Showing all 29Installation of part (snubrite) on mining shovel bucket was being performed. Lifting sling failed, causing a component of the snubrite to strike the employees left finger. Employee was diagnosed with fractures to left index finger.
Two employees were grinding on the center pin of the 495 BI shovel. The injured employee walked 2 to 3 feet away from the work area to plug in another grinder. He removed his safety glasses while plugging in the grinder and a foreign object flew into his eye.
Employee was repairing the Emergency Brake bracket on one of our service trucks. He was using a 3/8" electric drill with a 1/2" drill bit to bore out the bracket mount, when the bit got jammed. The drill twisted and hit the employees hand, resulting in a spiral fracture to the right hand.
EE stated that he had made a push with the rd630 -d10 rock dozer and was backing up and backed over a rock and felt pain to his back. EE refused medical treatment. He did not cease work. On Oct 7,2007, EE called in and stated that he went to the doctor and he had a strained back muscle.
Employee was welding and some welding slag flew into the employees ear. The employee continued to work and went to the doctor on a scheduled off day. The employee did not report the injury at the time it happen or when he returned to work. The first notice of injury was received on the day that we received paperwork from the Worker's Compensation Company (Brickstreet).
Employee was air arcing all day and had another employee welding within 2 feet of where he was working. The employee was wearing an automatic tint welding helmet that may not have been adjusted correctly. The employee suffered from flash burns to the eyes & face.
Employee was welding on the roof and fan house of the dragline and had too much exposure to welding rays.
EE was welding and received flash burns to his eyes.

USING AIR CHIPPER TO REMOVE SLAG FROM WELD, A PIECE FROM THE SLAG FLEW INTO HIS EYE.
EE WAS HELPING ANOTHER EE WELD A PLATE ON A NEWLY INSTALLED CRUSHER OF THE TRUCK DUMP AND SUFFERED A FLASHBURN WHILE ASSISTING THE OTHER EE, HE WOULD RAISE HIS LOAD WITH ONLY CLEAR SAFETY GLAS SES ON.
EE WAS OPERATING THE 801 EXCAVATOR REMOVING CLAY MATERIALS FROM A SPOIL BANK. AN UNKNOWN AMOUNT OF CLAY BROKE FLEE AND STRUCK THE FOREARD OPERATOR GLASS OF THE EVCAVATOR CAUSING IT TO SHATTER. THIS IN TURN IS BELIEVED TO HAVE CAUSED THE OPERATOR TO MAKE A SUDDEN MOVE INSIDE THE CAB AND STRIKE HIS LEFT KNEE AGAINST THE BRACE OF THE MACHINE RESULTING IN THE INJURY OF HIS LEFT KNEE.
EE WAS SEATED IN THE 730 ROCK TRUCK HE WAS OPERATING AND BEING LOADED BY THE 3600 HITACHI EXCAVATOR. HE STATES A LG. ROCK FELL FROM THE BUCKET OF THE EXCAVATOR INTO THE BED OF HIS TRUCK, JARRI NG HIM AND KNOCKING HIS GLASSES OFF HIS FACE. HEALSO STATES HE REC'D ADD'L JOLTS FROM LG. ROCKS THROUGHOUT THE REMAINDER OF THE SHIFT. EE DID NOT EXPERIENCE PAIN UNTIL AFTER THE SHIFT AND ALS
EE WAS HOLDING A PIECE OF METAL IN PLACE TO BE WELDED. OVER THE TIME PERIOD EE WAS EXPOSED TO WELDING FLASHES. THEREFORE CAUSING FLASH BURNS TO HIS EYES.
EE WAS OPERATING THE 285 DEMAG, LOADING OVERBURDEN SHOT MATERIAL WHEN A ROCK FELL FROM THE TOP, STRIKING THE BOOM & THE BUCKET OF THE DEMAG. THE FORCE SHOOK THE EQUIPMENT, JARRING HIM.
EMPLOYEE WAS LANCING A PIN ON A 992G AND TOOK HIS WELDING HELMET OFF AGAINST CO EMPLOYEE'S ADVICE AND HIT A GREASE SPOT AND WAS SPRAYED IN THE FACE AND NECK WITH HOT GREASE. EMPLOYEE DID LEAVE HIS SAFETY GLASSES ON.
EE EXITED THE SHOVEL CQAB TO THE WALKWAY WHEN THE SHOVEL BEGAN MOVING. HE TURNED AND WAS ENTERING THE CAB AGAIN WHEN THE SHOVEL TRAMMED OVER A ROCK CAUSING THE SHOVEL CAB DOOR TO SLAM SHUT ON HIS THUMB.
EE WAS PULLING CABLE FOR THE HIGHWALL MINER AS IT WAS TRAMMING TOWARD FACE. THE ADD CARS WERE TRAMMING OFF FRONT OF LAUNCH PAD. THE WHEELS DROPPED DOWN CAUSING THE CARS TO DROP AGAINST PAD. HI S FOOT GOT CAUGHT BETWEEN CAR AND PAD FRACTURINGHIS BIG TOE & FIRST TOE ON HIS RIGHT FOOT.
CLEANING TOP OF ENGINE WITH BLOW NOZZLE, HE BLEWDEBRIS INTO RIGHT EYE. HE WENT OUT OF TOWN THE FOLLOWING WEEK FOR SCHOOL; ALL WEEK EYE HURT & WAS IRRITATED. WENT TO DOCTOR WHEN HE RETURNED AND WAS DIAGNOSED WITH TRAMATIC CONJUNCTIVITIS TO RIGHT EYE.
AS EMPLOYEE WAS RIPPING BINDER THE DOZER HIS A SOFT SPOT CAUSING IT TO SPEED UP. AS THE DOZER WAS SPEEDING UP IT HIT A HARD SPOT AND CAME TO AN UBRUPT HALT, GIVING THE EMPLOYEE WHIPPLASH.
WINCH LINE BROKE ON DOZER THAT WAS HOLDING ANOTHER DOZER CLEARING BRUSH ON SLOPE. DOZER SLID DOWN HILL TURNING OVER APPROX. 4 TIMES, BLADE TO WINCH. DOZER ROLLED ON 60 DEGREE SLOPE ABOUT 400' . OPERATOR WAS IN BRUSH CAB WITH SEAT BELT ON.
