After drilling hole the steels were stuck together and in the chuck. The employee was trying to get them out. There was a roof bolt laying across EE's boom, as EE went to move the roof bolt and sump EE's pot forward at the same time, mashed EE's left pinky, ring and middle finger. This resulted in three fractured fingers.

Showing 50 of 55Roof bolter operator was installing bolt in roof, when he went to straighten bolt out, the bolt came out of the roof and struck him on his forehead. This resulted in a laceration to forehead that needed sutures.
Injured was standing at the 3rd to last row of bolts moving the miner from the 4th lift back to the other side to clean up the cut. The tail was swung over towards the operator, he thought the tail was straight. He turned to flag a car when the tail pinned him against the rib, and the miner drug him down the rib to inby the 2nd to last row of bolts.
EE was reaching over getting pins off of the bolter. The operator moved the bolter without ee knowing resulting in the bolter running over his right foot and causing an abrasion to his right leg, that required sutures to repair.
Employee was trying to break loose a fitting on a rotation hose with two pair of channel locks. When the fitting broke loose, he hit his right little finger on the boom resulting in a fracture.
Employee was struck and rolled down the coal rib of the # 2 entry by the ATRS of the DBT bolter on # 1 unit right side. Employee had been hanging curtain prior to the bolter tramming and he was trying to get to the opposite operators side of the bolter when struck. He was not seen by the operator.
The miner was moving into the face while the employee was rehanging a flapper on a backup curtain, when his left ankle got tangled up in the miner cable causing him to twist his left ankle. The twisted ankle resulted in a soft tissue injury which placed him off of work.
Employee was putting up a pin when the pin board smashed his right little finger between the pin board and the bottom of the canopy.
Employee was setting up to put up first pin, when he raised his canopy the collar for the second stage of the jack went up & it caught his pinner steel raising it up and putting it in a bind. When the employee grabbed it it sprung loose catching his finger between steel and pin tray. Causing a laceration to his left middle finger, requiring 5 stitches.
EE was pushing a roof bolt into the hole when he felt a pain in his lower back. He was taken to the Hospital and has a strained muscle. He began missing work on 12/13/2012.
EE was grinding key stock on a wheel unit when the grinder kicked back striking him on his right index finger causing a laceration.
The injured employee was struck by the seat of the roof bolter he was bolting on. This resulted in a laceration to his left leg and required three sutures.

Employee was pulling cable to the face. Employee looked through the curtain which was on the horn of the miner. The curtain caught the control levers causing the miner to hit the employee's knee.
Employee was in bad top. He bumped the ATRS against the roof to get the loose top to fall. A rock fell hitting the employee on the back and thigh causing him to fall and hit his head on a rock or lump cutting his forhead.
Employee was dropping the boom on his roof bolter when he put his hand on the boom to take the steel out of the roof. His finger got caught between the pot and the boom causing a fracture.
Employee was setting a 5 gallon can of oil on the back of his roof bolter. The bolter was tramming forward when the canopy hit the roof. The bolter then backed up no more than a foot when the back bumper got on the employee's foot.
Employee was bolting on the operators side of the pinner. He had finished drilling a hole and was lowering his pot, when the first steel had cleared the roof and the top steel was visible. When his hand got close to the steel, the bottom steel came out of the top steel. The steel jerked over and struck him on the inside of his thumb.
While installing the second steel, the employee lost his balance. He put left hand on the pot to catch himself, when the first steel fell out of the roof, striking his finger.
Operator was inserting second steel into the first hole in #5 entry. He trammed the pot up into the steel causing the steel to spring back hitting his right hand.
Employee was in the process of drilling a hole with a roof bolting machine. As he went to connect the sectional steels, he hit the boom lift lever on the bolter causing the steels to pop out and strike his left hand which resulted in sutures.
While installing roof bolts the pin steels stuck together. Employee pulled steels to separate them and the bottom steel struck him just above the eye.
EE was cutting a short 2x4 with a skillsaw. When he was almost thru the cut the saw bound up. EE moved his hand to the back of the board and pulled the trigger. The saw kicked bakc cutting his left thumb.
Employee was in the process of installing a roof bolt, as he was drilling the hole to install the bolt the drill steel caught on the lip of the drill pot on the bolter. When he applied upward pressure with the boom, the drill steel struck the employees right hand, causing a fracture.
Employee stated he was installing a roof bolt when the bolt bent. He put his arm up in a defensive posture and the bolt struck his left forearm. This resulted in a fracture.
Employee was in the process installing a 6' fully grouted roof bolt. At this time he swung the boom of the bolter out to put up his rib bolt and inadvertently hit the wrong lever and dropped the boom down onto his right foot causing a deep bruise to his foot.
Employee was operating a roof bolting machine when he noticed his back to start hurting. The employee does not know what he did to injure himself but only that his back is bothering him. Employee continued working and missed work on 1-29-07 and has not returned as of yet. We are treating this as an occupational illness due to DR determination of Sciatica inflammation.
Employee was installing a roof bolt when he repositioned his body causing him to slip, at this time he inadvertently pulled back on the joy stick on the bolter and lowered the boom of the bolter onto his right foot which resulted in a fracture to his foot.

