew outby from the cutter head on the miner approximately 26 feet striking him in the left forearm. Individual was transported to hospital and metal was surgically removed.

Employee was installing a roof bolt and during the rotation process a small slither of metal off either the bolt or the bolt plate became airborne and struck the employee in the right eye.

While lowering the drill steel. The steel came out of the chuck and struck the individual on the end of the left thumb. The individual was seen at the hopital and diagnosed with a fracture to the left thumb.

Employee was installing roof bolt when the roof plate spun lacerating his right thumb. Employee recieved 7 stitches

The employee was roof bolting and while installing the bolt into the drill head he inadvertently hit the steel isolater lever squeezing his left hand resulting in 14 sutures.

The employee was operating a dual boom Fletcher roof bolter when a piece of roof rock fell, striking the drill pot causing it to fly up, striking the indivual in the mouth. The laceration required 6 sutures to close the wound.

The employee was operating a fletcher roof bolter when he inadvertingly hit the steel isolator control closing it on his right hand causing a fracture.

The injured was drilling a hole for a belt anchor, when he let the head of the bolt machine down the drill steel stuck in the mine roof, momentarily, and then fell out of the drilled hole and struck the injured on the chin resulting in a laceration which required 2 sutures to close.

The injured was installing a resin roof bolt in the roof. As he was pushing the bolt up with the head of the bolt machine the bolt bent and the drill wrench broke striking him in the face, causing a laceration to his nose and lip which required sutures to close.

The injured was operating the radio remote continuous miner when he was caught between the cable handler and the coal rib resulting in crushing injuries to his left leg and thigh.

EE was installing a 5 foot resin roof bolt with 8"x8" plate & a metal curtain hanger. In an effort to align the hanger he attached a nylon rope to it, when he applied rotation to the bolt his hand & arm became caught and twisted by the rotating roof bolt resulting in fractures to his left arm, wrist, ring and middle fingers as well as a laceration to the palm of his left hand

Injured was watching the helper's side operator re-attach the hose to his drill head on a Fletcher DDR roof bolter. The helper raised the mast to attach the hose and was unaware of the injured, who was standing on the outside of the machine, placing his hand on the mast. After attaching the hose the mast was dropped and the injured fingers were caught in the mast.

The employee was standing on the right side of the Fletcher DD0 15 Roof bolter. As the bolter trammed past him to set up to begin a row of bolts, the right front tire ran over the side of a rock that was laying on the mine floor, measuring approximately 10? Long by 10? Wide by 5? thick, causing the rock to become an airborne projectile and struck the employee.

Injured was bolting wire screen to mine roof. He placed a bent drill steel against the top while his hand was on the drill steel. He started the drill rotation and the steel bent and struck him on the hand and finger.

Bending roof bolt while installing into top, injured felt pain in lower back. This became a lost time accident 5-23-06.

Injured was operating the helpers side of the roof bolter in the # 6 entry, he pinched the palm of his right hand between the pusher and the finisher steels, causing a laceration. Injured received 3 stitches.

The injured was on the operators side, pushing a cable bolt up to finish its installation when he was struck on the right side of his face by an 18" long bolt wrench which was ejected from within the scissor action of the bolter head under extreme force.

WHILE ROOF BOLTING, THE FINISHING STEEL GOT STUCK. EE REACHED FOR THE FINISHING STEEL. IT CAME OUT OF THE HOLE, SMASHING HIS RIGHT RING FINGER AGAINST THE BOTTOM STEEL. THIS RESULTED IN A FRACTURE. EE DID NOT MISS ANY WORK, MAKING THIS A MEDICAL REPORTABLE ACCIDENT.

A 3' X 20" X 1" ROCK FELL AND TIP END STRUCK THE EMPLOYEE IN THE BACK.

While he was bolting an overcast a piece of rock fell and cut his forehead.

Injured had scaled roof down and installed first row. They moved bolter to install the 2nd row of bolts. As he positioned his body to insert the resin and bolt, he placed his rt. hand on the bolter boom and a rock weighing approx. 20 lbs. fell striking his wrist. NOTE: DID NOT MISS WORK UNTIL 01/18/05.

A loose rock had set down on the roof bolter canopy. As the injured lowered the canopy to swing the boom of the bolter he had placed his hand on the head of the boom, the rock feel off the canopy striking his wrist causing a laceration
