After pumping #49 seal located on the TG9 panel at 25 block, elevated readings were found. Evacuation of the mine commenced immediately. No injuries occurred due to this incident. All employees were accounted for once reaching the surface with no issues. Reference number #05036739

Due to a guide wheel being damaged, the main mine elevator was down more than 30 minutes. No individuals were on the elevator at this time and the elevator was repaired in approximately 40 minutes time. After repair the elevator was tested and put back into service with no further issues.

While aligning a shield, shear operator stopped advancing the shear, during the stop a rock fell from the top and glanced off the spinning drum and hit the employee in the side of the face.

Employee was taking out belt structure. They pulled on a piece of structure that was laying between the top and bottom belt and felt pain in their back (strain).

Clevis broke on a retriever, striking other employees. When this occurred, a jack was about to fall on top of another employee and injured grabbed the jack to steady it, when EE did EE felt a sharp pain in EE's right bicep, received surgery to repair a torn bicep on the 26th of February.

EE was assisting with retriever operation, when a clevis broke, the retriever lunged and the hydraulic lines caught the EE foot pulling into into a post striking their head.

An unintentional roof fall occurred on the 9HG LW section that measured 45'Lx10'Hx18'W located in the #3 entry between 13 -14 block. There were no injuries and the escape way was rerouted to the LW tail. A meeting was held with all miners as to be advised of the new escape way. Reference # 04950714

A door switch malfunctioned on the elevator on the landing side in turn caused a fuse to blow. The blown fuse and loss of power caused the VFD to loose its program. Once the switch and fuse were repaired the VFD had to be reprogrammed. There were no individuals on the elevator at any time. The elevator was back in service at 2:18AM.

Injured was working on a valve bank while pan line was pushing, toe of boot was down in the connector for the relay bar to the pan. Pinched metatarsal on boot and pinched EE's big toe.

An unintentional roof fall had occurred at 4 Zone track in the #3 entry 3 to 4 block. No equipment was damaged with no injurie to persons. The area was dangered off with additional support set. The secondary lifeline was rerouted before work was resumed. No ventilation controls were affected at any time during this.

While cribbing track that was lifted with a jack, employee stepped over track to get a crib when the jack fell causing the rail to impact EE's left calf causing small abrasion and a broken fibula.

EE was assisting with final seal prep cleaning the bottom with a shovel when a piece of rock fell from the top and struck their hand against the shovel handle, causing a laceration. Injury was not the result of a training or compliance issue. 7000-1 was initially filled out 5/12/25, but the "Submit" button was inadvertently not clicked, thus this late submission.

Employee was working on the 6879 CM. EE had the light cover lifted and were working on the trailing cable junction box for permissibility purpose. An EE had turned their head to speak with another EE as that EE was walking by when the lid came down smashing IE's hand/ finger between the lid and top of the junction box cover.

A contractor who was walking across the parking lot from EE's parked car to the mine's training center when EE was struck by the car of an employee. The employee was arriving at work to begin EE's shift and searching for a parking space at the time of the accident.

Employee sustained injuries while trying to secure a miner cable into a multi-hook hanger that was beyond EE's reach. The employee attempted to jump to place the cable into the hanger but missed. EE's hand became caught between the miner cable and an adjacent roller hanger, leading to a laceration and a metacarpal fracture of EE's right hand.

EE was assisting in the changing of a ripper head motor on a C/M. EE was hitting a hose connection and inadvertently hit left thumb with a hammer. EE refused treatment, on 10/17/2024 EE decided on EE's own to visit the ER. It was determined by X-ray that EE's left thumb was fractured.

While operating a scoop, the injured person needed to adjust self in the seat. EE grabbed the canopy with the right hand and accidentally caught the palm of their hand between the rib and the canopy. EE was then transported to the hospital for treatment, which included surgery to set the bone in EE's pinky finger and repair the soft tissue damage.

The Mine Elevator here at Leer South Mine went down due to a faulty brake switch that had a bolt loosen which caused the micro-switch to short out. No individuals were on the elevator at the time of the incident. The elevator was repaired and put back into service at 6:28 p.m.

