The employee was using a 6-inch electric handheld grinder with a cutoff wheel. The employee was cutting a steel bar grading walkway when the grinder kicked back and struck EE's right thumb. The grinder caused a laceration just above EE's right-hand thumb knuckle that required 5 sutures to close.

IE was drilling a hole to install a roof bolt. The drill steel became damaged and struck the IE in the right hand between the thumb and index finger causing a laceration. The IE received 5 sutures to close the laceration.

IE was moving #31 remote controlled bolter forward from entry 6-7 to entry 5. When the bolter went through a curtain in 6-7, the cable got caught on the bolter. As the IE went to reach for the cable to move it, EE believes they hit one of the tram buttons causing the machine to shift and pin themselves between machine bumper and the coal rib. IE was treated for a fractured pelvis.

IE was adjusting a fan in the surface shop to direct air towards where EE was working to help keep EE cooler. The IE grabbed a hold of the side of the guard which surrounds the fan to swivel it, one of the fan blades came in contact with the guard causing a piece of the fan blade to break off, causing a portion of the fan blade to go through EE's glove into the palm of EE's hand.

Employee was in the process of installing the extension drill steel needed for drilling an 8ft bolt hole. The extension was coupled with the starter drill steel and the drill head was brought up to insert the extension into the drill head. The steel did not align correctly and kicked out into the injured employees thumb

IE stated that while roof bolting IE was installing the 2nd steel onto the 1st drill steel that was already in the drill hole. When IE attempted to put the 2 steels together IE raised the drill pot up to the 2nd steel, the steel sprung back striking IE's hand in the thumb area.

While operating 401RB on Rt Side of Unit 2, Entry #9, Inby Spad Location 7+90 IE drilled approx 4' with starter steel then dropped boom to couple extension steel. After coupling steels IE raised boom to seat extension steel into the chuck. IE slipped on a rock and pushed joystick into Up position causing it to strike their hand

IE was replacing a roof bolter hyd fitting near the base of the ATRS. IE completed the work, the control side operator approached, did not see the IE and thought maint work was completed. The operator started the machine and swung the boom in pinching IE between the boom and ATRS. IE was treated at the local ER for a broken clavicle and released.

IE was roof bolting on unit #2 and had installed two rows in crosscut 2 3. On the third row, after installing roof bolt closest to the rib, the IE started lowering the drill pot down. As the drill pot was being lowered the IE sat the drill pot down on their right foot. IE was brought out and taken to hospital, and was diagnosed with a fractured little toe.

The IE was roof bolting and had just finished drilling the 1st drill steel. When IE lowered the drill pot, the 1st steel momentarily bound up in the drill chuck insert pulling it out of the drill pot. IE stopped lowering the drill pot, reached to grab the drill chuck insert before it fell, the 1st drill steel came out of the roof, striking the IE on top of right ring finger.

Employee was hit in the neck and shoulder with a cable while tramming the roof bolter on 10/31/17. EE continued to work until 12/15/17. It was not until work comp. reviewed the case and received the Dr's report that this injury was due to the incident that occurred on 10/31/17. We were notified on 1/25/18 of this diagnosis.

EE was working to unplug the 1st North transfer chute. EE used a cutting torch and trimmed the chute to allow better access to material blocking the chute. A piece of slag dropped between glove and cuff and burned the top of hand. EE continued to work. On 8/23/2017 EE was put on restrictive duty then released to full duty on 8/25/2017.

IE was installing a 10ft cable bolt. It is believed the bolt was damaged during installation. IE said EE installed the bolt, dropped the pot and the bolt spun. A flying particle struck IE's safety glasses and contacted eye. E.R. exam showed a small cornea contusion - no foreign material found in the eye. Prescription ointment was issued as a precaution to prevent infection.

EE was drilling with a wrench in high top. When drilling down, the wrench came out from the drill pot, the wrench bounced off the drill pot and flipped over coming down and striking the middle of upper lip with the adapter end of the wrench causing a small laceration. EE was transported to the hospital and received 3 stitches to close the wound.

A Contract employee had just slid a hog panel on the ATRS. EE pulled the roof bolter up to the next row and set the ATRS. EE started swinging the boom out and had EE's left hand resting on the handle under the canopy. As EE swung the boom out, EE's elbow hit the rib which caused EE's wrist to fold striking it under the canopy, causing a fracture to EE's wrist.

EE was using a 7" electric grinder on a shovel bucket lip installation. EE stopped grinding to inspect the area EE was working on. EE sat the grinder down with the wheel up. As EE removed EE's left hand from the handle of the grinder EE's gloved hand came in contact with the grinding wheel causing a laceration to EE's left wrist.

IE stated he was installing a roof bolt and it bent. He lowered the drill pot and started retracting the boom. IE was retracting the boom, he had his hand lying over the lip of the tray. During this time, one of the roof bolts located in the tray slid forward, crushing his right middle finger between the bolt and the lip of the tray, causing a laceration and a fracture.

While the offside operator was bolting the slab the IE noticed a damaged roof bolt in the entry. IE decided to spot a bolt beside the damaged bolt. IE finished drilling the hole, inserted resin and bolt into the drill hole, started to push the roof bolt up into the hole, he felt a pinch on his finger. The IE received sutures to close the wound to the end of his finger.

IE was using a magnetic drill weighing approximately 60 pounds to drill out a hole on a dozer track frame when the drill bit caught in the hole, generating sufficient rotary force to overcome the magnetic drill's connection to the frame. The drill bit stayed in the hole while the drill rotated toward the mechanic, striking him in the left knee, resulting in a fracture to the tibia

The injured employee was loading coal with a continuous miner at the face entry. He had loaded two cars out of the left side of the entry on his box cut. When he was struck in the head by a projectile, knocking him unconscious.