At this time his knee hit the joy stick on the bolter causing the boom to lower and mash his fingers. This resulted in a laceration to 2 fingers on his right hand.
Employee was caught between a roof bolter and a shuttle car when the bolter trammed forward.
Employee was releasing the foot jack on the roof bolter and was struck on the head by the canopy.

EMPLOYEE WAS REMOVING A BOLTER CABLE FROM A CABLE HANGER WHEN THE BOLTER OPERATOR ENGAGED THE CABLE REEL CAUSING THE CABLE TO STRIKE THE EMPLOYEE IN THE NOSE.

Employee bent a pin while installing it in the roof. The pin spun around and struck the employee on the left arm and wrist.
EMPLOYEE WAS INSTALLING A ROOF BOLT WHEN THE BOLT FELL OUT OF THE HOLE AND STRUCK THE EMPLOYEE ON THE L/CHEEK CAUSING A LACERATION REQUIRING 6 STITCHES
Employee was operating a Mucker, trying to position an empty hoist bucket. The hoist bucket raised suddenly and struck the employee.
While working in shaft bottom, muck bucket descended down upon employee bruising lower back area. He was taken to hospital and released to return to work. Upon follow up visit on 01/11/06, he was told to remain off of work until next visit on 01/18/06.
EMPLOYEE WAS ATTEMPTING TO BEND A 6' ROOF BOLT AND HE FELT A POP IN HIS LEFT SHOULDER. AS HE WAS STRAIGHTENING THE BOLT, HE FELT ANOTHER POP IN HIS SHOULDER ALONG WITH A TINGLING SENSATION IN HIS LEFT ARM. HE DISLOCATED HIS LEFT SHOULDER AND IT POPPED BACK IN PLACE.

THE FOREMAN WAS TAKING EE INTO THE PIT, WHEN HE HAD HIM MOVE THE HITACHI BACKHOE. AFTER HE DISMOUNTED THE BACKHOE, HE WALKED BACK TO THE TRUCK WIPING THE MUD OFF HIS HANDS. WHEN HE WIPED THE M UD OFF HIS HANDS, HE NOTICED HE HAD A LACERATION TO HIS LEFT HAND. HE DOESN'T KNOW WHEN OR HOW HE CUT HIS HAND. HE WAS TAKEN TO THE EMERGENCY ROOM WHERE THEY PUT 14 SUTURES IN THE HEAL OF HAN
EE WAS CUTTING IN THE #1 ENTRY WHEN A ROCK 2 1/2X 6 X 4 FELL OUT BETWEEN THE PINS STRIKING EE ON HIS RT. FOOT AND ANKLE.