While bolting, the injured individual began drilling a hole. After penetrating the first layer, a section of rock loosened and fell down the drill steel. When it hit the drill pot, it broke, causing a 3-foot piece to drop onto the platform and land on EE's right foot, resulting in a diagnosis of a fractured small toe.

While dismantling the monorail on the longwall tailpiece, the monorail slipped from the injured person's left hand and struck the tip of their right-hand pinky finger. This resulted in a crushing injury to the fingertip and the fingernail, necessitating six stitches to close the wound. The injured individual was released without any restrictions.

While tramming C/M with the boom angled towards them, injured walked up to the boom to adjust the cable when the miner came towards them pinning them between the boom and the rib, Injured was taken to medical facility where they were diagnosed with a fractured pelvis.

On 6/23/24, while using channel locks to pull a belt across a belt sled, injured twisted right knee and fell to the ground when the channel locks slipped from the belt. EE finished the shift, the next day sought medical evaluation from primary care doctor. MRI found a torn meniscus and ACL ligaments. *On 7/5/24, Leer South decided to put EE off work due to potential further injury.

A face ignition occurred on the TG9 section in the #1 entry at 26 1/2 block while intersecting the #9 gas well leg. All precautions from the well plan were in place with no injuries, damage to equipment,escapeways, or ventilation controls affected. Actions plans were put in place for corrective measures.

While placing a pallet of track sand onto the elevator with a motorized dolly, dolly made contact with the elevator door knocking it off track. Elevator was down from 07:15 until 09:00pm.

Main mine fan developed a vibration, steadily got worse. Leer South made the determination to shut if off and correct the issue. Inspected and found the bolts for the main fan motor loose, tightened and restarted the fan. down at 5:44pm Started fan back at 6:18pm.

On 1/12/24 the elevator at Leer South went down with an employee on it. The employee was removed within 1hr & 20min. The elevator was down from 235pm on 1/12/24 until 158pm on 1/13/24. The main ABB drive failed on the elevator and had to be replaced.

EE was repairing a high stopping that was damaged from an earlier explosives detonation. The stopping started to fall towards EE, and EE reacted by jumping off the ladder. EE landed wrong and EE's right ankle turned outwards. EE was transported outside where we iced and bandaged EE. On 1-8-2024 EE was taken to a medical facility and after x-rays was diagnosed with a broken ankle.

The mine lost power in turn causing the main mine fan to become inoperable. The current server for the automatic switches on the high voltage capacitor bank malfunctioned. The incident occurred at 9:04am and was restored at 9:31am. Crews were withdrawn from the mine after the fan power was not restored within 15 minutes and the power circuits were de-energized.

An unintentional roof fall has been discovered on the King Knob Panel in the #5 entry at #5 block on 8/24/23. No injuries occurred due to this incident with no damage to any equipment, ventilation controls, or escapeways. The air course examiners travel is also unobstructed as well. The dimensions of the fall measured 27'L x 18'W x 10'H. All approaches have been dangered off.

The main mine elevator was inoperative from 6:55AM to 10:30AM on 08/20/2023 due to a main front door switch failure. No employees or individuals were on the elevator at the time of the incident. The elevator was repaired and returned to normal service.

During inspection of the hoisting rope it was discovered that over lapping had occurred during operation. This was discovered on 08/19/2023 and the hoisting rope was changed out on 08/20/2023.

Due to a malfunction of a door switch, 21 men became entrapped on the Main Mine Elevator. ICE elevator services were notified and the switch was repaired with no injuries to anyone involved.

During the time that the main mine elevator was down, while testing the brakeman car it was discovered that there was a faulty wheel speed sensor. The elevator became operational before the brake car was repaired. Later the brakeman car was repaired and put back into service. There were no injuries involved during this incident.

Main Drive Control for main hoist motor is malfunctioning causing it to shudder at times during operation. This issue does not cause a safety malfunction but mechanism is not operating properly so we have locked the unit out until this is repaired.