(IE) was roof bolting on Unit 2. He had just pushed an 8ft. roof bolt up into its hole when he felt pain and a pop in his left forearm. He was transported out of the mine for medical attention. He was examined at 4 different medical facilities before having surgery on May 28, 2015 to repair a partial thickness tear to his left distal bicep.

IE was using a hand held side grinder with a cut brush. Apparently a piece of wire came off the cut brush and embedded in his upper abdomen. He sought medical attention on 9/2/2014 and a piece of wire was removed from his abdomen.

A roof bolter operator (IE) stated that he installed a drill steel coupling into the starter steel to complete drilling the hole. The insert broke at the base of the starter steel. The 2nd steel came out of the drill chuck and struck the IE just below the elbow, right above the kevlar sleeve he was wearing. The IE went to the Dr. and received two staples to close the wound.

A contract employee was standing in the intersection of # 1 entry hanging tie wire to hang the continuous miner cable. The continuous miner was parked between #2 and #1 entry. The continuous miner operator started tramming the miner to #00 entry. When the continuous miner entered the intersection, the contract employee was struck by the head of the continuous miner in the leg.

Employee was roof bolting and had positioned a roof bolt in front of him resting on the bolter wrench holder. As he lowered the bolter boom it came in contact with the roof bolt knocking off the expander shell and it struck him in the mouth. He received a laceration to his lip requiring 2 sutures and chipped 2 teeth.

A mechanic was using a service truck crane to install pull down chains on an overburden drill mast. He had tied a nylon rope to the chain and then to the crane. As he was pulling the chain/rope, the rope broke and made contact with the mechanic. He was taken to the ER and give 11 sutures in his lip and nose.

The IE was loading roof bolts on the offside of the bolter. When the operator started to advance to the next row, he heard the IE scream from his foot being ran over. The IE was taken to the hospital and treated for a contusion on the top of his foot. The IE continued to work, and then on 4/8/14, IE was seen by Dr. and failed to report to work on his scheduled afternoon shift.

Employee was installing a 8' bolt. As he was adding a 4' extension steel on the initial steel, he dropped the pod and it came down on the steel he had resting on the boot of his right foot. This caused the steel to come thru the top of his boot, smashing the middle toe of his right foot. Toe was amputated on 10/22/13.

IE was operating the continuous miner, when he was struck by a foreign object that went into his upper arm (triceps). He was administered first aid and then it was determined that an object was embedded in his arm. He was transported to the ER. He had surgery on the morning of 8/15/13 to remove a piece of metal. He was taken off work by the surgeon until his next Dr's visit.

IE was beveling edges on a metal plate when the air grinder he was using started slowing down. The IE added oil to the grinder and was pointing the grinder at the floor, exhausting the excess oil from the end of the grinder. At that time the grinding wheel came apart striking IE in the left leg. He received 6 sutures to close the wound.

The employee stated that he was installing a roof bolt next to a bent roof bolt that he had just previously tried to install. When he was raising the drill pot up to push the roof bolt up to the top, he put his hand on the roof bolt plate to turn it so that the two plates would not overlap it caught his finger tip causing an avulsion.

The employee stated that as was roof bolting (drilling a hole), the IE was looking up to see how deep the hole needed to be drilled. While he was looking up, the drill pot was approx. at head level or slightly above his head. He was struck by an unknown foreign object that he thinks spun off of the drill pot and hit him in the forehead, causing a laceration that required sutures.

The I.E. bolting glove was caught between his bolting wrench and the drill chuck on the #29 Roof Bolter when he engaged the rotation. His left arm became entangled with the wrench. He sustained a closed fracture to his left ulna and radius.

Employee was beginning to start roof bolting. He had inserted a drill steel into the drill pot. He was holding the steel with his right hand when he thought he reached for the lever to raise the pot. Instead he activated the lever to close the auger clamp and it caught his right hand. He received fractures to his right ring and middle finger.

The injured employee was installing a spray block cover on the # 10 continuous miner. While holding the cover to put the bolts in, the cover slipped out the employees hand. The cover smashed his left ring finger. It was determined he had a tuft fracture at the tip of the finger and required three stitches.

The employee stated that he had just completed installing a corner roof bolt at 5-6 xc. He had his hand resting on the tray of the bolter, and as he was retracting the boom assembly back in toward the roof bolting machine, the roof bolts on the tray of the roof bolter slid forward, pinching his left index finger between the lip on the tray and the roof bolts.

Operator was hauling dirt from cut area. He had just dumped a load on spoil and was driving on return ramp when he drove up on berm, proceeded approximately 80 feet and then drove off of highwall, resulting in injuries and damage to equipment.

The miner operator was cutting in #10 entry on #2 Unit. He had approximately 10 foot cut out of the right side, when he was struck by a small piece of flying metal. The piece of metal struck the employee in the left nostril, embedding the piece of metal in his nose. He was transported to the hospital,where the piece of metal was removed. He received 5 sutures to close the wound.

Employee was operating a remote control miner and had a large rock caught in the conveyor. While attempting to load it into a shuttle car, the rock came off the side of the boom and pinned him against the rib causing a fracture to his left leg.

This employee was standing beside a roof bolter and his buddy turned on the bolter and the cable took his feet from under him, and he landed on his back injurying it. This did not become a lost time till 6/23/10.
